OnMay19th2010,inhonorofNationalEMSWeekandourEMSstudentbody,CityCollegehostedthefirstAnnualALS(advancedlifesupport)Competition,alsoknownastheJamesMalcolmALSCompetition,whichwasnamedintributeforourEMSclinicalcoordinator.FifteenteamsfromaroundtheStateofFloridasuchasFlaglerCountyFireRescue,St.Johns,andTempleTerraceEMSsystemsattendedtheCollege’sfirstAnnualEMSALScompetition. Theeventincludedalargescaleinteractive(mock)scenariothatchallengedateamofthreeparamediccertifiedteammemberseachtotakeonascenethatencompassessavingmultiplevictimsofacardiacevent.TheeventpurposefullychallengedALSteamstorespondtoascenariowhichisonlybecomesrevealedtothematthepointofcall.Theteamhadfifteenminutes
tosortthroughaseriesofeventsthatledtopatientstoacriticalcondition.Metro-DadeFireRescuewontheevent,withBocaRatonFireRescuesecond,andMiramarFireRescuecomingin3rd.HonorablementiongoestoDavieFireRescuewhocameinat4thplaceinaverycloserunforthird. Over200teammembers,judges,staff,EMSstudentsandparticipantswerepresenttowatch(andparticipate)inthisintensiveandeducationalmasscasualtyscenario. Fortunatelyformethepasttwoyearshavebeenverybeneficial.Eachyearitisourgoaltotravelthe‘world’twice;oneoutofStatetripandtheotherisusuallyclosertohome.Notwotripsarethesame;eachisveryunique.Forexample,thispastAprilthebroadcastingclubhoppedonaflighttoAtlantatotourCNN World Headquarters,Crawford CommunicationsandV-103 (CBS)radiostation. Thesearethreecompletelydifferentareasinthefieldofcommunications,buteachtourleftadifferentimpressiononallnineteenofus.
AsidefromthefactthatAtlantaisawonderfulcity,weallleftwiththereassurancethatnomatterwherewemayendupafterCityCollege,thereisaplaceforusallinthemarketplace.Withvastknowledgeandhands-onexperience,IcanhonestlysaythateachandeveryoneofuscangraduatetodayandtakeourplacedoingexactlywhatwecametoCityCollegetolearn. Mr.KevinCooper’sparamedicclassenjoyedauniquehands-onexperienceofworkingwithrealpigtracheas,lungsandheartsalongwithrealchickenlegs.Apig’sanatomy,especiallyitsairway,isextremelysimilartothatofhumans.Mr.Cooper’sparamedicIIclasshasbeenstudyingtherespiratorysystemandadvancedairwaytechniquesthissemester.Thislabgavethestudentstheopportunitytotaketheoryandputitintopractice. Mr.CooperwasassistedinthelabexercisebyEMSinstructorsAlainRodriguezandSteveSweetman.ThislabcouldnothavebeenpossiblewithouttheEMSadministrationbacking.WethankEMSDirectorMr.ChrisCrespowhowaskindenoughtoorderandpickupthepigorgansfromalocalmeatpackingcompany. Ittookacoordinatedefforttoconductthislabduetothenumerousskillstationsthatweresetup.ThestudentshadtorotatethroughfourskillstationsandhadtodemonstratetheircompetencyinSurgicalandNeedleCricothyrotomy,IntraosseousInfusion,Endotracheal
IntubationandPleuralDecompression. Abriefdescriptionofeachprocedurelistedabovemighthelptocreateabetterpictureofthisuniquelab.SurgicalandorNeedleCricothyrotomyisaninvasivetechniquethatplacesanendotrachealtubedirectlyintothetracheathroughasurgicalincisionatthecricothyroidmembrane.IntraosseousInfusionisusedwhenanIVisnotavailableandincludestheinsertionofarigidneedleintothelongbonecavity.Forthisskillthestudentsusedchickenlegs.EndotrachealIntubationisoneofthemostimportantparamedicsskills.Whenapatientisnotbreathingorunconsciouswithsevereairwaycompromiseendotrachealintubationisindicatedtomanageandsecureapatient’sairway.APleuralDecompressionisdonebyplacingalargeneedleintothethoraciccavity.Thisprocedureisperformedtorelieveatensionpneumothorax(collapsedlung)usuallycausedbytrauma. Thestudentsappearedtoreallyenjoythislabandthefeedbackfromthemwasquitepositive.

Fainoutilizeshisownsmoking,roastingandbarbecueequipment.OntwooccasionshewaskindenoughtopurchaseallthemeatsandfixingsfortheCollege.Foroneoftheevents,Fainosmokedthemeatsfor18hourspriortoserving.Hestationedhiscookingequipmentintheparkinglotthenightbeforeandslowcookedthefoodtoperfectionallnightlong.Ifthestomachisthewaytowinhearts,Fainocertainlyhasmanyadmirersamongthefaculty,staffandstudentsatCityCollege. Hehasbeenactivewiththestudentambassadorsfortwoyearsandisalwaysavailabletoassistforeventssuchasnewstudentorientation.
