MEDICATION
Give proper instructions on how to take home meds.
o Teach mother and/or significant others on indications of specific
medications especially dosage, frequency, precautions and side
effects.
o Teach about the drugs and food interactions.
o Instruct the mother and/or significant others to report any
unwanted adverse reaction to a specific medication.
TREATMENT
Colostomy Care/Ileostomy Care
o Assess and inspect stoma for color, protrusion and any signs of
infection.
o Make sure that the stoma bag is always intact.
o Monitor stool consistency and color.
o Irrigate the colostomy with saline solution or warm water.
o Proper instruction on positioning the patient in irrigating the
colostomy.
o Educate in signs and symptoms that will alert for need to seek
medial help.
HYGIENE
Bathe the baby everyday after colostomy care.
Maintain clean environment to prevent further complication and infection.
Provide relaxing and comfortable environment to the baby.
Proper sterilization of baby’s utensils such as bottles, rubber nipple, and
pacifier.
Properly wash and iron baby’s clothing to prevent allergies and skin
irritation.
OPD FOLLOW-UPS
Instruct mother to return for follow up check-ups for the baby.
Inform mother to religiously follow the exact date of follow up check-ups.
DIET
Health teaching on breastfeeding.
o Teach mother on proper breastfeeding and its benefits on the baby.
o Educate mother to maintain strict aspiration precaution.
o Proper positioning of the baby during and after breastfeeding.
o Inform mother that breastfeeding is important until 2 years of age
and that food supplement can be given only after 6 months of
breastfeeding.
