Script: The next few questions are voluntary and confidential and are meant to ensure everyone gets the best care regardless of background.
Race/Ethnic
Script: Which of the following best describes your race or ethnicity: American Indian, Asian, Black, Hispanic, Middle Eastern, White or Multiple Race/Ethnicity? If patient selects a race, read the CCC options available for that race.
