Indicación de tratamiento de infección por VHC enpacientes coinfectados por VIH.
Consenso de diferentes guías
1. En todas la personas coinfectadas por VIH/VHC debe considerarse eltratamiento con AADs frente a VHC (preferiblemente en pautas librede interferon) independiente del estadio de fibrosis.
2. Los regimens terapéuticos serán los mismos indicados para pacientesmonoinfectados por VHC.
3. Los únicos requisitos específicos en pacientes infectados por el VIHson los derivados de las interacciones entres AADs y ARVs

Enfermedad hepática crónica avanzada compensada. Workshop Consenso Baveno VI 2015: estratificación del riesgo e individualización del manejo de la HT portal
• Define el espectro de fibrosis avanzada y cirrosis no complicada (pacientes asintomáticos)
• Diagnóstico por elastografía: 10-15 kPa (sugestivo / posible) y > 15 kPa (altamente sugestivo / muy probable).

Conclusiones (I)
• Muchos pacientes coinfectados por VIH/VHC tendrán una enfermedad hepática crónica avanzada compensada en el momento de la curación de la hepatitis C
• Estos pacientes requerirán un seguimiento donde se evalúe de acuerdo a consensos establecidos:
• existencia de hipertensión portal clínicamente significativa• presencia de varices esofágicas• existencia de encefalopatía hepática mínima y el • cribado de hepatocarcinoma.
• En la mayoría de los casos este seguimiento, incluido las profilaxis primarias de complicacioines, puede (y debe) ser llevado a cabo por los clínicos que manejan la infección por el VIH

Conclusiones (II)
• Algunos pacientes curados de hepatitis C presentan un alto riesgo de reinfección y de ser fuente de infección por VHC para otros pacientes.
• En estos casos se recomienda la vigilancia de reinfección al menos anual (y antes según riesgo) con determinación de ARNVHC
• En los casos con reinfección se recomienda un tratamiento precoz como prevención.
• Otras causa de hepatopatía crónica o aguda no relacionada con VHC deben tenerse en cuenta en el diagnóstico diferencial
