DALLAS COUNTY LABORATORY SERVICES CONTRACT HONEY CEDAR CREST TRAIL TAPP PROJECT No. 90162 ALPHA TESTING, INC. ORK ORDER NO.I This Agreement is entered into this 21st day of November , 201 7, to supplement the Indefinite De li very/Indefinite Quantity contract for laboratory services between Dallas County (the "County") and Alpha Testing, Inc. (hereinafter refe1Ted to as "Laboratory"). Whereas , pursuant to Dallas County Commissioners Court Order 2016-1 3 76, dated November I, 2016, and RFQ 2016-024-6561, Dallas County awarded an Indefinite Delivery/Indefinite Quantity (IDIQ) co ntract for laboratory services to Alpha Testing, Inc. Now therefore , this Work Order No. 1 is made by and entered into by the County and Laboratory fo r the mutual consideration stated herein. I. Purpose County and Laboratory both mutually agree to supplement said Contract in accordance with the tenn s of the existing Agreement, except as modified below. The purpose of this Work Order is to supplement the IDIQ Agreement to expand the scope of services to provide laboratory services on the Honey Springs- Cedar Crest Trail, TAPP 90 162 Project, as well as pro vide for the compensation accordingly. Thi s Work Order will be paid from Fund 196, Project 90 162. II. Supplemental Provisions A. A1ticle 3. "Compensation" of the IDIQ Agreement shall include the services specifically set fo rth in Attachment "A", which are attached hereto and incorporated herein by reference. The fee to be paid to the Lab oratory under this Work Order for all of the scope of services li sted in Attachment "A" shall not exceed Forty Three Thousand Five Hundred and Seven Do ll ars and no cents ($43,507.00). Fu1thennore, any additional fee is approved by the Commissioners Court in accordance with the te1ms of this Work Order and the IDIQ contract. Anticipated completion date is Augu st 2019. The Laboratory shall provide reports and invoices with attached documentation as required by Dallas County. Each report and invoice shall identify the services perfonned according to the scope of services. B. A1t icle 4. This Agreement is entered into this 21st day of November , 201 7, to supplement the Indefinite Delivery/Indefinite Quantity contract for laboratory services between Dallas County (the "County") and Alpha Testing, Inc. (hereinafter refe1Ted to as "Laboratory").

Whereas , pursuant to Dallas County Commissioners Court Order 2016-1 3 76, dated November I, 2016, and RFQ 2016-024-656 1, Dallas County awarded an Indefinite Delivery/Indefinite Quantity (IDIQ) contract for laboratory services to Alpha Testing, Inc.

Now therefore, this Work Order No. 1 is made by and entered into by the County and Laboratory for the mutual consideration stated herein.

I. Purpose

County and Laboratory both mutually agree to supplement said Contract in accordance with the tenns of the existing Agreement, except as modified below. The purpose of this Work Order is to supplement the IDIQ Agreement to expand the scope of services to provide laboratory services on the Honey Springs- Cedar Crest Trail, TAPP 90 162 Project, as well as provide for the compensation accordingly. This Work Order will be paid from Fund 196, Project 90 162.

II. Supplemental Provisions

Specifications (MWBE/MWBES) for the Honey Springs- Cedar Crest Trail as filed by the Laboratory with the County's MWBE office in response to RFQ No. 2016-024-6561. Said Laboratory's response is fully incorporated herein by reference for all purposes as if reproduced word for word.

III. Effect of Work Order

This Work Order shall not change or waive any contractual prov1s10ns, clauses, or conditions of the original Contract, as well as previous and/or subsequent Work Orders, unless otherwise provided for herein.

IN WITNESS WHEREOF, THE COUNTY OF DALLAS has caused this Contract to be signed by its County Judge, duly authorized to execute the same in its behalf by Co mt Order No. 2017-1520 , approved by the Commissioners Comt on November 21 , 20 lZ__, and signed by and through Alpha Testing, Inc. 's duly authorized representative, thereby binding the parties hereto, their successors, assigns and representatives for faithful and full perf01mance of the te1ms and provisions of this Contract.

ATTACHMENT A SCOPE OF SERVICES

It is our understanding that a new trail will be constructed alongside Corinth St, Illinois Ave, and Van Cleave Dr. in Dallas County. The trail extends approximately 5.4 miles and consists of 6" think reinforced concrete. Some intersections, driveways, and turnouts will also require concrete paving. We understand that sanitary sewers manholes wi ll be adjusted and storm drains will be installed at the proposed development. In this cost estimate, we are providing our estimate of the testing anticipated based on our understanding of the project information provided in the contract documents: civil plans dated: 7/28/2017; quantities provided.

SITE PREPARATION, FILLING, BACK FILLING (BASE BID) Based on our understanding of the information provided for this project, field density testing is required on the sub­grade and fill materials placed. It is estimated that an engineering technician could be required on site for about 80 hours to perform field depth verifications and 87 field density tests. In addition, the technician could obtain approximately 3 soil samples for laboratory proctor analysis.

UTILITIES (BASE BID) Based on our understanding of the information provided, fie ld density testing is required on the backfill material placed. We have estimated that approximately 370 linear feet of storm drains would be excavated and installed. In addition, about 13 linear feet of water lines, and an estimated 5 sanitary sewer manholes would be adjusted. We have estimated that our engineering technician could be required on site for approximately 24 hours to perform approximately 76 field density tests. In addition the technician could obtain approximately 2 soil samples for laboratory proctor analysis.

CONCRETE TESTING (BASE BID)

Based on information provided, it is our understanding concrete wil l be placed. It is estimated that an engineering technician could be required on site for an estimated 225 hours to mold approximately 358 test cyl inders and beams.

SITE PREPARATION, FILLING, BACK FILLING

Paving: 1 FDT each 3,000 cubic yards feet and 6 inch lift

Wall Backfill: 1 FDT each 3,000 cubic yards feet and 6 inch lift

All areas supporting slab foundations, flatwork, pavement or areas to receive new fill should be properly proof-rolled , compacted and tested.

UTILITIES

Trench Back-fill: 1 density test for every 300 linear feet each 6 inch lift.

Trench backfill for util ities should be properly placed and compacted in accordance with requirements of local City standards

CONCRETE TESTING

Paving: 1 set of 4 test cylinders every 100 cubic yards.

Utility Structures: 1 set of 4 test cylinders every 50 cubic yards.

Hardscapes or Sidewalks: 1 set of 4 test cylinders every 50 cubic yards.

Note: This cost estimate assumes sanitary sewer structures are pre-cast and will not require any testing and storm drain structures are cast- in place and will require testing. Should some of these items not require testing, this cost estimate could be revised upon request.

Based on information provided, it is our understanding concrete will be placed. It is estimated that an engineering technician could be required on site for an estimated 71 hours to mold approximately 114 test cylinders and beams.
