During the completion of a self-exclusion form, you may choose from one of the following irrevocable exclusion terms:

• One year (If a one-year term is chosen, access to licensed cardrooms and gambling activities are automatically restored one year after the form is completed.)

• Five years (If a five-year term is chosen, access to licensed cardrooms and gambling activities are automatically restored five years after the form is completed.)

• Lifetime (Remember, if a lifetime term is chosen, it is completely irrevocable and access to licensed cardrooms and gambling activites will never be reinstated.)

What is the Self-Exclusion Program? What are the Exclusion Terms? What Happens After the Form is Completed?The California Department of Justice’s Bureau

of Gambling Control maintains a Self-Exclusion Program. The program helps people who have a gambling problem stay out of California licensed cardrooms. You can volunteer to self-exclude yourself from entering these cardrooms by completing a self-exclusion form for either one year, five years, or for life.

Follow these easy steps to participate:(Steps 1-3 may take anywhere from 10 to 15 minutes.)

Step 1: Complete a self-exclusion form.

Step 2: Have the form witnessed by a licensed cardroom key employee or have it notarized.

Step 3: If notarized, send us the form immediately; if witnessed by a key employee, give the form to the key employee for submission.

Step 4: Don’t enter any licensed cardrooms in California, and please seek treatment from Gamblers Anonymous or inpatient/outpatient counseling centers.

Self-exclusion forms are available at all licensed cardrooms and can be downloaded from our website at www.ag.ca.gov/gambling/responsible.php or from the California Gambling Control Commission’s website at www.cgcc.ca.gov/forms by clicking “Responsible Gambling.”

Before the expiration of a one-year or five-year exclusion term, we send a letter to all licensed cardrooms and the exclusion participant to notify them of the expiration.

If an exclusion participant visits a licensed cardroom, the cardroom is required to remove the participant from the premises and notify us of the incident.

If an exclusion participant is found playing in a licensed cardroom, their money is confiscated and sent to the Department of Alcohol and Drug Programs, Office of Problem Gambling, for problem gambling prevention and treatment services.
