1. Please describe every grant the agency is, or is considering, applying for
this fiscal year.
Response:
Grants the Department has applied for:
FEMA/DHS - Assistance to Firefighters / Fire Prevention Grant for Smoke
Detectors for our Community Smoke Detector Programs.
Grants the Department is considering to apply for:
A DHS State Homeland Security Grant for an Information Sharing Officer that
would fund a position for two years to develop policy, plans and procedures to
move intelligence and information from the Washington Regional Threat and
Analysis Center to the EMT and Firefighter at the street level in order to
improve their preparedness and ability to recognize threats and hazards.
FEMS / DHS Assistance to Firefighters Grant for improved Lifepacks and
AEDs.
Grants the Department will benefit from, but is not the lead agency:
A DHS Homeland Security Grant for an Automated TIER II program. This
program will improve the community’s knowledge of the hazardous materials
present and allow multiple agencies to coordinate planning efforts. This grant
will come through the Emergency Planning Council and probably have
HSEMA as lead.
A DHS Homeland Security Grant to SERVE DC to compensate Fire and EMS
for costs associated with using members of the Fire Department to teach CERT
Training.
2. Please provide the name of each employee on administrative leave in FY
2010, and thus far for FY 2011. In addition, for each employee identified,
please provide: (1) their position; (2) a brief description of the reason they
were placed on leave: (3) the dates they were on administrative leave; (4)
whether the leave is paid or unpaid: and (5) their current status.

Please see the attachment for Question 2. Administrative Leave is paid leave not
charged against the employee. Enforced Leave uses the employee’s accumulated
leave and is charged against the employee.

4. (a) D.C. Law prohibits chauffeurs, take-home vehicles, and the use of
SUVs (see D.C. Code 50-203. 50-204). Is your agency in compliance with
this law? Please explain any exceptions.
Response:
The Department has seven (7) employees who reside outside of the District and
who are assigned take-home vehicles. These members include our Medical
Director, Assistant Fire Chief of Operations, Battalion Fire Chief for Homeland
Security, Canine Dog Handlers (3) and the Department’s Communications
Director. All of these individuals were assigned take-home vehicles because they
hold mission critical and specialized positions which require rapid deployment to
emergency calls 24 hours a day, seven days a week. Specifics for each individual
are included in the Attachment to Question 4.
(b) If there are exceptions, please provide the following: (1) Type of
vehicle (make, model, year); (2) individuals (name/position)
authorized to have the vehicle; (3) jurisdictional residence of the
individual (e.g.. Bowie, MD); and (4) justification for the take-home
status.

5. Please provide the following with regard to total overtime spending by
your agency:
a. In table format, broken down by local funds, etc., provide: (1) the
actual overtime costs for fiscal years 2007, 2008, 2009, 2010, and 2011
to date; and (2) the projected actual costs for fiscal years 2011 and
2012.

b. Identify what specific efforts the agency has implemented to reduce
overtime expenditures. Include in your response any protocols the
agency has implemented, and associate with each the overtime savings.
Response:
Because of the overtime limitation requirements of the “FEMS Overtime
Limitation Amendment Act of 2010” and management controls beginning in
January, 2011, overtime has been reduced by approximately forty five percent
(45%) during FY 2011 compared to FY 2010. Some of the measures put in place
to control overtime include the following:
Returning detailed personnel back to operational positions.
Monitoring Administrative Leave by only granting reimbursable or
contractually required Administrative Leave.
Reducing the number of extra support units placed in service (Water supply
engines and command unit).
Closely monitoring the number of additional units placed in service for natural
disasters such as weather related events.
c. Identify the person or persons within the agency is responsible for
monitoring the overtime problem and identifying means to reduce
overtime expenditures.

6. Please provide the date on which TeleStaff went “live.” Is TeleStaff fully
operational now? Provide a detailed description of how this program is
helping to streamline staffing efficiencies and reduce overtime?
Response:
The TeleStaff application went “live” on March 14, 2010. The system is now fully
operational and is staffed Monday through Friday by one Administrator or an
Officer with acting capabilities. The weekends are covered by an on-duty Officer
with acting capabilities or by an overtime Officer with acting capabilities.
TeleStaff replaced the “paper and pencil” method of staffing with a computerized
staffing method. Prior to TeleStaff, the on-duty Deputy Fire Chief Aide would
contact each individual Battalion Fire Chief Aide to obtain the Battalion’s staffing
requests. With TeleStaff, this practice is no longer necessary. TeleStaff provides
the Deputy Fire Chief for the on-duty shift with a roster of each member’s
assignment. Additionally, the application shows vacancies by fire station and
where the closest available member is located to fill the vacancy.
The TeleStaff application streamlines staffing efficiencies and reduces overtime
by:
TeleStaff provides a list of qualifications for each member. This helps to
reduce overtime by matching up on-duty members with special unit vacancies
that in the past may have been filled by overtime.
TeleStaff allows the Deputy Fire Chief to monitor the amount of Annual Leave
taken on any tour of duty. This limits the amount of overtime by ensuring that
no Battalion Chief grants more leave than they are authorized.
TeleStaff allows the Deputy Fire Chief to quickly obtain the amount of Sick
Leave, Emergency Annual Leave, Minor Illness Program, and other leave used
by members by using the advance calendar function for any tour of duty.
TeleStaff monitors the amount of overtime any member works provided it was
entered into the system. TeleStaff has audit capabilities that can determine if the
overtime list was followed.
7. Please provide a list of all unbudgeted expenditures (e.g., expenditures not
contemplated in the budget as adopted by the Council) by the agency in
fiscal year 2010 and thus far in fiscal year 2011. Include in your response
the amount of the expenditure and a brief explanation of each.

Currently there are twelve (12) employees that have not passed the NR-EMT
certification examination. The status of these employees is as follows:
One (1) Firefighter. This employee just entered the examination process after
returning from extended Administrative Leave.
One (1) Firefighter/Technician. This employee is currently on Administrative
Leave and will need to take the sixth (6th) and final examination attempt.
Two (2) Firefighter Recruits. One (1) was scheduled for the examination on
March 9, 2011, and one (1) failed the first examination attempt.
Four (4) EMTs or EMT-Ps (single role). One (1) employee is scheduling a third
(3rd) examination attempt, one (1) is appealing the NR-EMT examination
process and two (2) will be testing for NR-EMT Paramedic certification.
Two (2) employees have been placed on Administrative Leave after six (6)
failed attempts.
Two (2) employees have been re-assigned to the Operations Bureau.
Additionally, there are four (4) employees in the NR-EMT examination process
working in other bureaus or divisions of the Department who have failed less than
two (2) NR-EMT examination attempts, along with seventy five (75) pre-1987
employees that are not EMT certified and have not entered the NR-EMT training
program.
b. The number of personnel currently at the training school for the
NREMT.
Response:
Currently, there are seven (7) employees reporting to work at the Training
Academy for NR-EMT or NR-EMT Paramedic testing.
c. The number of individuals that have taken the exam twice, three
times, four times, etc., as well as the number of individuals terminated
by the agency for failure to pass the exam.

The Department is only able to estimate the total cost of National Registry training
for all employees since 2007 at $10.3 million, including the cost of examination
failures. This estimate accounts for the cost of NR-EMT vouchers, books, supplies
and instructor costs; employee salaries while assigned to the NR-EMT training,
remedial NR-EMT training or NR-EMT refresher programs and overtime for
testing. It does not account for back-fill overtime to the Operations Bureau on shift
days when the number of employees available to work could not cover the
temporarily vacant positions assigned to the Training Academy for NR-EMT
programs.
e. The policy for renewal of NREMT re-certification (e.g., for
everybody? How often? etc.)
Response:
The renewal requirement for National Registry certification is each two (2) years,
provided continuing education (CE) hours are complete. Employee NR-EMT
certifications expire each March 31st, based on this two (2) year CE recertification
cycle.
f. The Department's policy for those personnel who do not now have
NREMT certification.
Response:
Department employees hired before January 1, 1987, were not required to have
NR-EMT certification as a condition of employment and, for budgetary reasons
moving forward into FY 2012, will not be required to obtain it. However,
Department employees hired after January 1, 1987, must obtain and maintain NR-
EMT certification as a condition of continuing employment. Employees in this
category who do not have the certification will be sent to the NR-EMT training
certification program.

The Department is receiving support from the two major EMS physicians groups:
the National Association of EMS Physicians (NAEMSP) and the U.S.
Metropolitan Municipalities EMS Medical Directors Consortium (The “Eagles”

We performed outreach and recruitment at the annual conferences for NAEMSP
(January 2011) and EAGLES (February 2011).
b. The number of candidates as of your answer date, and of these the

As of March 9, 2011, eleven (11) candidates have expressed interest in the position
of Medical Director. The Department is reviewing the qualifications of candidates
with the goal of determining which will be invited to interview.
c. The timeframe for submitting a nominee to the Council.
Response:
The Department is working to complete the interview and selection process during
the month of April, with the goal of presenting the Mayor’s nominee to Council for
confirmation no later than May 30, 2011.
11. According to FEMS' March 8, 2010 responses to questions from this
Committee, the projected completion date for the Insurance Services
Office (ISO) review of the Public Protection Classification review of the
District was May 2010. Please submit, as an attachment, a copy of this
report to the Committee.

12. According to FEMS' March 8, 2010 responses to questions from this
Committee, the Table of Penalties was completed on February 23, 2010,
but was being reviewed for legal and financial sufficiency. Please provide,
as an attachment, the Table of Penalties for standardizing the penalty
assessment phase of the disciplinary process. Please explain the status of
this Table - e.g., was it implemented? Is it in use today? If union
agreement is sought, what is the state of those discussions? Etc.
Response:
The purposed Table of Penalties was not implemented and is not in use today. It is
one of the items being sought by negotiation of a new Collective Bargaining
Agreement with IAFF Local 36. Because this is a topic of ongoing negotiations, it
would be inappropriate for the Department to comment any further on this topic.

On March 1, 2011 (Platoon #1 on-duty, operations bureau personnel), there were:

5 employees on Administrative Leave.
9 employees on Enforced Leave or Leave Without Pay because of discipline.
76 employees (from all bureaus, divisions and offices) were carried on Sick Leave
by the Police and Fire Clinic (PFC). Of these employees, 39 were absent due to
Performance of Duty injuries or illnesses and 38 were on their own Sick Leave.
Additionally, 50 employees were carried on Limited Duty by the Police and Fire
Clinic (PFC). Of these employees, 23 were on Limited Duty due to Performance of
Duty injuries or illnesses, and 27 were on Limited Duty due to non-POD medical
conditions.
16. With regard to FEMS recruits, please provide the following:
a. How much does it cost to send a recruit through a complete physical
and psychological testing?
Response:
Recruit physicals for up to 400 Fire EMS and MPD recruits combined are covered
under the Police and Fire Clinic contract at no additional cost to the agencies.
Additional physicals are charged to the agencies at the rate of $494.00 for the
medical examination and $275.00 for the Employment Suitability Assessment for a
total cost of $769.00 per recruit.
b. How many recruits have gone through this testing but have not been
hired by FEMS?
Response:
For the employee candidates that participated in the 2008 application and testing
process:
382 employee candidates participated in the medical examination and suitability
assessment screening process.
235 employee candidates were not hired or did not accept offers of employment.
147 employee candidates accepted offers of employment.
c. Of the answer to (b), how many are still awaiting a decision to be hired
(as opposed to having been rejected)?
Response:
For the employee candidates that participated in the 2008 application and testing

17. The Mayor's FY 2011 budget included a cost savings of approximately
$1.8 million through closure of one firehouse, the hazardous materials
unit, and combining this unit with an existing firehouse: Engine 12
(which itself will be closed when the hazardous materials unit is
responding to calls). What is the current status of the hazardous
materials unit? If the original location of the hazardous materials unit
has been closed, and the proposed merger with Engine 12 completed,
what function does that original firehouse serve, if any?
Response:
There were no staffing or location changes implemented affecting the Hazardous
Materials Unit or the fire station described by the question. The cost savings were
made up by a reduction in the total number of Department personnel, mostly at the
Firefighter Recruit level.
18. The Mayor's FY 2011 budget proposal includes a cost savings of
approximately $1 million by transferring the fire hydrant inspection
program back to WASA (now DC Water).
a. What are the actual savings this fiscal year? Include in the calculation
WASA's hydrant fee.
Response:
On October 1, 2010, the Fire and EMS Department eliminated thirteen (13)
positions (FTEs) at the recruit level from closing down the Customer Service Unit
(CSU) program. Using the salary and benefits paid to entry level firefighter recruits
($50,348), this resulted in an estimated annual savings of $654,524.
Additionally, the thirteen (13) employees working the CSU units were reassigned
to the Operations Bureau, decreasing overtime pressure by 3.25 FTEs per shift day
(there are 365 “shift days” per year). Assuming, on average, the additional FTEs
saved between one (1) and two (2) 24 hour shift days of overtime each day from
the period of October 1, 2010 through January 15, 2011, this resulted in an
estimated savings of between $90,720 and $181,440.
Because of the “hydrant fee” rule change by DC Water during the FY 2011 budget
process, and subsequent discussion and negotiation between DC Water and District
Government by the previous Mayoral Administration, it remains unclear if actual
cost savings were realized at the District level. However, Department level cost
savings, as described, did take place.
b. What is the status of the memorandum of understanding with DC
Water? Is there only one with regard to hydrants? If it is completed,
please provide a copy as an attachment.
Response:
A new MOU with DC Water was not completed by the previous Mayoral
Administration, City Administrator or Fire and EMS Chief. Please see the
attachment for Question 18, the “current” MOU, parts of which are no longer
valid.
c. What is the status of the hydrant inspection program (i.e., are
hydrants currently being inspected and, if so, how often, by whom,
and what are the timelines for repair? Replacement?)?
Response:
Per the Department’s understanding, DC Water is starting a hydrant inspection and
maintenance program that incorporates some of the services performed by the
Department’s CSU program, including once per year flush testing of fire hydrants.
Out-of-service or damaged hydrants discovered during the course of Department
operations are reported to DC Water through the Operations Bureau and the chain-
of-command. The terms of DC Water’s responsibility for hydrant inspection and
maintenance following the end of the CSU program were a subject of negotiation
between the previous Mayoral Administration, the City Administrator and the Fire
and EMS Department just prior to transition.
19. Please explain what recommendations of the Rosenbaum Task Force
remain to be completed? Include in the explanation what will be done,
when, and for how long to complete each incomplete recommendation.
Response:
Forty two (42) of the fifty (50) action items contained in the Task Force Report andRecommendations have been completed. Eight (8) are incomplete, as follows:
From Recommendation 1: The Department of Fire and Emergency MedicalServices shall transition to a fully integrated, all hazards agency.
1) a (Part 2) All operational employees shall be cross-trained at basic levels ofEMS, fire prevention, fire suppression, hazardous materials and technicalrescue.
Currently, 87% of the operational workforce (approx. 1,730 personnel out of1,987) is cross-trained and fully functional in the all-hazards model. As ofFebruary 2011, approximately 75 pre-1987-hire firefighters remain who have notyet begun the EMT certification process, and approximately 156 single-role EMSworkers remain who are not yet certified as firefighters. 32 former single-rolesupervisory employees began All-Hazards training (firefighter and haz-matcertification) in September 2009. The Department continues to explore options forcertifying the 156 remaining single-role EMS workers, however, with the currentpolicy of only hiring fully cross-trained employees, it is expected that the challengeof employing single role providers will be resolved through attrition. The purposefor requiring only employees hired after 1987 to be completely cross-trained is aresponse to the increasing overtime that the department faced while attempting torequire employees hired prior to 1987 to meet a standard that was not in placewhen they were hired.
1 b) The Department shall offer current single-role providers basic trainingfor all hazards on a phase-in basis. The Department shall allow single roleproviders to meet adjusted fitness standards that fairly and reasonablyaccommodate their incumbent status, including their age and level ofexperience.
The Department continues to explore options for cross-training the approximately156 remaining single-role EMS workers. 32 former single-role supervisoryemployees began All-Hazards training in September 2009. We are waiting for thecompletion of the third of three consecutive and concurrent (with some overlap)
recruit classes before we begin a series of new internal training classes. We arealso facing the impending retirement of our Training Director and have tasked hisassistant to develop a cross-training schedule for our single role providers.
From Recommendation 3: Improve the level of compassionate, professional,clinically competent patient care through enhanced training and education,performance evaluation, quality assurance, and employee qualifications anddiscipline.
3 e) part 2) Online medical direction to FEMS personnel shall be providedonly by licensed physicians who are adequately trained and are designated asqualified by the Medical Director. Online medical direction shall be subject tothe QI process.
The Department has no objection to this recommendation in principle, howeversufficient resources do not exist to accomplish the recommendation as originallyintended. One option that had been considered was centralizing medical controlsuch that a team of physicians functioning as Department employees (full-time orpart-time) would perform medical control for all units from a central site such asthe OUC. These physician medical control positions do not currently exist in theagency budget, and this option would likely be prohibitively expensive.
The interim solution is that the Department, in collaboration with the directors ofthe local emergency departments, has reengineered the online medical controlprocess such that each receiving facility is now responsible for providing medicalcontrol to patients en-route to its facility. This places the responsibility forensuring knowledge and qualifications of the medical control physician on thereceiving facility. The Department offers protocol training for any facility whichrequests it, and has endured the updated copies of the protocols are in place at eachmed-control base station. Med-control is subject to the quality review processoverseen by the Office of the Medical Director, and when quality issues arise, theyare addressed with the relevant facility through the peer-review protected medicalquality management process.
The Department will re-visit this recommendation when the permanent MedicalDirector is appointed to determine if alternative strategies should be pursued, or ifthe existing process is sufficient to ensure quality and consistency of medicalcontrol guidance.
From Recommendation 4: Enhance responsiveness and crew readiness byrevising deployment and staffing procedures.
4 b) The Mayor and Chief shall work together to come with arecommendation to the Council to implement shorter shifts for all employeesand other recommendations to ensure the goal of having alert and awakeemployees who can provide competent patient care.
The current Transport Unit Staffing Plan, developed with Local 36, IAFF, offersfirefighters detailed to three month ambulance rotations the option of choosing towork a 12-hour, rather than 24-hour shift. None to date have chosen this option.The Department plans to renegotiate the current Transport Unit Staffing Plan.
4 j) Part 1) The Chief shall require, effective with the next contract: Allpersonnel to maintain or acquire EMS certifications in order to be retained asemployees of the Department after December 31, 2010.
Implementation of this recommendation requires collective bargaining.
4 j) Part 2) The Chief shall require, effective with the next contract: Allcandidates for promotion to the rank of Sergeant or higher to have served as afield-certified EMS provider, according to criteria established by the MedicalDirector that requires a minimum cumulative number of patient contactsassessments and treatments.
Implementation of this recommendation requires collective bargaining.
From Recommendation 5: Reduce misuse of EMS and delays in patient transfers.
5 g) The Medical Director shall, no later than September 30, 2008, develop aprocedure to authorize patients to be transported to a pre-approved clinic orother non-emergency medical facility, appropriate to the patient's need.
Currently, the only pre-approved clinic that the Department transports to is thePolice and Fire Clinic, for Department employees with non-critical illnesses orinjuries that can be managed appropriately at that facility.
At this time, there are very few facilities in the District that F&EMS canimmediately bring non-emergent patients to when responding to a 911 call thatmay not necessarily merit transport to an emergency room. In the absence of suchdestinations, it is not yet feasible to implement the procedure called for by thisrecommendation, although doing so remains the long-term goal of F&EMS.
Using the Community Health Care Financing Fund, and guided by the RANDreport recommendations, DOH has provided capital development grants for the
construction of new or expanded facilities to increase primary, urgent, andemergency care capacity. We have requested a status update on this project fromthe new leadership at DOH and will provide an update to the Committee after wehave received their information.
5 h) The Medical Director and the Director of the Department of Health shalldevelop and implement, no later than September 30, 2008, a system ofalternative transportation options (such as scheduled van service, taxivouchers, or MetroAccess vouchers), as well as protocols to refuse transportfor non-urgent patients, when appropriate, subject to the authorization of amedical supervisor.
The implementation of a protocol to refuse transport of patients who call 911 for
assistance is contingent on the District’s ongoing efforts to develop alternative
transportation options and non-emergent delivery points. While F&EMS agrees
with the goal of this recommendation – to reduce misuse of 911 and unnecessary
ambulance transports – it is the Department’s position that this issue is best
addressed by connecting patients to appropriate medical homes and enhancing the
District’s primary, urgent and emergency care capacity. Further, it is the decision
of the Medical Director that the risk of implementing a provider-initiated non-
transport protocol outweighs the benefit at this time. The Department is focusing
its efforts on ensuring that patients encountered by F&EMS are aware of their right
to treatment and transportation, and that any patient-initiated refusals receive
supervisory review and approval.

services (CPR) at the Department’s training academy.
