New CMS 1500 Claim Form
Reason for change:• The revised CMS 1500 (08/05) claim form accommodates the reporting of the National
Provider Identifier (NPI) number. The NPI must be used by all HIPAA covered entities, such as health plans, health care clearinghouses, and health care providers.
No Barcode:• The barcode has been removed on the revised CMS1500 form. Scanners can be
programmed to read the 1500 symbol in black ink.
Recommended transition timeline:
• October 1, 2006: Health plans, clearing houses, and other information support vendor's should be ready to handle and accept the revised (08/05) 1500 Claim Form.
• October 1, 2006 – March 31, 2007: Providers can use either the current (12/90) version or the revised (08/05) version of the 1500 Claim Form.
• April 1, 2007: The current (12/90) version of the 1500 Claim Form is discontinued; only the revised (08/05) form is to be used. All rebilling of claims should use the revised (08/05) form from this date forward, even though earlier submissions may have been on the current (12/90) 1500 Claim Form.

New UB04 Claim FormWhy is it changing?
• The UB04 contains a number of improvements and enhancements that include better alignment with the electronic HIPAA ASC X12N 837-Institutional Transaction Standard. The New UB04 paper billing form will be able to accommodate the reporting of the National Provider Identifier (NPI) Number. The NPI will be a single provider identifier, replacing the different provider identifiers healthcare systems currently use for each health plan with which you do business. The NPI Identifier, which implements a requirement of the Health Insurance Portability and Accountability Act of 1996 (HIPAA), must be used by all HIPAA covered entities, which are health plans, healthcare clearinghouses, and healthcare providers.
What are the timelines for the transition of the new UB04?
• March 1, 2007: Health plans, clearinghouses, and other information support vendors must be ready to handle and accept the new UB04 Billing Form.
• March 1, 2007 – May 23, 2007: Providers can use either the current UB-92 or the new UB04 Billing Form.
• May 23, 2007: The UB-92 is discontinued; only the new UB04 billing form is to be used. All rebilling of claims should use the UB04 billing form from this date forward, even though earlier submissions may have been on the UB-92.

Fujitsu and Kofax VRS solutions
• Kofax VRS software is a great compliment to the complete line of Fujitsu document scanners
• VirtualReScan (VRS) software and hardware performs image clean-up and advanced image processing of scanned documents
• With the ability to capture images at rated speeds up to 300dpi (dots per inch) AND in color, Fujitsu high-speed scanners make your overall scanning process much more efficient
• VRS can be used in conjunction with other in box application software including Adobe Acrobat 7.0 and ScandAll21

Benefits of Three Independent Ultrasonic Sensors
Healthcare Insurance Form example
Bob,Please review
The left Ultrasonic sensor can be configured completely OFF or to ignore only specific areas of a document. In this case, set to ignore the area of the document that contains the image of the patient where clinic has taped the photo to the page.
The middle Ultrasonic sensor is configured to detect multifeed documents. No multifeed detected on this example therefore scanning would continue normally.
The right Ultrasonic sensor also has been configured to detect any multifeed documents. In this case a yellow sticky note is detected and the scanner can be setup to notify the operator and stop scanning and/or pause scanning in order to immediately take corrective action. Important information under the sticky note is not lost.

Optical Character Recognition
NO. While it is legible to the human eye, the quality is too poor for use as an archival master file. The characters are too pale and
uneven to OCR properly.
CausesPoor Camera Technology / Inadequate Lamp Brightness
NO. This scan is easier to read than the one above, but the characters have bled into one another and there is too much
"noise", or interference like the black marks surrounding the text, to OCR the image.
CausesLack of Image Processing / Inefficient Image Enhancement
YES. This scan is easily legible, looks like the original page, and will OCR without difficulty.

Back-File Conversion vs. Day-Forward
Batch Mode Capture
Pros Cons
Back-File Electronic access to all paper files
Document sharing, reduce lost files
High level of customer satisfaction
Time-consuming
Storage costs
Bandwidth considerations
Day-Forward Quicker implementation
Clear-cut process for scanning
Inconsistent access to information
Possible lack of compliance
As Needed Build your repository over time
Only relevant information in system
Poor disaster recovery
Lack of automation and access
Single Mode Capture

MFP vs. Production Scanner Considerations• MFP used primarily in Ad Hoc scanning• Production Scanners used in Mission Critical scanning applications
• When to use a dedicated Production Scanner?• Basic Criteria• Daily scanning volume greater than 100 documents per day• Mixed paper sizes, weights and colors
• Forms Processing applications• Optical Character Recognition (OCR), ICR, color dropout• Image Enhancement, decreased document preparation
• Vertical Market Applications• Healthcare – HICFA forms, Medical records, Plastic cards• Legal – Sarbanes Oxley, Check 21, Compliance• Banking – Loan documents, Lease forms• Government – Homeland security, Local/State agencies• Human Resources – Applications, Reviews, Records• Real Estate – Deeds, Wills, Trusts, Affidavits• Transportation – Airbills, Proof of Delivery, Signatures• Service Bureau – Backfile conversion, Outsourcing
Production Scanning = Speed,Paper Handling, Image Quality

Forms Dropout Color Example
• Sample claim form with excellent form structure including red constraint areas to be eliminated
Dynamic Color Dropout
ICR on handprint
OMR on check marks
Deskewed Image
Image Enhanced
• After Dynamic Color Dropout only the relevant form data is exposed for ImageNow Forms Processing
Fujitsu & Artsyl, The Importance of Accuracy in Medical Forms Processing
Forms Design Recommendations
• Well defined Forms enable a much higher level of automation
– Form structure itself can aid in increased productivity
– Color Dropout eliminates constraint boxes for scanning
– Background color is unnecessary and not recommended
– Paper type should be consistent
• Give automation the best possible chance to succeed
– Higher resolutions (300 dots per inch for OCR/ICR)
– Image enhancement (Deskew, cropping, auto-orientation, etc.)
– Paper handling
• Take your forms design seriously
– Many form types are fine, structure is most important
– These documents most likely will be needed for years

Challenges of ‘EOB’ processing
• Physical document format– Small font size– Poor print quality
• Time consuming• Labor intensive• Access to information• Form structure• Index fields• Error prone
– Incorrect index– Inaccurate totals
• Lost/misplaced documents• Volume of transactions

Camera Technology
• Overall comparison summary:• CCD – high cost, best image quality, low power efficiency.
• CMOS – costs less than CCD, better quality than CIS.
• CIS – low cost, lower resolution, but good image quality.
