Under the direct supervision of the emergency department faculty, fellows, and resident staff, the student will be given the opportunity to participate in the triage, evaluation, stabilization and management of pediatric and adult patients with a wide variety of clinical conditions. Students will have the opportunity to contribute to patient care through patient examination, data collection, radiology and laboratory studies evaluation, as well as participating in clinical rounding. The ED preceptorship occurs on Friday evenings and Saturday mornings or evenings during Periods 1 and 2 and is conducted at a number of hospitals in the Miami area: o o o o o o o Miami Childrens Hospital Jackson North Medical Center South Miami Hospital Baptist Hospital Mt Sinai Medical Center, Miami Beach Memorial Regional Hospital Mercy Hospital

In Period 1 the schedule is designed so that each student will have exposure to each of the sites. In Period 2 students will have flexibility to concentrate experience at a smaller number or single sites if they can arrange this with classmates.

The experience is intended to: 1. Provide the student an exposure to clinical medicine early in the medical school program and allow an opportunity for the student to see and practice skills that they are learning in the Clinical Skills course at the Gordon Center. 2. Allow the student to become familiar with the hospitals which subsequently will be utilized for core clinical experiences and clerkships in Periods 3 and 4. 3. Become familiar with the staffing of an Emergency Room and how the various individuals (e.g. physicians, nurses, physician assistants, nurse practitioners) function and interact 4. Become familiar with the EMS system and how patients arrive into the medical system via an emergency room 5. Recognize the general medical status of the patient as they arrive in the Emergency Department and become familiar with the way in which the Triage system works. 6. Become familiar with the ways in which consultations from other services are arranged and performed in the Emergency room 7. Become familiar with emergency use of ancillary facilities such as Imaging 8. Become familiar with the problems associated with lack of medical insurance and the ability to obtain medical care 9. Be fully aware of HIPAA regulations and be respectful of patient confidentiality 10. Become familiar how history, patient examination findings, laboratory studies and consultations are incorporated into the medical record. Students should not, however, directly enter information into the record. 11. Become familiar with alternative sites of care. Could this emergency visit have been avoided by seeking care elsewhere? How could this visit have been managed more efficiently? 12. Where possible, observe the communication of bad news to family members 13. See, at first-hand, pathology which you may be learning about in your classroom and begin to become familiar with some of the diagnostic tests ordered and the pharmacologic agents and other treatments used

PROCEDURES During your rotations you should become familiar with the following procedures. Most of these will be observed however some may be performed under the direction and supervision of the Attending and at their discretion. Starting an intravenous line Insertion of a Foley catheter into the bladder CPR observe Drawing venous and arterial blood samples Accompany patients to Radiology for studies Clean wounds, apply dressings, become familiar with suturing Become familiar with acute hemorrhage and its treatment Observe intubation/management of airway Insertion of nasogastric tube

CLINICAL SCENARIOS It is likely that you will be exposed to a wide range of clinical conditions in the ED. However it is expected that you make the effort to see each of the following conditions at least once during your rotations. Acute psychiatric admission Chest Pain Shortness of breath Bleeding in pregnancy Abdominal pain Substance abuse Domestic violence Stroke or stroke like symptoms/seizures Medical problems of homeless Orthopedic injury diagnosis and management of a limb fracture Shock Altered mental status

Specific Objectives for the Miami Childrens Hospital have been developed by Dr. Biehler. EDUCATIONAL OBJECTIVES: PEDIATRIC EMERGENCY DEPARTMENTS PATIENT CARE 1. Students will learn the skills necessary to obtain vital signs from pediatric patients. The following clinical skills will be included in this objective:

a. Recognizing the general state of well-being of a child at initial Emergency Department triage b. Determining the capillary refill time and interpreting the clinical significance c. Measuring the heart rate in patients of different age groups and interpreting that measurement in relationship to the expected normal values for age d. Measuring the respiratory rate in patients of different age groups and interpreting the measurement in relationship to the expected normal values for age. e. Measure the blood pressure of children of different age groups and interpret the measurement in relationship to the expected normal values for age. Student should also recognize the difficulty of obtaining BP measurements in young or uncooperative patients. i. Students should practice manual BP determinations and flush measurement of BP on infants ii. Students should observe the automated methods of determining BP in children f. Students should practice obtaining oral and axillary temperatures

g. Students should observe the nursing staff obtaining rectal temperatures in pediatric patients

2. Students will learn to take a preliminary history regarding patients presenting to an Emergency Department. Specific skills for development include: a. Obtain and record a Chief Complaint from the family of a young child or and directly from older children b. Obtain a symptoms directed history of present illness. c. Obtain a brief Past Medical History for a pediatric patient. Students should recognize the importance of obtaining the following information: i. Birth history ii. Significant past illness and chronic conditions iii. Current treatments/medications and their dosages 3. Obtain information about the patients Medical Home and recognize the importance of identification of that home, communication with the providers of care from the Medical Home, and the importance of referral to the Medical Home for continuity of care. 4. Students will observe how to appropriately document information in the Emergency Department medical record. Students are not expected to make entries into the medical record but should review the medical record with regard to the initial triage notations, nursing entries,

and physician documentation. 5. Students should identify an individual patient presenting to the Pediatric Emergency Department with a medical complaint related to respiratory symptoms. Pediatric examples for such conditions could include: wheezing, asthma, congestion and cough, croup, respiratory distress, pneumonia, cystic fibrosis, or general complaint of breathing problems. Students should then focus on following that patient as the child progresses through the processes of evaluation and treatment. a. Students should carefully examine the pulmonary and cardiovascular system of the patient with direct mentoring by either the resident or attending physician. b. Students should focus on the detection and description of breath sounds, effectiveness of inspirations, ratio of inspiratory to expiratory times, any diminishment of breath sounds, and the presence of signs of respiratory distress. c. Students should observe and note the initial steps in stabilization and treatment of their patient. i. Note any respiratory support measures initiated (i.e. oxygen administration or patient positioning) ii. Note all medications provided to the child 1. Note dosages and how those dosages were determined 2. Note the delivery method (IV, IM, oral tablets, oral liquids, rectal, or respiratory treatments by nebulization) 3. Note how the child reacted to the administration of the medication. 4. Note how the staff determines which medications could present a risk to the patient because of allergies or medical condition 5. Medications used: a. in the placement and stabilization of an artificial airway

b. to support circulation and tissue perfusion c. the treatment of severe asthma and respiratory failure d. in the treatment of status epilepticus e. for patient sedation iii. Note the response of the patient to medical interventions. iv. Students should reassess the vital signs of their patient with the nursing and medical staff several times during the Emergency Department stay.

d. Students should recognize and note the importance of the interaction between the Department staff, the childs family, and the patient. Was the level of communication to the child age appropriate? Did the staff communicate well with the family regarding the illness, causes of illness, treatment plans, expectations for clinical progress, and outpatient instructions? e. Students should review the medical plan of care for either the discharge of their patient to home or admission of the patient to the inpatient service. i. Note the decision of the attending physician regarding disposition.

ii. Ask the attending physician about the decision making process regarding thedisposition of the patient. MEDICAL KNOWLEDGE Student will integrate human biology course concepts that present themselves in patient encounters. Student will perform the system-specific physical examinations as directed in the clinical skills course objectives.

PRACTICE-BASED LEARNING At MCH students will obtain a required reading packet from the attending physician. These articles are to be taken home and reviewed after your Emergency Department experience in preparation for your next rotation at the ED. At all sites students will keep a log of all patient encounters they observe documenting the following relevant information: Demographics including age, gender, ethnicity Acute or chronic diseases discussed or managed at the visit Mental illnesses Psychosocial issues that arise Ethical dilemmas

INTERPERSONAL AND COMMUNICATION SKILLS Experience the documentation process for a pediatric Emergency Department Develop skill, through observation and participation, in communicating patient related information to families of ill patients Experience and understand the importance of working with the many non-clinical members of the

medical care team

SYSTEMS-BASED PRACTICE Students will become familiar with the role of each of the following members of the health care team at the preceptorship site: registration, nurse, medical assistant, social worker, medical records. Students will become familiar with the process for making referrals to a mental health provider, to a medical subspecialist, for an imaging study, and for social services in the primary care and emergency room settings. Students will recognize barriers that exist to efficient communication and collaborative care within the health care and social service systems.

PROFESSIONALISM DOS AND DONTS 1. You need to check your schedule and make sure which ED it is that you are assigned to. Note that different sites have different times for starting the rotation and that some sites have more than one session on Saturdays. You will not get credit if you go to the wrong ER. 2. Do not aim to arrive on time arrive 10 minutes early and introduce yourself to the Attending in Charge. Stay for the assigned length of time. If you wish to stay longer you must obtain permission from the ED Attending you are working with. 3. Always wear your FIU White Coat and your FIU ID. In addition wear any hospital ID which has been issued to you. All sites have approved scrubs as a suitable dress for this rotation. Take your stethoscope. 4. A short grace period will be given at the beginning of the semester to allow students to switch sessions. In Period 1 sessions can ONLY be switched for sessions at the same hospital and obviously you will need to find someone who will agree to switch with you. You will need to contact Tatiana Chase ([email protected]) with the desired change. This email should be ccd to the student with whom you wish to switch so that we know that student is aware. Ms. Chase will approve the change and notify both students. After that initial grace period all requests for change will need to be made through the Office for Student Affairs (OSA) and will only be granted in exceptional circumstances. 5. If you are unable to attend a session because of illness or other unanticipated reason then you need to call the OSA 24-hour helpline at (305) 348-0696 and notify the on-call Dean as soon as you realize that you will not be able to make a session. You must also contact the ED Department at the respective site and ask to speak to the Attending in charge and notify him/her. Phone numbers of each ER are in the syllabus. You will need to make up this session contact Ms. Chase for instructions. 6. Following each session you will need to complete a patient log in eValue, which will summarize

your clinical experience in the ED. This log needs to be completed within 48 hours. After that time you will not be able to enter patient records. You will be considered to have an unexcused absence, you will not be credited for having attended that session and you will need to repeat the session and also do an ADDITIONAL session to make up. This log should NOT include any patient identifying information such as name, hospital ID etc. 7. At the sites you should identify yourself as an FIU medical student in all encounters with patients and staff at the rotation site. Do not give the impression that you are the primary caretaker of the patient. Wear your FIU white coat and FIU and Hospital ID and introduce yourself as a medical student working with Dr. X when you first talk to the patient or family. NEVER introduce yourself as Dr. _ 8. Students will demonstrate sensitivity and responsiveness to a diverse patient population, including but not limited to diversity in gender, age, culture, race, religion, disabilities, and sexual orientation. 9. Students will demonstrate compassion, integrity and respect for others and will perform all duties in a professional manner with emphasis on punctuality, organization, professional appearance and demeanor. 10. Students will maintain strict confidentiality regarding all matters of patient care. You must NEVER remove from the ED or related services any chart or any paper or anything in copied or electronic form which has identifiable patient information. While you should take notes for your logs, do not have identifiable patient information on them i.e. name, address, medical record number etc. 11. Students will always act in the best interest of the patients and their families and demonstrate respect for patient privacy and autonomy.

SCHEDULEEach student will be assigned to a rotation schedule and responsible to report to the physician in charge at the ED. Rotation schedules occur on Friday evenings and Saturday mornings or evenings, depending on the site and are 4 hours in length. Students can elect to stay for a maximum of 8 hours with approval of the attending physician. PREREQUISITESCompletion of site-specific HIPAA training, Certificate of Good Health, PPD, Blood Born Pathogen and Universal Precautions Training. All students should have completed the orientation required by each site and received ID badges where required. Students must complete site requested paperwork in a timely fashion before going to site. If a session is cancelled because of a student not submitting requested paperwork it will be considered an unexcused absence.

ATTENDANCEAttendance at Emergency Department Preceptorship rotations is MANDATORY (i.e. you MUST show up for your assigned shift at the correct site and the correct time). You cannot make switches with other students unless this has been approved in advance by Office of Student Affairs. If you are scheduled for a rotation and it appears on short notice that you cannot attend, for example, because of illness or other last-minute reason then you must: 1. Contact the OSA 24-hour line at (305) 348 0696 and report to the on-call Dean. You must also contact the ED Department main number at the respective site and ask to speak to the Attending in charge and notify him/her. You will need to make up this session contact Tatiana Chase for instructions. 2. When you return to COM you must contact Tatiana Chase in the Office of Medical Education (HLS-II 490), in person, by phone (305-348-2376) or by email ([email protected]) and schedule a make-up session. If you have an UNEXCUSED absence from a scheduled session it will result in: 1. Your needing to make this up at a later date, quite possibly during vacation since the schedule is very full. You will also have to do an additional shift i.e. if you miss one session you will have to make up two. 2. You will have an Incomplete grade for Clinical Skills until such a time as any rescheduled session is made up 3. Two unexcused absences in a semester, even if remediated, is grounds for failure of the course

GRADINGGrades will be assigned to this experience based on attendance and completion of case logs. This grade will be part of the total grade assigned in the Clinical Skills I or Clinical Skills II course. Absences from a rotation for whatever reason will result in an incomplete grade for the entire Clinical Skills course until such a time that the missed session(s) are remediated.

REQUIRED READINGPrior to rotating at Baptist Health students are required to view Dr. Andrulonis short PowerPoint presentation on patient communication. This will be circulated to all students prior to the schedule starting and will also be available online. This presentation has information which is well worth reviewing and would be useful at any of the sites. There is no other required reading for this course. However, since one of the objectives is to reinforce what is being taught in the Gordon Center, you should be up-to-date on your current Gordon Center material to gain the most from this experience. Patients seen in the Emergency Department commonly are sent for imaging studies. SITES USEDPlease note that students may not rotate to any clinical site in which they have not yet completed the necessary paperwork/orientation. No exceptions. Students not in compliance with required immunizations will also not be allowed to participate in clinical activities. If you have any questions on this please contact Tatiana Chase in the Office of Medical Education on HLS-II 4th Floor. If you cannot attend a scheduled session because of your failure to comply with immunization requirements or submission of paperwork you will be considered to have an unexcused absence. This will need to be remediated by completing TWO sessions for every unexcused absence.

DIRECTIONS TO SITESBAPTIST HOSPITALAddress: 8900 Kendall Drive, Miami FL Main ED Phone: 786-596-1960 Press 3 Baptist Hospital is located at 8900 South Kendall Drive, south of the FIU Campus. The easiest route from Campus is to go south on 107th street (east side of campus) until Kendall Drive, turn left and the hospital is a little distance down on your right side. Alternatively you can go east on 41 (Tamiami Trail) from the main Campus entrance, get onto 826 South (Palmetto) and then the Don Shula (874) and go down to the Kendall Drive exit . Turn left onto Kendall Drive and Baptist Hospital is a short distance along on your right hand side. Emergency Department is to the left of the hospital campus as you look from Kendall Drive. There is free parking in front of hospital or in parking garage adjacent to ED. Enter ED main entrance and report to the Attending in Charge.

BROWARD GENERAL HOSPITALAddress: 1600 S. Andrews Avenue Fort Lauderdale, FL 33316 Main ED Phone: 954-355-5701 From campus the best route to Broward General is as follows: 1. Turnpike North to I595 East 2. I-595 East to 84 East go along a few miles. Once you go over the railroad tracks in Ft Lauderdale, you go one block and turn left at S. Andrews Street. Go up a few blocks and you will see the main hospital entrance and the parking on your right hand side The distance is 27 miles going the above route should take you about 45 min. Be aware if you go up I95 from downtown on an evening you might have significant delays. The turnpike is usually a quicker option although it may be longer in miles.

Directions Cleveland Clinic Weston is an easy ride up the Turnpike and then 75 North. It is 22 miles and will take you about 25-30 minutes. Take the Turnpike to 75 North and then get off 75 North at 13B (Griffin Road West) the second of the Griffin Road exits. Go to the light at the corner of Weston Road (very close to 75 exit). Go up a mile or so and the hospital is on the right side enter via South Commerce Blvd it is well signed. (If you come to Royal Palm Blvd you went past it). The ED is on the ground floor to the left of the front of the building as you look at it. Enter and report to the physician in charge.

JACKSON NORTH MEDICAL CENTER 160 Northwest 170th Street, North Miami Beach, FL 33169 {Note North Miami Beach is not on Miami Beach} but is near the intersection of I-95 and the Turnpike (Golden Glades)

The easiest route to Jackson North Hospital from Campus, which will avoid the persistent construction on the Dolphin (alleged) Expressway and I-95 North congestion, is to take the Florida Turnpike North from campus. Once you get on the Turnpike North go about 22 miles to the second Toll plaza. At this plaza take the leftmost lane (Stadium) and this will lead you in the direction of the Golden Glades Interchange. After the next Toll plaza take Beaches direction (left of highway) and it will drop you right off at 167th St. Jackson North is situated 1 block behind the light. If you are in the leftmost two lanes just go straight ahead at this light and you will come to JNMC on your right side. Plenty of free parking close to the Emergency Room. Park and enter the emergency room where you will see a Security Guard at the desk. He will buzz you into the Dept where you should report to the Attending in Charge.

Here you will go to the Security Office (near to the Emergency Room) and turn in your drivers license in exchange for a temporary badge. Once your ED shift is complete, you will return to the Security office, return the temporary badge, and receive you drivers license. The shuttle will then drop you off at the parking lot you parked in. The Security Office and Shuttle service operates

24 hours/7days a week, so students will not have any problems with getting the badge or traveling between the hospital and the parking lots.

MERCY HOSPITALAddress: 3663 South Miami Avenue Miami, Florida 33133 Main ED Phone: 305-285-2171 From Campus the quickest (but not most direct) route would be to go up to 836 and take east to I-95 South and approach from north. Alternatively just go east on 8th street from front of campus then go south on SW 27th avenue to Dixie Hwy and approach from the south. Shorter but many more lights. There is parking garage on your left just after you enter property (1st garage is for employees, 2nd for visitors). Use the second (well marked) which will cost flat rate of $3. , come out of garage and walk to your left towards the hospital and Emergency Room. Report to the Attending in charge.

MIAMI CHILDRENS HOSPITALThis is situated on 3100 SW 62nd Ave, Miami, FL and is about 20 minutes SE from the FIU campus. Main ED Phone: 305-669-6543

From Campus East Exit (107th Street) go South to Coral Way and then go East on Coral Way to SW 62nd Avenue this leads directly to Miami Childrens Hospital. As you come onto the campus the ED is well signed on the left side. There is plenty of free parking on your right side and also if you continue past the ED to the T-junction and turn left. Some parking is reserved for staff but the free parking should be obvious. Go into the Hospital, identify yourself at the main desk, where they will likely make you a sticky label with your picture. Wear this and your FIU ID on your white coat. Report to the ED reception desk and ask for the Attending in charge.
