• The current long term services and supports (LTSS) system involves numerous funding streams administered by multiple federal, state and local agencies resulting in fragmented and duplicative processes.
• Individuals in need of services, often at times of crisis, are faced with multiple requirements such as screening, intake, needs assessments, service planning, and eligibility determinations.
• 70% of Americans who reach age 65 will need some form of long-term care for an average of three years.

No Wrong Door Systems• No Wrong Door (NWD) Systems provide the coordinated infrastructure to support the collaboration of local service organizations, making access to critical services and supports more efficient and person-centered.
• All 56 states and territories have transformed some function of an LTSS access system. States are at different stages of implementing a NWD System - training staff and restructuring access points around person-centered principals, streamlining programs and eligibility, and increasing coordination between agencies and partners.
• 33 grants issued for planning and implementation of NWD Systems.

Purpose and Goals – 2018 Investment• New NWD funding opportunity focuses on measuring and quantifying the impact of NWD Systems.
• Funding up to 9 states
• Four primary goals:1. Assess current efforts in business case development.
2. Develop a methodology and toolkit for calculating a return on investment (ROI).
3. Share with ACL key data elements needed to test and calculate ROI.
4. Produce a business case model that demonstrates the impact of NWD Systems.

• Matching funds are not required.
• Applicant must provide a letter of support from the Office of the Governor and/or a Cabinet Level official from the state agency responsible for the administration of programs that serve older adults, individuals with disabilities and caregivers.

Expectation #1• Grantees would work with ACL to identify, develop and capture data elements at the individual, program and organizational level needed to demonstrate ROI calculations for NWD Systems.
• Grantees shall have a systematic data collection methodology that captures qualitative and quantitative data elements.

• Care transitions from hospitals• Care transition from nursing homes• Veteran Directed Home and Community Based Services (VD-HCBS) program• Pre-screening/nursing home diversion programs
• Grantees must identify a geographic area(s) for the evidence-informed model(s) and provide an estimated number of people served through this model during the grant project period.
• Grantees must have a letter of support from one or more health care institutions within the first quarter of the grant period
