I. ACKNOWLEDGMENT
I would like to thank God by giving me strength all the time
and the wisdom make me egger to finish my training.
I cant reach all this things without him, every time I failed I lost and Iwant to stop in my OJT I just pray because I know God gives me,
tomorrow is another day.
To my manager in MED101 Diagnostic Center, Maam
Michaela Ayapana. Thank you for the chance to be part in your
clinic, because your clinic is the path to finish my practicum
requirement. Within 300 hours I experience all the things that I need
and also how to interact to other people, the patients and staff of
your clinic.
Especially I would like to thank my parent to support me in my
expenses, projects and activities in school everyday.
II. INTRODUCTION
I was on Training at MED101 Diagnostic Center. I was disap-pointed because the clinic is small and not familiar like the other
company but it has a Complete Laboratory. In our clinic I was as-
signed in some department like Ultrasound, ECG, X-ray, Assisting the
patients, typing the expenses receipt of clinic, documents, Medical
Examination of urinalysis, CBC, Fecalysis and other specimen from
Patients. getting the information and vital sign. Filing their records,
Printing and Scanning, Writing Charges of the patients fees on the
Log Book. Logging and releasing the Lab results everyday. Cooper-
ation with others on the job.

IV. OBSERVATIONS IN THE COMPANY
What I have observed at MED101 Diagnostic Center I like their facilities,
By department because it is clean and fresh, it has a complete equipments
and complete laboratory machines.
The strength and challenges of this company is that when I have afault or mistake to what Im doing it give challenges to me and they give a
new knowledge that give me strength.
I think the best practice of this company is to how I use the machine of
ultrasound, ECG, X-ray and other equipment of the laboratory. Assisting the
patients and filling the records Examination.
My over-all observation in MED101 Diagnostic Center is when they treatthem and assist the patients they give a good services and cooperation on
what they have to do.
V. PERIODIC REPORTS (daily, weekly, monthly)
What we do daily are the following:
Encoding the expenses receipt of the clinic.
Filling the records of the patients.
Releasing the result of the medical examination of patients.

Printing the result and scanning the documents
Assisting the patients.
What we do monthly are the following:
Releasing the result of the medical examination of patients.
During my whole training in MED101 Diagnostic Center,
I think the problem/s that I have encountered is when the patient is rugged
to us because the result of their examination is not release on that time so
we need to talk them calmly, and some of the attitude of employee are
plastic thats why Im became angry and irritate of them.

VII. SELF-ASSESSMENT
A. Skills and current technology learned/enforced:
When I was on my training in MED101 Diagnostic Center I have
learned a lot, I learned the other things like ECG, I learned how to set-up
and use the machine, I learned how to indicate the things into the body of
the patients to make the result of examinations become good. I learnedhow to get the blood pressure, I make things that we use in examinations.
Its good for me when the others things that make me scared on that time I
getting stronger because of the weaknesses in my self.
B. Equipment, tools, testing apparatus handled (software's used included)
Many things that I try inside the laboratory and some gadgets ,
Equipments that we use in our medical examination like microscope Iknow how to use it and how to indicate the specimen from the patient. Im
the one who get the sample of urine and etc. and the result of the ultra-
sound, I know how to set-up the x-ray machine, how to use it and how to
develop the film image into the darkroom I will be the one who always
encode in laptop using excel Microsoft etc. I scanning the documents and
printing the result of examination.
C. Strong points versus weak points
My strong points is to makes my work easily. I always pay attention to
the instruction of my supervisor.
My weakness points is my mistake like when I'm encoding sometimes I
have a wrong putting the name of the patient.

VII. SELF-ASSESSMENT
D. Best experiences on the job:
I experience to try the ECG machine to the patient and also I know
how to put the things on the body of patient. In ECG examination the pa-
tient doesn't have no accessories coin, belt, and other things made by
metal because it has a tendency that will detect the machine, it make theresult of the examination is good.
also have know how to x-ray and develop the image on film to de-
veloping the film inside the darkroom.

Faults committed (its causes and suggestion given by the supervisor)
You need to focus to what you do, listen very well to the instruction.you must done all your work on time. Always cooperate in staff of the clin-
ic. You have a 3 chance to ask again, if you don't understand the task
that gave to you.
