2003: (ASHP) statement on the role of health-
system pharmacists in emergency preparedness
- general principles
1) On the basis of their education, training, experience,
and legal responsibilities, pharmacists should have a
key role in the planning and execution of
• pharmaceutical distribution and control
• and drug therapy management of patients during disasters.
2) The expertise of the pharmacist should be sought in
• a) developing guidelines
• (b) selecting pharmaceuticals and related supplies for national and regional
stockpiles and local emergency inventories in emergency-preparedness programs
• (c) ensuring proper packaging, storage, handling, labeling, and dispensing of
emergency supplies of pharmaceuticals
• (d) ensuring appropriate deployment of emergency supplies of pharmaceuticals,
• (e) ensuring appropriate education and counseling of individuals who receive
pharmaceuticals from an emergency supply in response to a disaster

2003: (ASHP) statement on the role of

preparedness - general principles
3) Pharmacists should be in a position to advise public health officials on
appropriate messages to convey to the public about the use of essential
pharmaceuticals in response to disasters, giving consideration to issues
such as adverse effects, contraindications, the effectiveness of alternative
pharmaceuticals, and the potential development of drug-resistant infectious
agents.
4) In the event of a disaster, pharmacists should be called on to collaborate
with physicians and other prescribers in managing the drug therapy of
individual victims.

Commitments made by ASHP
In support of the efforts of health-system pharmacists in
emergency preparedness and counterterrorism, ASHP will
1. Maintain an electronic communications network of hospital pharmacy
department directors that can be used to transmit urgent information related
to emergency preparedness and counterterrorism;
2. Disseminate promptly to ASHP members important new information related to
pharmacist involvement in emergency preparedness and counterterrorism;
3. Disseminate to ASHP members and others in the health care community
timely evidence-based information about pharmaceuticals used when
responding to disasters; and
4. Meet with government officials and others when necessary to clarify promptly
important issues that affect the involvement of health-system pharmacists in
emergency preparedness and counterterrorism.

Centers for Disease Control and Prevention (CDC) operate
the Strategic National Stockpile (SNS)
• The SNS is a supply of antibiotics, chemical antidotes,
antitoxins, life-support medications, intravenous (IV)
administration devices, airway maintenance supplies, and
medical and surgical supplies.
• The SNS is not an immediate source of medications, but
instead it supplements and re-stocks local and state public
health agencies with drugs and medical supplies when local
resources are depleted. The first allotment is a 12-hour Push
Package.
• Push Packages are caches of drugs and medical supplies
located strategically across the United States
• A single Push Package weighs approximately 50 tons and
contains almost 90 medications

CHEMPACK
• The time to delivery for SNS is not quick enough to manage
a chemical terror attack with a nerve agent such as
organophosphates. As a result, the SNS also operates a
voluntary program called CHEMPACK
• Content: atropine sulfate, pralidoxime chloride, and diazepam
(available as auto-injectors and vials) are stored within
CHEMPACK containers.

for assisting in managing the CHEMPACK expiration dates and
supply.
• A program allows the expiration date of these products to be
extended, based on criteria set forth by US Food and Drug
Administration (FDA) stability studies, as long as the contents
remain unopened and under specific environmental conditions

Stand van zaken noodprocedures in
ziekenhuisapotheken in de USA • An electronic cross-sectional survey : To assess the preparedness of hospital
pharmacies in New Jersey to provide pharmaceutical services in mass casualty
scenarios
• Results
• Out of 60 invitations to participate, 18 surveys (30%) were completed.
• Seventeen sites (94.4%) reported the existence of an institutional disaster preparedness
protocol;
• 10 (55.5%) indicated that there is a specific plan for the pharmacy department.
• Most respondents (10, 55.5%) were unsure whether their hospitals had an adequate supply of
analgesics, rapid sequence intubation agents, vasopressors, antiemetics, respiratory
medications, ophthalmics, oral antimicrobials, and chemical-weapon-specific antidotes.
• Conclusie
• This survey demonstrates a lack of general consensus regarding hospital pharmacy
preparedness for mass casualty scenarios despite individualized institutional protocols for
disaster preparedness. Standardized recommendations from government and/or
professional pharmacy organizations should be developed to guide the
preparation of hospital pharmacy departments for mass casualty
scenarios.

• The hospital develops, maintains, and tests an emergency
management program to respond to emergencies and natural or other
disasters that have the potential of occurring within the community. The program provides processes for
• a) determining the type, likelihood, and consequences of hazards, threats, and events;
• b) determining the structural integrity of existing patient care environments and how they would
• perform in the event of a disaster;
• c) determining the hospital’s role in such events;
• d) determining communication strategies for events;
• e) managing resources during events, including alternative sources;
• f ) managing clinical activities during an event, including alternative care sites;
• g) identifying and assigning staff roles and responsibilities during an event; (Also see FMS.11.1, ME 4)
• h) managing emergencies when personal responsibilities of staff conflict with the hospital’s
responsibility for providing patient care.
The disaster preparedness program is tested by
• • an annual test of the full program internally or as part of a communitywide test; or
• • testing of critical elements c) through h) of the program during the year.

Kaliumjodide:
• in elke officina minstens 500 g in bulk en / of onder farmaceutische
vorm waarvan zij het enige actief bestanddeel uitmaken
• MB 10/12/1987 voor ziekenhuisapotheken, KB 21/01/2009 bijlage 2
voor het publiek toegankelijke officina’s
• KI-tabletten in ZH-apotheken die binnen een straal van 20 km rond
een kerncentrale gelegen zijn. Het aantal tabletten is in functie van
de grootte van het ziekenhuis (aantal bedden, werknemers,…)
• verspreiding KI-tabletten voor inwoners gebeurt via voor publiek
opengestelde officina’s

Atropine:
• In elke ziekenhuisapotheek 2g atropine of zouten waarvan 20
inj.amp. van 1 mg atropinesulfaat (cf. MB 10/12/1987)
• In elke voor het publiek toegankelijke apotheek minstens 2g
waarvan 20mg injecteerbaar (cf. KB 21/01/2009 bijlage 2)
• + eventueel atropine ampoulen volgens contract met de Belgische
staat in het kader van bioterrorisme (bepaalde ziekenhuizen). Het
aantal ampoulen en de concentratie (0,25/0,50/1mg/ml) afhankelijk
van het contract dat de ziekenhuisdirectie ondertekende met de
FOD Volksgezondheid.

Noodplan in de ziekenhuizen
• In het KB van 17 oktober 1991 betreffende de normen
waaraan ziekenhuizen en hun diensten moeten voldoen,
werd bepaald dat ieder ziekenhuis over een actieplan moet
beschikken om te reageren op een groot ongeval binnen óf
buiten het ziekenhuis.
• Het KB verplicht dat elk ziekenhuis over een permanent
comité moet beschikken, onder leiding van de
hoofdgeneesheer, dat instaat voor het opstellen, actualiseren
en goedkeuren van het noodplan.

Conclusie
• Ziekenhuisapotheek speelt een belangrijke rol in tijden
van interne en externe dreiging
• Ziekenhuisapothekers moet nauw betrokken zijn bij
ontwikkeling noodplannen en interne plannen periodiek
evalueren
• Samenwerking tussen ziekenhuizen is noodzakelijk o.a.
gebruik capaciteit andere ziekenhuizen, …

Gzondheidsraad zal hopelijk vele vragen beantwoorden
