Introduction
Wounds propose a substantial health hazard to patients, providers, and the global health economy, affecting millions
of patients and costing billions dollars annually. It has been observed that wounds have anecdotal effects on the
life quality of the patients or those affected, their caregivers and families. However, in the past two decades,
numerous changes have occurred in the science of wound healing. There has been pronounced advancements in in
the development of wound dressings that possess rapid healing properties compared to traditional wound dressing
products such as gauze, cotton and bandages. At present, owing to the rigorous research and development activities
along with positive outcomes of clinical trials, players operating in the wound market have been successful in
developing advanced and cost effective wound products such as foams, hydrocolloids, hydrofiber, alginates,
Negative pressure wound therapy (NPWT) devices, and biologics among others that provide better wound healing
care and faster recovery.

2 Introduction to Global Advance Wound Care Technologies
2.1 Key Industrial Developments
Wound management is an orchestra of extremely integrated cellular and molecular events of cell migration,
proliferation, extracellular matrix deposition and re-modelling. Certain metabolic and physiochemical conditions can
modify this normal sequence of events so that healing is compromised or hampered, resulting in complex, non-
healing wounds. Chronic wound conditions such as pressure ulcer and diabetic foot ulcers readily become non-
healing, and very challenging to manage. The costs associated with the healing of an ulcer has been noted to be as
high as $70,000; however, these approximations do not comprise the deleterious effects on the patient’s quality of
life because of impaired mobility and significant loss of productivity. Although wound care challenges remain,
advancements over the past decade have improved the clinician’s armamentarium in the medical and surgical
management of wounds. The incorporation of technological advances with the comprehensive understanding of the
complex molecular mechanisms of wound healing has led to the development of various advanced wound healing
modalities, such as synthetic skin and tissue substitutes, Negative Pressure Wound Therapy (NPWT), and Hyperbaric
Oxygen Therapy (HBOT).

2.1.4 Industry Trends
The evolution of traditional wound care to advanced wound care technologies such as NPWT, biologics, and advanced
dressings has been steady. The use of simple bandages and dressings gave way to hydrofibers, alginates, collagen,
hydrogels and silver containing dressings. Replacement of traditional wound management regimens with advanced
wound care products is mainly due to their efficiency in treating wounds by preventing the growth of excess exudate,
while preserving the precise level of moisture to stimulate healing with minimum scarring. These products are
progressively being accepted by acute care centers and emergency departments to reduce the length of patient
stay and reduce the overall cost of wound management. Growing awareness of precisely formulated products and
swift technological innovations are also contributing towards market growth. Moreover, the escalating base of
geriatric population and increasing incidence of chronic conditions such as diabetes that result in chronic wounds
which need advanced level of treatment, are also spurring opportunities for the market.
Figure 2.3
Industry Trends in Advanced Wound care Management
BIS Research Analysis
Another important trend in the wound care market is the development of a combination of advanced dressings
which are prepared using several other available wound dressings in different combinations. Currently, biological
agents in combination with antimicrobials dressings are getting traction. These combination products have gained
extensive usage due to their ability to safeguard the body against microorganisms and absorb extra wound exudate.
Given their better efficiency than advanced dressings such as hydrocolloids in handling highly oozing wounds,
combination dressings are emerging to be one of the bright spots in the moist wound dressings market.

overcome market entry, and regulatory hurdles, they are under enormous pressure to ramp up sales in order to
capture market share and generate an adequate return on investment on their huge investments. As the U.S.
advanced wound care moves towards episode-based payment models, many companies will likely take a more
dynamic part in the risk sharing for healing outcomes, specially related to chronic wounds. The future advanced
wound products most likely to be acquired/licensed by the major companies will need to be extremely cost effective
while influencing positive clinical outcomes. Precisely, the advanced wound care domain will experience significant
activity in the wound dressing, single-use NPWT and wound care biologics segments.
Figure 4.3
Advanced Wound Care Market Share Analysis, 2016
BIS Research Analysis
Recently, several new wound care biologics have been launched in the U.S. market. Many of these biologics have
been driven by recent amendments in the fee-for-service reimbursement model for regenerative medicines including
biologics. Moreover, the episode-based reimbursement models is also expected to boost the wound care biologics
market. The wound care biologics will continue to play a vital role in wound healing, but the emphasis will shift
towards those biologics with cost saving characteristics (not just retail price, but storage ease/cost,
preparation/application time, etc. as well).

Furthermore, the company’s Avance Solo hybrid single-use NPWT device is clearly intended at seizing market share
from Acelity and Smith & Nephew. However, it is seems to be a challenging job as both of these competitors have
large GPO accounts which is very difficult to make an entry into. Molnlycke recent acquisition of Sundance Solutions’
offloading products may allow it to protect the infringement of several foam competitors such as Smith & Nephew.
The company’s recent acquisitions are both defending against new launches in the market, besides taking an aim
at pressure ulcer treatment market.

The global advanced wound care market, by products, was valued to be $XX million in 2015 and is estimated to
reach a market value of $XX million by 2022 at a CAGR of XX% from 2016 to 2022. Despite the tight competition
in the advanced wound care market and the rising demand by the government to reduce spending on healthcare,
the future growth is expected to be driven by favorable demographic factors such as growing aging population and
rising incidences of chronic wounds. Growth is particularly expected in Brazil, China, India, and other emerging
countries where the market penetration of advanced wound care products, and especially biologics, is just a fraction
of the market penetration in the U.S.
The advanced wound care biologics market is currently witnessing highest growth so far, owing to the highly
competitive environment, favorable reimbursement scenario, and growing demand for bioengineered tissue grafts.
This segment is also characterized by more levels of innovation than any other segment of the advanced wound
care market, though this is the smallest segment and market entry is very restricted owing to stringent government
regulations.

NPWT system has a life of seven days and the dressing kit also has a wear time of seven days, depending on the
type of wound exudation. Single-use NPWT systems are designed to deliver suction pressure of 80 to 125 mmHg.
However, these systems do not contain bulky canisters which make the device more accessible and simplified for
the patients.
In early 2014, the Centers for Medicare and Medicaid Services (CMS) revised the reimbursement rules for single
use NPWT systems. According to the new rules, CMS increased the mean reimbursement amount for single use
NPWT systems from $XX to $XX. This increase in reimbursement applies to the HCPCS G-codes (G0456 and G0457)
and Ambulatory Payment Classification 0016. These new rules have been effective from January 1, 2014.

The skin/dermal substitutes encompasses an array of biological products including biological skin substitutes
(allograft and xenograft), amniotic and placenta tissues, synthetic skin and bioengineered skin substitutes. Biological
skin substitutes act as a natural skin and have the benefits of being comparatively abundant in supply chain and
not very expensive. Although the most of the synthetic or bioengineered skin substitutes can be synthesized and
modified for any precise requirement with improved control over alignment and composition, the biological skin
substitutes (allograft and xenograft) have a more intact and native extracellular matrix structure which allow the
development of a more natural new dermis. They also allow exceptional re-epithelialisation features due to the
presence of a basement membrane. However, natural constructs can exhibit problems with slow vascularisation of
the material. The most extensively used skin/dermal substitute globally are bioengineered skin, synthetic
substitutes, porcine skin xenograft and amniotic membrane.

The skin/dermal substitute market was valued at $XX million in 2015 and is estimated to reach $XX million by 2022
at a CAGR of XX% from 2016 to 2022. The skin and dermal substitutes have caused a paradigm shift in wound care
therapies. Scientific advances and discoveries have allowed the shift from traditional methods that treat the
symptoms of the disease to a focus on improving the primary causes of the disease. Some of the key factors driving
skin and dermal substitute market include the increasing ageing population which is putting a strain on the global
healthcare industry; increase in chronic diseases like neurodegenerative disorders, diabetes, and obesity; change
in reimbursement policies; and the increasing private insurance coverage, which is willing to pay for these new
treatments.

6 Global Advanced Wound Care Market by Wound Type, 2016-2022
6.1 Overview
Wound management is a colossal market that accounts for a significant share of medical expenses in any country.
Owing to its broad extent, many healthcare experts considers it to be an umbrella category that includes any
therapeutic technique, product services, from basic cotton gauze to complex collagen-based dressings, that helps
one recover from wounds on the body. However, in general, wound care is largely divided into two distinct types
namely; traditional and advanced. Each category has different products developed to improve the healing process
precise to certain types of wounds.
Advanced wound care products for the wound management are usually recommended by physicians after a visit to
the clinic or hospital. This class generally includes film and foam dressings, hydrogels, alginates and hydrocolloids.
Such products are mostly focused on keeping the wound moist to boost healing process, following the philosophy
that a moist microenvironment promotes the natural healing process (human cells require water to live and grow)
without occluding the wound.
The science of wound care is evolving swiftly, predominantly as a result of new healing approaches such as skin
substitutes, growth factors, advanced dressing and stem cell therapy. Over the past two decades, numerous
advanced wound care products have been established for their capability to accelerate the healing of acute and
chronic wounds. A chronic wound is defined as one that is physio-chemically compromised due to a disturbance of
the wound healing cycle as a consequence of diminished angiogenesis, cellular intervention, innervation, among
other causes. The most common chronic wounds comprise non-healing wounds, contaminated or infected surgical
wounds, pressure ulcers, diabetic foot ulcers, venous leg ulcers, and ischemic ulcers. Unlike chronic wounds, acute
wound is an injury to the skin or deeper tissues that occur suddenly rather than over a course of time. It heals at
an anticipated and predicted rate according to the usual wound healing process. Acute wounds can occur anywhere
on the body and range from small superficial injuries to deep tissue cuts damaging blood vessels, nerves, and
muscles.
The advanced wound care market earned revenues of $ XX million in 2015 and estimates this to reach $XX million
in 2022 owing to increasing awareness among physicians and other healthcare experts on the benefits of advanced
wound care products that allow quicker healing than traditional products. In addition, a burgeoning population,
greater awareness among patients, rapid rise in lifestyle diseases and the related risk of chronic wounds, and higher
buying power will further push the market.

6.2 Chronic Wounds
Chronic wounds result in substantial functional deficiency, the decline in quality of life, and huge financial burden
for patients and the health care system. Yet the underline clinical profile of chronic wounds hasn’t been well
established and explored. This interrupt precise assessment of the global wound care burden and so the information
that could improve the distribution of precious healthcare resources towards prevention and management of this
global problem.
Chronic wounds usually take months or years to fully recover, or may never improve. Chronic wounds adversely
disturb the patient quality of life and are a prominent cause of escalating healthcare expenditure. Spending on
chronic wound care is significant for healthcare providers around the globe. In the U.K for example the total annual
cost of wound management has been reported to be as high as $XX billion. In the U.S. wound care costs the
healthcare system more than $XX billion each year including more than $XX billion on advanced wound care
products.
One of the cardinal drivers for chronic wound care is the growing geriatric population across the globe. Chronic
diseases, such as metabolic syndromes, cardiovascular conditions, and autoimmune syndromes influence the
healing process as a result of their effect on a number of cellular functions. Ailments that cause the most significant
wound problems include diabetes, arteriosclerosis, chronic obstructive pulmonary disease (COPD), angiogenesis,
peripheral vascular disease (PVD), cardiovascular disorders, and any conditions leading to hypotension,
hypovolemia, edema, and anaemia.

7 Global Advanced Wound Care Market by Geography, 2016-2022
7.1 Overview
The chapter focuses on over four major regions - North America, Europe, Asia Pacific and the RoW with respect to
the usage of advanced products such as a wound care therapy. The wound dressings market is highly diverse and
largely dependent upon the country and its geographic location with respect to the region it lies in. The diversity of
the wound dressings market is largely due to the varied infrastructure, health care systems, social acceptability
(culture, traditions, etc.), affordability and availability. Wounds are among the most common ailments besides
cold/fever for a physician visit. In most parts of the world, this has been addressed using traditional wound
dressings. However, rise in bedridden geriatric population has triggered the demand for advanced wound care
globally. The North American market is expected to retain its dominant position globally owing to a high incidence
rate of diabetic foot ulcer cases. Moreover, other regions such as South-East Asia are also expected to be a lucrative
market due to the high incidence of severe burn related wounds in such regions.

The global advanced wound care market is forecast to grow, from $XX million in 2015 to $XX million by 2022, at a
CAGR of XX%. As the global economic power shifts away from established developed economies in North America,
Europe and Asia (Japan), the advanced wound care market is expected to witness immense growth in developing
countries such as China and India. China has already overtaken developed European wound care markets including
Italy in 2015 to become the 6th largest single market for advanced wound care product. However, the recent
experience in economic slowdown has reemphasised that a comparatively rapid growth is not anticipated for all
developing countries, as directed by current economic problems in Russia and Brazil. It requires persistent and
operational investment in healthcare infrastructure and improving political, economic, and legal healthcare
framework. It also requires openness to the free flow of advanced technology and innovation that are key drivers
of the wound care market growth.

As the advanced wound care market enters 2017, the U.S. remains the world leader by market size, with total
revenue of around $XX million, approximately one-third of the $XX million global pie. As of October 2014, U.S.
companies held two of the top five standings for wound care companies with the most revenue and represented
more than XX% of the global market. An aging population, growing prevalence of chronic wounds, and increased
accessibility of healthcare as a result of the Affordable Care Act (ACA) will continue to keep the U.S. well positioned
in the global advanced wound care market. The U.S. wound modalities devices export grew at a CAGR of XX% from
2010 to 2015 and are anticipated to grow at an extensive rate in the future.
Figure 7.5

However, the U.S. advanced wound care market also faces multidimensional challenges such as stringent regulatory
and economical requirements that are hampering its growth. In the last 3 years, the energetic pace of the growth
of the U.S. advanced wound care market has slackened. As a consequence, the U.S. advanced wound care market
is expected to expand by only XX% to XX%, which is considerably down from its XX% to XX% growth rate, a decade
ago. To improve its forecasts, the industry will need to exploit on its strong point, overcome its weaknesses, grab
opportunities for growth, and address threats that could obstruct its progress.

In the Asia Pacific region, advanced wound dressing products are gaining importance over traditional products owing
to the trend shift toward home care and the growing need to discharge patients early from hospitals. In addition,
the wound dressings market in China is undergoing rapid development. Advanced wound dressing products are
imperative for treatment of chronic wounds such as diabetes, pressure ulcer, and venous leg ulcers. According to
the Chinese Medical Association Diabetes Society in 2010, there were approximately 95 million cases of diabetes
mellitus in China and the number is expected to reach up to 117 million by 2020. This rise in the diabetic population
would further lead to diabetic ulcers, boosting the demand for advanced wound dressing products such as alginates,
hydrogels and hydrocolloids. Owing to robust support from the Chinese government, international firms such as
Smith & Nephew plc, Kinetic Concepts, Inc. and Johnson & Johnson Inc. have established their offices in China.
Also, rising disposable incomes is expected to pump the growth of the advanced wound dressings market in the
near future.

Some of the primary factors contributing to the growth of the wound care market in this region are increased
incidence of chronic wounds such as venous ulcers, diabetic foot ulcers and severe burn injuries. In addition, factors
such as rising elderly population and improved economic growth are expected to drive the growth of the NPWT
market in the Asia Pacific region. However, other factors such as disinclination in commercialization of advanced
technologies and increased demand for elective surgeries are hampering the overall growth of the market.
