Driving Innovative Care

Millennium Collaborative Care is supporting the development of new quality

improvement interventions through the Innovation Fund Program.

Recently, Niagara Falls Memorial Medical Center was awarded funds for several

Innovation programs and has been achieving solid results since January. Sheila Kee,

Executive Vice President & Chief Operating Officer recently sat down with

Millennium’s Steve Nelson, Director – VBP and Performance to discuss how

NFMMC is putting Innovation funding received from Millennium to work on behalf

of NFMMC's Medicaid patients.

Patient Quality of Care Across WNY Medicaid

Population

On any given day, a percentage of Millennium's 279,000+ Medicaid clients access

healthcare services across Western New York.

Whether it be a planned visit to their doctor for an annual physical; a follow-up lab

test; an x-ray or MRI; medication refills at their local pharmacy or emergency room

treatment – to name just a few – each interaction generates records, reports,

medical images, doctor’s orders, billing statements and more! CLICK HERE to

ENHANCED PATIENT SERVICE UPDATE

Patient Success Story: "Finding the Light... "

Recently, members of Millennium’s Enhanced Patient Services Team, including

Community Health Worker Ebony White, attempted to visit a patient home as part

of the Team’s outreach priorities.

After arriving at the home and becoming immediately concerned about the safety

of the surroundings, Ebony brought in additional support services to offer

additional “helping hands.” The outcome? Thanks to Millennium’s Enhanced

Patient Services Team, a previously unconnected patient was united with the health

care and social support services needed.

DSRIP UPDATE

Supporting Maternal and Child Health;

Reducing Premature Births

Niagara Falls Memorial Medical Center’s women’s services offers high quality health and

wellness services for women of all ages.

The primary objective of the Delivery System Reform Incentive Payment Program

(DSRIP) is to promote community-level collaborations and focus on system reform;

specifically, to achieve a 25 percent reduction in statewide avoidable hospital use

over five years.

Recent Millennium DSRIP project highlights contributing to this goal include

significant advancement of initiatives linked to the primary effort to

provide “Support for Maternal and Child Health; Reduce Premature Births”.
