Introduction
ABIM’s Maintenance of Certification (MOC)ABIM has implemented changes to their MOC program. The new requirements apply to all certified physicians, including FITs and those certified prior to 1990 (grandparents). Physicians are now required to participate in at least one MOC activity every two years and earn 100 points every five years. Of those 100 points, 20 must be earned in Self-Evaluation of Practice Assessment (commonly referred to as MOC Part IV).
In an effort to help you to determine your MOC status, ACC has developed an MOC Algorithm (page 5), which identifies the requirements for each of the four parts of MOC, as well as a complementary Decision Tree (page 6), which addresses Self-Evaluation of Practice Assessment. Using the Decision Tree, you can determine how to satisfy ABIM’s MOC Part IV requirements.
ABIM’s Completed Project PIMOne method of earning MOC Part IV points is by completing ABIM’s Completed Project Performance Improvement Module (PIM). This module can be completed by cardiologists who have completed a Quality-Improvement (QI) project in their practice within the past two years, using nationally recognized quality measures.
The requirements are as follows:
• Projects must have been completed within the past 24 months
• Projects must have used at least 3 performance measures with data from 25 or more patients
• Cannot be used by physicians during residency

How to Access the Completed Project PIM
1. Log into your ABIM profile by clicking “Physician Login” in the top right-hand corner of the ABIM webpage (abim.org).
2. Hover over the blue “maintain & renew” tab and select “Requirements & How to Earn Points” from the drop-down menu.
3. From this page, click “Practice Assessment” on the left-hand side of the screen. The menu will expand to show additional options. (Please note: Practice Assessment is often referred to as “Part IV” of Maintenance of Certification requirements, however ABIM does not use this language.)
4. Select “Product Info, Demos & Ordering”, this will take you to the ABIM’s list of available products, including the Completed Project PIM.

5. Scroll down until you see “Completed Project PIM” under the list of ABIM Products. Click the link for the “Completed Project PIM”. Information will populate beneath the link. (Please note: this will not direct you to a new screen.)
6. Underneath the “Completed Project PIM” heading you will see several useful links. In the top right-hand corner there is a link to a tutorial. We highly recommended that you view the tutorial prior to beginning the PIM.
You will also see a link to the “ABIM Measures Library”. This will be used to select your data measures. By clicking on this link you will be taken to a tutorial on measure selection.
Once you are ready to order the PIM, click “Order as ABIM Diplomate”.

Completed Project PIM InstructionsPart A: Orientation7. There are four parts to the Completed Project PIM:

To begin the module, click “Part A: Orientation”.
8. The first step in “Part A” is a brief overview of the module. There are two screens of information within the “Overview” tab. Click “Continue” to advance to the second screen.

9. The next page explains your involvement as a physician. Check the box at the bottom of the screen to indicate that you have read and understood the physician involvement requirements. Click “Continue” to advance to the next tab in Part A.
10. In the next tab in “Part A”, you are asked to read and accept important disclaimers. Once you have read the information, check the box at the bottom of the screen and select “Continue” to advance to the next screen.
11. On this page, you will read the disclosures, check the box at the bottom of the screen and select “Continue” to advance to the last tab within the “Orientation”.
12. The last tab within the “Orientation” asks when you completed your QI Activity. Keep in mind that your activity must have been completed within the past 24 months in order to complete this module for CME/MOC credit. Click “Continue” to advance to “Part B” of the module.

Part B: Measures & Data
13. In the first tab, “Tell Us About Your Data”, you will describe how you conducted your needs assessment for your QI project, and how you decided where to focus your improvement efforts. There are three screens of information within this tab, click “Continue” to advance to the second screen.
14. You are asked to provide the dates covered by your baseline and remeasurement data. Enter the appropriate dates and then click “Continue” to advance to the last screen within this tab.
15. In the last screen within the “Tell Us About Your Data” tab you will select the box indicating where your baseline data comes from. Click “Continue” to advance to the next tab.
16. In this tab, you will provide information about your care setting. Select the appropriate setting and click “Continue” to advance to the next tab.
17. It is now time to use the “Measures Library” to select your measures and enter your baseline data. Check the box circled below in orange and click “Continue” to search for measures.

“Measure Sets Groupings” are listed in the “Measures Library” tutorial at abim.org/ml and also in Appendix I online at CardioSource.org/PartIV. We recommend that you review this tutorial which will familiarize you with the process for selecting your measures for this PIM.

18. In this step, you will search for a condition within the “Measures Library”, then a measure set.

19. If all three of your measures are in the same measures set in the “Measures Library”, select them by checking the boxes to the left of each measure and continue on to Step 20. (Please note: you must have data from a minimum of 25 patients for each measure.)
If you do not see three measures you used in your project in the same set in the library, or you used one or more measures which do not appear in the “Measures Library” at all, you must request approval from ABIM to use them. In the bottom left-hand corner of the screen you will see a link to “Submit your measures for approval”. Clicking on this link will take you to four forms. Select one of these forms to complete and submit to ABIM (Examples of completed forms are available in Appendices II – V at CardioSource.org/PartIV).
20. At the “Enter Baseline Performance Data” tab, your chosen measures will appear on the left of the screen. Record the number of patients in your sample as well as your baseline performance measure values in the columns on the right of the screen.

Part C: Intervention
21. In the first tab of “Part C”, you will be asked to choose the single measure that you targeted for improvement and for which you will provide remeasurement data. There are four screens of information within this tab, click “Continue” to advance to the next screen.
22. Select whether or not you set a numeric performance goal for your improvement activity, and click “Continue” to advance to screen three of four.
23. You will now be asked to enter in your goal, if you set one, for improvement on your targeted measure. Click “Continue” to move on to the last screen in this tab.
24. You will now be asked to enter in the remeasurement data for your targeted measure. Please note that you are not required to meet or exceed your goal in order to receive MOC or CME credit. Click “Continue” to move on to the next tab, “Describe Your Project”.
25. Write a brief problem statement related to your selected measure. Click “Continue” to advance to the last screen in this tab.
26. Identify the nature of your improvement activity and click “Continue” to advance to the next tab.
27. In this tab, you will provide information about your care setting. Select your care setting and click “Continue” to move to the tab in “Part C”.
28. Select the members of your multi-disciplinary team who were included in your improvement team. Click “Continue” to move to the last tab in “Part C”. Make sure to scroll down and answer all questions.

29. In this tab you are asked to describe your intervention.
a. Select whether or not you used a structured process (e.g. Root Cause Analysis using a Pareto Chart, Fishbone Diagram, 5 Whys) in your QI project. Click “Continue” to advance.
b. Describe your root cause analysis in the space provided. Click “Continue” to advance.
c. Describe up to three root causes related to your selected measure that you identified. Click “Continue” to advance.
d. Describe the interventions you used to improve your targeted measure. Click “Continue” to advance.
e. Answer the questions about the practice systems relevant to your intervention. Make sure to scroll down and answer all questions. Click “Continue” to advance to the sixth of seven screens in this tab.
f. Write a brief description of the intervention that your team implemented to improve your targeted measure. Click “Continue” to advance.
g. Select any approaches that you used in your intervention. Click “Continue” to advance to “Part D” of the PIM.

Part D: Completion & Credits
30. In “Part D” of the module — “Completion & Credits” — you are asked to reflect on your QI Project. Once you make your selection from the options, click “Continue” to advance to the next tab.
31. Answer the question regarding your future quality-improvement projects and briefly describe your plans. Click “Continue” to advance to the last tab in the module.
32. Complete a brief survey about your QI Project and the module in order to earn MOC and/or CME credits. Make sure to scroll down and answer all questions.
33. To claim MOC points and CME credit for this module, select “I want to claim CME credit for this PIM.” While claiming MOC points is not included as an option on the screen, by selecting the option to claim CME credit you will earn 20 Part IV MOC points. Click “Continue” for confirmation and to complete the module.
34. Be sure to log in to the ABIM home page to ensure that your MOC points and CME credits have appeared.
35. Congratulations! You have now finished the Completed Project PIM.
