It is best completed with the free Adobe Reader application. You may download it from get.adobe.com/reader. This form is a contract between you and the university. The Graduation Evaluation office will expect every course on this form to appear on your transcript. If you diverge from the courses on the form after it is sent to the Registrar, see the Environmental Studies Chair right away. Only courses that are part of your major appear on this form. GE courses that are not included in the preparation, core, concentration, or electives for your major do not go on this form. If you have an Environmental Studies minor, fill out a new, separate Major/Minor form. Your minor courses will go on that second form. So you will have two of these forms filled out – one for the major and one for the minor. If you have a minor in another department, those courses do not go on this form. You will need to fill out a minor form with that department. Bring your completed and signed minor form, as well as your graduation application, to the Environmental Studies Chair to send to the Registrar. After you complete the Major/Minor form, make an appointment with the Environmental Studies Chair and bring it on a flash drive, so when you see the Chair, errors can be fixed right then so that your form can be sent in immediately. SPECIFIC NOTES 1. At the top of the form, fill out your information, including the semester and year you expect to graduate, your concentration, if you have one, and your minor, if you have one. The minimum number of units for both the BS and BA is 120 units. 2. The “Courses Completed” boxes on the left side of the form are only for courses for which you have a grade. In the top box, Box A, type the information for all Environmental Studies core, concentration, and elective courses you have completed for your degree. Abbreviate course titles if it does not fit. Do not forget the units and grade for each course. If you took a course at another institution, use the number and name of the course at that college. After the name of the course, put an asterisk. In the line at the bottom of this box, you will see an asterisk. Put the name of the institution where you took the course. If you took courses at more than one institution, list the name of the other institution on the second column with the double asterisk. 3. In Box B, “Courses Completed – Preparation,” put all the preparation courses. If you took courses at another institution, see the directions in #2, above. 4. In Box C, “Courses In Progress,” put all the courses – Environmental Studies and Non-Environmental Studies – that you are currently taking. If it is summer and you have registered for Fall courses, put these here. 5. Box D, is for Environmental Studies courses you need to take to complete your degree. Box E is for Non- Environmental Studies courses you need for the major (for example, BIOL 1A). 6. Put your name and curriculum at the bottom, where required.
