that participation in athletic events necessarily involves risk of physical

injury. In consideration for accepting the registration of the above named

individuals and permitted the voluntary participation of said individual in itsprograms, I hereby release, discharge, and hold harmless Randy Nesbit,

First Baptist Concord, Concord Christian School, and all their respective

ministers, employees, volunteers and other representatives from any

claims arising out of or in relation to any physical injury that may result to

said individual while participating in summer sports camp events, including

any physical injury by negligence of anyofficial, referee or coach whileperforming his/her duties during any practices or games.
