DS-2019 Form Data Field Description Overview This document provides an overview of the DS-2019 panel in M-Pathways. The information entered in this panel is sent to SEVIS in order to create the Exchange Visitor (EV) SEVIS record and generate the paper DS- 2019 form. The DS-2019 panel in M-Pathways displays data that is recorded in MPathways for each DS-2019 form, including the following information: Details about the DS-2019 form, including visitor’s category, creation reason, and start date Biographical and address data about the exchange visitor The exchange visitor’s estimated financial support/sources The Responsible Officer for this DS-2019 form Information about the dependent(s) accompanying the Exchange Visitor, if applicable The location of the EV’s program activities Navigation Main Menu > Campus Community > SEVIS > DS-2019 Forms > DS-2019 Form Find an Existing Value Page 1 Type the individual’s UMID in the EmplID field. DS-2019 Form Data Field Description

Overview

This document provides an overview of the DS-2019 panel in M-Pathways. The information entered in this

panel is sent to SEVIS in order to create the Exchange Visitor (EV) SEVIS record and generate the paper DS-

2019 form. The DS-2019 panel in M-Pathways displays data that is recorded in MPathways for each DS-2019

form, including the following information:

Details about the DS-2019 form, including visitor’s category, creation reason, and start date

Biographical and address data about the exchange visitor

The exchange visitor’s estimated financial support/sources

The Responsible Officer for this DS-2019 form

Information about the dependent(s) accompanying the Exchange Visitor, if applicable

The location of the EV’s program activities

Find an Existing Value Page

1 Type the individual’s UMID in the

EmplID field.

NOTE: If the EmplID is unknown, enter

the appropriate data in the Campus

ID (uniqname), National ID

(Social Security number), or Last

Name and First Name field.

2. Click “Search” button.

3. Confirm the individual’s name and ID.

The following pages will provide information about the fields on the DS-2019 Form in M-Pathways.

Letter Field/Button Description

A Effective Date Displays the date on which this DS-2019 data was entered.

B Status Displays the status of this DS-2019 data row in M-Pathways. Valid values

are Active and Inactive.

Note: Inactive is rarely used.

C Visitor’s Category Displays the J-visa category for the exchange visitor. Categories used by

UM include Professor, Research Scholar, Short-term Scholar, Specialist,

and Student.

Note: Student and Specialist is rarely used. Student not used for J-1 Scholars.

D Academic Institution Displays the name of the institution if a student category displays in

the Visitor’s Category field. Defaults to UMICH. Not used for J-1 Scholars.

E Academic Career Displays the individual’s academic career that is associated with this DS-

2019 form if a student category displays in the Visitor’s Category field. Not used

for J-1 Scholars.

F Academic Program Displays the individual’s academic program that is associated with this DS-

2019 form if a student category displays in the Visitor’s Category field. Not used

for J-1 Scholars.

G Admit Term Displays the four-digit term for which the individual was admitted if a

student category displays in the Visitor’s Category field. Not used for J-1 Scholars.

Exchange Visitor Information and US Address sections

Letter Field/Button Description

A Family Name, Middle Name, First Name

Displays the exchange visitor’s name.

B Gender Displays the exchange visitor’s gender abbreviation.

C Date of Birth Displays the exchange visitor’s date of birth.

D City of Birth Displays the city in which the exchange visitor was born.

E Birth Country Displays the country code and country in which the exchange visitor was born.

F Citizenship Country Displays the exchange visitor’s country of citizenship.

G Permanent Country Displays the exchange visitor’s country of legal permanent residence. Note: Cannot be US or US Territory.

H Position Code Displays the three-digit numeric code representing the most recent position or occupation that the exchange visitor held in their home country.

I US Address section Displays the street address, city, state, and postal code (ZIP code) of exchange visitor’s US address. If the Exchange Visitor has not yet entered the U.S. it displays the Site of Activity address.

Creation Reason and Subject/Field of Study section

Letter Field/Button Description

A Creation Reason Displays the reason the DS-2019 was created. Note: Continuing is rarely used.

B Start Date Displays the date that the exchange visitor is expected to begin his/her program.

C End Date Displays the date that the exchange visitor is expected to end his/her program.

D Subj/Field Code Displays the code that represents the exchange visitor’s subject or field of study. This is also known as the CIP code (see http://nces.ed.gov/ipeds/cipcode/Default.aspx?y=55).

E Subj/Field Remarks Displays the exchange visitor’s subject/field description and EmplID. This field is required.

A Current Program Sponsor Funds Displays the amount of financial support provided by the program sponsor.

B Received US Government Funds checkbox

Indicates that exchange visitor is receiving funds from the US Government. This is almost never used.

C US Government Agency Code and Amount

Not used.

D US Government Agency Code 2 and Amount

Not used.

E Intl Organization Code and Amount

Not used.

F Intl Organization Code 2 and Amount

G Other Organizations Support Displays the amount of funding provided by any other organization.

H Name If any other organization is providing funding, displays the name of the organization.

I Exchange Visitor’s Government Not used.

J Binational Commission Not used.

K Personal Funds Displays the amount of personal funds available to the exchange visitor for expenses.

L Total Financial Support Displays the total funding available to the exchange visitor.

M Funding Verified checkbox Indicates that the exchange visitor’s means of support has been verified and the DS-2019 form is ready for the Alternate Responsible Officer (ARO) to review.

N Remarks Displays remarks regarding the exchange visitor’s DS-2019 form or funding, if applicable. Typically IC internal remarks.

Responsible Officer Data and Dependent Information sections

Letter Field/Button Description

A Responsible Officer Displays the Responsible Officer (RO) responsible for the exchange visitor’s DS-2019 form.

B Address Line 1, Address Line 2, City, State, Postal

Displays the location where the DS-2019 form was issued.

C Date Prepared Displays the date on which the DS-2019 form data was entered.

D Date Signed This field is not used.

E ID Displays the EmplID of the dependent, if applicable.

F Name Displays the name of the dependent, if applicable.

G Relationship Status Displays the status of the dependent’s relationship with the exchange visitor for whom the DS-2019 form was created. Valid values are Active, Deleted, Ended Status, and Terminated. Note: Deleted and Terminated are rarely used. Ended Status is used when the SEVIS Status for the principal visa holder is Active and a request to remove a dependent has been submitted by the EV.

H Relationship Displays the relationship of the dependent to the exchange visitor for whom the DS-2019 form is being created. Valid values are Spouse and Child.

I Permanent Country Displays the dependent’s country of permanent residence.

J Details link When the Relationship Status is Terminated or Ended Status, this link

displays. Click Details to view the Dependent Details page, which displays the reason for the

relationship status.

Site of Activity section

Letter Field/Button Description

A Site of Activity Displays the code and description for the location where the exchange visitor is participating in his/her program.

B Primary Checked to indicate this address is the Primary Site of Activity.

C Status Displays the status of the exchange visitor’s site of activity. Valid values are Active and Inactive.

D Address 1, Address 2, City, State,Postal

Displays the street address for the exchange visitor’s site of activity.

E Departmental Contact link

Click Departmental Contact to view the name of the Administrative

Contact, Supervising Faculty, and Dean, Director, or Chair for this exchange visitor. These fields are required. See the following page for Departmental

Contact page field descriptions.

Displays the name of the exchange visitor’s job title or academic career.
