SHBP 2016-Carrier Options2016 Vendors and Plan Option Offerings: SHBP will continue to offer Blue Cross Blue Shield of Georgia (BCBSGa), UnitedHealthcare and Kaiser Permanente (KP) plan options for 2016.

How the HRA Plans WorksThe HRA provides first-dollar coverage for eligible medical and pharmacy expenses and is funded by SHBP. When going to the doctor, you will not pay a co-payment. Instead, you pay the applicable deductible and co-insurance. If you have remaining wellness credits in your current HRA account, those credits will roll over to the next Plan Year. Bronze, Silver and Gold options with different levels of coverage

All services are subject to the plan deductible and/or coinsurance, no copays apply

The medical and pharmacy out-of-pocket maximums are combined

SHBP contributes HRA credits to your HRA account based on the plan option and tier of coverage you choose These credits provide first dollar coverage for medical and pharmacy expenses Able to earn additional credits with wellness activities

HRA credits left at the end of the year will roll over to the next plan year

How the HDHP WorksThe HDHP offers in-network and out-of-network benefits, and has a low monthly premium. However, you must satisfy a high deductible that applies to all covered medical and pharmacy expenses (except preventive care). How the Statewide HMO WorksAn HMO allows you to receive covered medical services from in-network providers only (except for emergency care). It is important to verify your current provider is in-network when selecting an HMO Plan Option.Available through BCBS or UHC Rates differ between the 2 carriers

Well-Being Incentive Credits Now able to rollover well-being incentive credits across vendors and plan options

2015 rollover credits available in April 2016

For Blue Cross Blue Shield of Georgia (BCBSGa) and United Healthcare Elections: Health actions must be completed between January 1, 2016 and December 15, 2016 in order to earn the well-being incentive credits. Members who elect BCBSGa and United Healthcare will have access to a variety of Healthways’ tools, activities and services such as the

Well-Being Assessment, wellness incentives, Well-Being Coaching, Biometric Screenings, and on-site activities. Healthways will also administer 2016 wellness incentives that will allow you to earn credits for offsetting certain health care costs. Members can earn up to 480 well-being incentive credits toward medical expenses, plus an additional 480 for a covered spouse (total of

960 per household)

For Kaiser Permanente (KP) Election: Members that elect KP will have access to a variety of their tools, activities and services such as the Total Health Assessment, Biometric

Screenings and Online and On-site Healthy Living Classes. As a part of the KP Wellness Program, if you sign up on kp.org and complete all four health activities (e.g. completing a biometric screening,

completing an age/gender appropriate preventive cancer screening, completing one online health education class), you will receive a $240 Visa gift card (up to $480 per household for you and your covered spouse).

State Health – 2016 PremiumsThe Forsyth County Board of Education pays $49.38 for all employees participating in the health insurance program through the State Health Benefit Plan. Any premiums in excess of the $49.38 are listed below and will be deducted from your monthly paycheck.

State Health Open EnrollmentOpen enrollment is from October 19th (opens at 12:00 a.m.) – November 6th (closes at 11:59 p.m.) Members must make their health election at www.mySHBP.adp.com If you do not login to enroll or make changes, you will be defaulted to your current plan, coverage tier and tobacco status. If you are not currently covered and do not enroll, you will continue with no coverage.You may go online as many times as you like but the last election confirmed at the time OE closes will be your election for the 2016 Plan Year.You should print and keep a copy of the confirmation page which will contain a confirmation number - once OE is closed, you will be able to go online at www.mySHBP.adp.com and view your 2016 election

Voluntary Life with AD&DYou may elect Employee Voluntary Life and AD&D coverage in increments of $10,000 up to a maximum of $500,000 or 5x your annual salary, whichever is less. Employee Guarantee Issue:$220,000 or 5X Salary , whichever is Less

You may elect Spouse Voluntary Life and AD&D coverage in increments of $5,000 to a maximum of $100,000 or 100% of your combined Group Life and Employee Voluntary Life amounts, whichever is less. Spouse Guarantee Issue: $25,000

You may also elect Child Voluntary Life and AD&D coverage in increments of $2,000 to a maximum of $10,000 without electing voluntary life coverage on yourself. Dependent Guarantee Issue: $10,000

If you currently have coverage you may increase coverage by $10,000 up to the guarantee issue amount at each annual enrollment with no E of I. Spouse elections and increases require EOI. All Child is GI, EOI not required. Payroll Deductions:

This year you MUST enroll or decline in the FSA plan/Section 125 online (NO LONGER A PAPER FORM) as well as verify your and your dependent information, make changes to your dental, vision, life and disability coverage and review/update your beneficiaries for life insurance. Please Remember… To make your medical elections you will log onto myshbpga.adp.com & pre-register if needed. Otherwise, follow the

instructions starting on page 6 of the State Health Decision Guide.

If you are currently enrolled and do not go online and make an election you will be default enrolled in your current plan, coverage tier and tobacco status. If you are currently declined and you do not go online and make an election, you will remain as “declined”. (Any applicable new plan year premiums will be applied.)

Your FSA elections do not roll-over. You are required to go to the Forsyth bswift online enrollment portal (www.forsyth.bswift.com) to elect or waive the FSA/Section 125 Plan, verify your personal & dependent data as well as re-assign/confirm your Beneficiaries for Life Insurance even if you are not making changes.

Your deadline to enroll is 11:59 p.m. on November 6th .
