  DLIV is presented by the Cardiopulmonary Research Science & Technology Institute (CRSTI). Established in 1998, CRSTI was created to meet the growing demands for clinical research in the fields of cardio-vascular technologies, patient care and clinical outcomes. The organization aims to foster and promote medical research and education that benefits the health of the community; facilitate the physician’s ability to deliver state-of-the-art medical care; access new therapies; and improve quality clinical outcomes. In 2010, DLIV hosted 700 plus attendees representing 36 U.S. states and 33 countries. With a diverse domestic audience and strong international attraction, the meeting offers the best resources in the world for improvements in the field of valvular care. This year, we continue to build the strongest faculty possible and welcome a wide range of perspectives and specialties to the 2012 meeting.

In the current medical climate, our physicians agree that cardiologist-surgeon collaboration is not only beneficial but necessary for optimal patient care. Through an increased cardiology representation, DLV 2012 will confront issues facing both fields, as well as those that bring the fields together.

TERMS AND CONDITIONSExhibitors agree to abide by the con-ditions of the application and contract published in these regulations and by all conditions under which space at The Westin Galleria Dallas Hotel is leased by CRSTI. Exhibitors accept responsibility for informing all of their employees and agents of these condi-tions and agree that they will abide by them also. Your signature on your application and contract indicates you have accepted these terms and conditions. Your understanding and conformance is greatly appreciated.

CRSTI reserves the right to interpret these rules and regulations, as well as make final decisions based on the CRSTI’s sole discretion, on points these rules do not cover. In all inter-pretations of the rules and regulations, the decision of CRSTI is final.

Americans with Disabilities ActExhibitors are responsible for fully complying with all applicable provi-sions of the Americans with Disabili-ties Act.

Booth LocationApplications are processed in the order that they are received. Booth location selection is dependent on receipt of application and payment, booth size and sponsorship level.

Booth TrafficWe pledge to make all efforts to increase and sustain attendee traf-

fic to the Exhibit Hall. However, we cannot guarantee traffic nor can we be held responsible if companies do not achieve the leads and results expected.

CME Compliance and Industry SupportExhibitors must comply with the Ac-creditation Council for Continuing Medical Education (ACCME) stand-ards and guidelines for medical edu-cation, including those guidelines for commercial support. Commercial sup-port from industry does not influence educational content, faculty selection and/or product usage at DLV.

Distribution of Product InformationIn accordance with ACCME guide-lines, product information cannot be distributed at any scientific sessions. Distribution of printed materials by exhibitors and/or their agents is limited to the exhibitor’s booth area in the exhibit hall. Companies may not display or demonstrate products, solicit orders, or distribute advertising materials at any location (outside of the exhibit hall) other than in their as-signed booth space, including any of the contracted hotels. This guideline is strictly enforced.

Exhibitor HousingWestin Galleria Hotel rooms with negotiated rates of $159/night are available to DLV attendees. Exhibi-tors may secure their housing needs once their applications are received. A housing form will be sent to each exhibiting company once an Applica-tion for Booth Space is received.

Cancellation PolicyIn the event that DLV 2012 is abbrevi-ated or cancelled because of circum-stances beyond our control, CRSTI reserves the right to end the agree-

ment between CRSTI and the exhibi-tor. Should this occur, the exhibitor hereby agrees to waive any claim the company may hold against CRSTI.

Notification of cancellation prior to the event and barring the circumstanc-es explained above must be received by CRSTI in writing on company let-terhead. Any company canceling its agreement is subject to pay its 50% deposit if it cancels before August 31, 2012. If a company cancels after August 31, 2012 it will pay 100% of its pledged fees.
