The objective of this document is to provide necessary guidelines to Learning Centers to facilitate them in the verification of Fresh Admission forms and the eligibility documents of Fall (August) 2011 Session.

The Learning Centers are advised to follow the suggestions as given below at the time of verification of eligibility documents and admission forms. This would help in smooth processing of the admission at SMU-DDE without any delay.

Last Date for Submission by LC: The SMU-DDE will allow a maximum transit period of SEVEN calendar days (inclusive of holidays) for the Admission forms to be received at Manipal, with fees and all necessary support documents. SMU-DDE is not responsible for any postal delay.

Any Admission form received at SMU-DDE after the last date for submission will attract late fee or rejection, as applicable.

II. Application or Admission Status on the WEBSITE.

The fresh admission process status would be uploaded on daily basis on to the SMU-DDE website www.smude.edu.in. The Learning Centers and the candidates are required to verify the same from time to time to know the application/admission status. The learning centers can

access the information under LC login. The candidate can refer to the status on homepage by giving the Application No. Please refer to the website links as provided below.

(a) Center login – List of Students Section – ‘Admission Status – Fall 2011’.

(b) Home page – Apply Online – ‘Fresh Application status for student Registration’.

Please note, no written communication will be sent to the learning centers or to the candidates. The learning centers are hereby advised to refer to the website for up-to-date admission status and to follow up with the aspiring students wherever necessary in case of pending applications due to shortfall documentation.

The refund of fee will be made after deducting the processing charges as prescribed in the case of rejected applications. Refund will be made through demand draft in favour of the Candidate only. The refund will be processed after the closure of admissions and hence, LC is required to appraise the candidates accordingly.

No cancellation and refund is applicable against the confirmed admissions. So, LC is advised not to entertain and forward such requests.

For timely communication, it is the responsibility of the learning center to provide the information like Address, contact phone numbers, etc., to keep the Learning Center profile up-to-date at university end. This would help all the concerned staff at SMU-DDE to communicate with the Learning Center in case of an emergency.

Any communication through email will be provided to the university email ID issued by SMU-DDE to the learning center, so, it is the responsibility of the learning center to refer to the university email ID for all correspondence.

III. Points to remember during document verification

Whether the candidates have used the current session admission form or not.

If any important columns left blank (Qualifications, Experience, etc.) has to be rectified with help of the candidate before forwarding to SMU-DDE.

Note:

Admission form of previous sessions should not be considered.

Photocopy of the admission form should not be considered.

Incomplete admission form (without mentioning details like Name of the Candidate, Father’s/Mother’s/Guardian’s Name, Date-of-Birth, Nationality, Program Title, Semester, LC Code, Payment details, Declaration by the Candidate, Declaration by LC Head, etc.) should not be accepted.

Use of Whitener should be avoided.

The same is liable to be rejected without any notice. LC is supposed to check the form before forwarding to university for admission, otherwise, LC will be held responsible for rejection of such application.

Whether the applicant name mentioned in the admission form and the name mentioned in Mark cards and Certificates enclosed along with the admission form is same or not is to be verified.

Note:

The Learning Centers should ensure that the application is properly filled by the Candidate. Extra care is to be taken while writing letters like ‘ U ’, ‘ V ’, ‘ I ’, ‘ T ’, ‘ M’, ‘ N ’, ‘ D ’, ‘ O ’, etc.

If case of variation in the name the following steps to be followed:

2.1 If Candidate is applying for Diploma or Bachelor or Master Degree Program

Enclosure of affidavit along with the admission form: Name variation, if any, in the 10th Mark Card or +2 / Diploma Mark Card or Degree Certificate/Mark Card (even minor difference), a duly notarized Affidavit (in English) clearly mentioning the correct name to be incorporated in University records while granting admission as well as that these documents pertain to one person, has to be produced.

In case of change of name after marriage, the marriage certificate is to be attached with the admission form. If marriage certificate is in regional language other than English, an affidavit mentioning the name to be incorporated in the records of the University is to be provided along with the admission form. The name mentioned in the affidavit is to be mentioned in the admission form.

Name variation, if any, between the admission form and the eligibility documents, the name as per the 10th Standard or Date of Birth certificate will be considered for admission process unless the candidate has submitted Application for Name Change request with other necessary support documents along with the admission form.

Note:

Photocopy of the affidavit should not be considered.

No name change request will be accepted once registration process is completed by SMU-DDE.

3 Father’s/Mother’s/Guardian’s Name

Father’s name mentioned in the admission form should be as per 10th standard mark card.

In case of any mismatch in the name an Affidavit (in English) is to be produced mentioning the correct name. Without support documents Father’s name will be considered as per the 10th Standard mark card/ certificate.

Note:

If father’s name is not mentioned in the 10th mark card other support document having father’s name like School Leaving Certificate/ 12th mark card/ Bachelor Degree mark card can be produced.

4 Date-of-Birth

The Date of Birth filled in the admission form has to be considered as per the 10th standard mark card. If it is not mentioned in 10th mark card other document (like Date

of Birth Certificate, School Leaving Certificate/ 12th mark card/ PAN Card, Passport copy) which is having Date-of-Birth mentioned in can be produced as support document.

5 Candidate’s Correspondence Address

The candidate address for correspondence should be mentioned in the columns provided, clearly mentioning the complete address with City, Pin code, Contact Number and e-mail address, etc as required for mailing purpose.

Note:

The Learning Center address should not be mentioned under this column. The same is not recommended, whatsoever may be the reason.

6 Learning Center Code and Name

Whether the candidate has mentioned in clear and readable form -

a 4 digit LC Code for IT & Management Programs

a 5 digit LC Code for Allied Health Science programs

7 Program and Semester

Whether the Program code and along with the Semester applied is mentioned or not in the columns provided in which the Candidate is seeking admission is to be verified.

Note:

Candidate may be advised to take necessary care while filling the details as given below.

The semester applied for is to be mentioned clearly in words like FIRST / SECOND/ THIRD, etc. Notations like 1st, 2nd, I, II should be avoided.

If the Candidate is applying for Lateral Entry, Re-entry, Direct Entry a box provided for Lateral Entry, Re-entry, Direct Entry should be properly ticked.

The admission will be given based on semester mentioned in ‘Semester Applied for’ column. If the candidate filled Semester Applied for is ‘FIRST” and the tick mark against Lateral entry column is ‘yes’, the fee paid for Lateral entry, the admission will be granted only to FIRST Semester due to the discrepancy. The LC has to be cautious in verifying these details.

8 Payment Mode

Following are the facilities available for payment of Fee for Fall (August) 2011 Session -

o SBI Challan

o Demand Draft

o Credit/Debit Card of MasterCard or VISA

Multiple modes or Single-multiple modes of payment by a candidate should be avoided.

(a) Semester Fee (Tuition Fee + Examination Fee + Alumni Fee)

(c) Late Fee (if applicable)

(d) Application Fee

(e) Prospectus Fee

(f) Certificate Fee(if applicable)

Example:

1) a Candidate seeking admission into MBA, FIRST Semester has to prepare a DD of Rs.13525/- i.e. {Tuition Fee (Rs.12,100)+ Examination Fee (Rs.950) + Alumni Fee (Rs.100) + Application Fee (Rs.300)+ Prospectus Fee (Rs.75)}

2) a Candidate seeking admission into Lateral Entry to MBA, SECOND Semester, has to prepare a DD of Rs.19,725/- i.e. {Tuition Fee (Rs.17,350) + Examination Fee (Rs.1,900) + Alumni Fee (Rs.100) + Application Fee (Rs.300) + Prospectus Fee (Rs.75)}

3) a Candidate seeking admission into MBA, FIRST Semester with late fee of Rs.500/- has to prepare a DD of Rs.14,025/- i.e. {Tuition Fee (Rs.12,100) + Examination Fee (Rs.950) + Alumni Fee (Rs.100) + Late Fee (Rs.500) + Application Fee (Rs.300) + Prospectus Fee (Rs.75)}

Note:

Admission form with short fee is liable to be rejected.

Payment made other than in the above forms will be at own risk of the candidate.

Payment of fee made through Cheque, Consolidated DD or any other mode other than the prescribed are not acceptable.

LC will be held responsible if the fee is accepted in the form of cash or cheque from the candidates. A strict action will be initiated against such learning centers.

LCs are advised to explain the following procedures to the candidates:

8.1 Payment through - SBI Challan

The payment can be made through SBI Challan from Core Banking Branches of State Bank of India.

1) The Candidates have to make enquiry with SBI local branch whether it is a core Banking Branch or not. University will not take any responsibility if the amount is deposited in non-core banking branch of SBI and shall not entertain any correspondence in this regard.

2) Specially printed SMU-DDE SBI Challan forms made available at the Learning Centers are to be used.

3) Photocopy of the SBI Challan should not be used.

4) Payment through only one Challan per candidate is permitted. Candidates have to mention the Application form number in the Challan without fail. Partial payment of fee through Challan is not permitted.

5) Quadruplicate Challan form to be uniformly filled with all the details without leaving any column blank. Candidate has to mention the LC code and LC need to affix the center seal on all the portions of the Challan.

6) SBI will accept the payment of fee only through cash.

7) No commission is payable by the Candidate for payment through SBI Challan.

8) Any corrections or modifications in the SBI Challan whatsoever the reason will lead to rejection of the admission form.

9) Candidate has to retain Student copy of the Challan for any further reference and enclose University copy with the Admission form without fail.

10) If the payment is made after the last date prescribed by the University, the same will not be considered and the refund will be made as per the rules.

11) Payment made by one candidate is not transferable to the other.

12) Care should be taken to deposit the exact fee due for the Semester. The refund request for the excess fee deposited, if any, through this mode will not be entertained.

8.2 Payment through - Demand Draft

1) Semester fee can be paid in the form of Demand Draft.

2) The Candidate should endorse the Name, Admission form number and Learning Center code on the reverse of the DD.

3) The Demand Draft should be

- drawn in the favour of ‘Sikkim Manipal University DE’

- payable at Udupi OR Manipal.

- Amount (the candidate has to prepare a Demand Draft for the required amount)

The Leaning Center has to verify the following before forwarding Demand Draft along with the admission form to SMU-DDE.

(a) Demand Draft has been prepared for the requisite amount or not.

(b) DD is drawn in favour of ‘Sikkim Manipal University DE’ or not.

(c) Whether the Demand Daft is having proper date. {DD’s with validity date over or DD’s with future dates are not acceptable}.

(d) Whether the ‘payable at’ mentioned is ‘Udupi OR Manipal’ OR not.

(e) The DD not signed by the Banker should not be accepted.

(f) Whether the Demand Draft Number is properly printed or not.

(g) Demand draft of previous session or due for expiry or prior to current session should not be considered for various reasons.

8.3 Payment through - Credit/ Debit Card

1) Candidate can make the fee payment using valid Credit/Debit Card. However, this facility is available only at select learning centers.

2) The candidate has to forward the Credit/ Debit card Merchant copy which is having all the digits of Credit/ Debit Card number printed on it.

3) The Learning Center has to verify the following before forwarding the Credit/ Debit Card payment Merchant copy along with the admission form to the SMU-DDE.

4) The LC should not forward the Customer copy against Merchant Copy along with

the application form.

5) Whether the fee payment has been made for the requisite amount or not is to be verified.

6) Whether the Approval Number (Authority Code), Credit/Debit card number, Date of Swiping, Batch Number are properly printed or not is to be verified.

7) Whether the Credit/ Debit card Merchant copy is signed by the Candidate or not is to be verified. Unsigned credit/debit slip is not acceptable.

8) The use of Credit card/ Debit card for making payment by Learning Center Person or others for Candidate is to be avoided. SMU-DDE is not responsible for such practices and problem arises out of it.

9 Pass Port size Photograph

Recent photograph having good clarity should be used for this purpose. The Candidate and LC have to ensure that the name of the candidate and application number is mentioned on the reverse side of the photograph.

Paste the passport photograph properly at all edges so that the same will not be lost in the transit or in application handling.

Photograph must be pasted exactly to the box provided.

Photograph is essential for issuing of Identity Card, Admit Card and Degree Certificate.

Note:

Application is liable to be rejected in the absence of Photograph.

Old photograph or photograph of earlier period should be avoided, since photo identification may cause problem at the time of examination.

Xerox of photograph must be avoided.

Previously used Photograph must not be used.

All such applications are liable to be rejected without notice.

10 Educational Qualification Details

Verify whether the candidate has clearly mentioned the qualification details in the table provided in Page No.4 of Admission form.

Advise the candidate to mention the qualification details in the ascending order. The order should be

(a) 10th / SSC

(b) 12th / PUC / HSC / Diploma Title

(c) Bachelor’s Degree Title

(d) PG Degree/ Master’s Degree Title (if applicable)

(e) Additional Qualification Details (if applicable)

11 Employment details.

LC has to verify the employment details filled in Page No.4 of Admission form. (wherever applicable)

Check whether the experience certificates (in the prescribed format) have been

enclosed or not with the admission form.

For specimen copy of the experience certificate refer to Fresh Admission Circular –Fall Session.

12 Signature of the Candidate

Verify the signature of the candidate on Page No.3 & Page No.4 of admission form.

Both the signatures of the candidate should match to consider the admission form for processing by SMU-DDE.

Note:

In case of signature mismatch the admission form is liable to be rejected.

The signature pasted on application form is liable to be rejected.

13 Declaration by the LC Head

The Learning Center Head has to sign the declaration in the Admission form only after verifying the eligibility documents confirming the eligibility of the Candidate for the said program.

Declaration by LC head is mandatory with Signature and Seal.

Note:

In the absence of declaration by LC head, the admission form is liable to be rejected.

14 Eligibility Documents.

Before filling up the admission form the Learning Center has to verify the eligibility documents of the Candidate in original.

The Candidates be advised to submit the eligibility documents as per the qualification prescribed in prospectus. Learning Centers are advised strictly to follow the eligibility criteria prescribed in the current session prospectus and other communication as communicate by SMU-DDE from time to time.

The qualification mentioned in previous prospectus/ accepted in previous sessions will not be acceptable in current session. In case of any deviation by the Learning Center a suitable action will be initiated against such center.

14.1 Document Attestation by Gazetted Officer or Notary Public

LC to advise the candidate to submit the eligibility documents duly attested by Gazetted officer / Notary Public.

Attested documents should have the seal & signature of the attestation officer. The seal should contain the name, designation, address. If Name, Designation and Address of the attestation officer are not clear, then the admission form will not be considered for processing.

If the documents are attested by Notary, the seal of the Notary should always contain the Registration number and place of the Notary.

The Headmaster/ Headmistress/ Principal of Government colleges are eligible for attestation of the documents. The seal should contain Name of the person, College/ School Name and Place of the College/ School. The attestation by Headmaster / Headmistress / Principal of Private School / Colleges will not be considered.

Photocopy of attested documents are not acceptable. Attestation on fax copy of the document is not acceptable.

The LC Head has to countersign along with seal having Name, Designation on all the eligibility documents. LC head has to counter sign the eligibility documents only after verifying the same with the originals. This will help in automatic issue of CMC and PPC.

The Name, Seal and Designation details of the attestation should be in English/ Hindi.

14.2 Photocopy of Documents

The following may please be noted with regard to Eligibility documents.

1) The photocopy of all eligibility documents should be clear and readable. Photocopy of the documents from original is advisable for better readability.

2) In case of Mark cards the following details should be present

(a) The Board/ University name

(b) Month and Year of Examination

(c) The name of the Candidate

(d) Course pursued

(e) Semester / Year completed

(f) Pass / Grade remark

(g) Signature, Name, Designation of issuing authority

(h) Seal of the Board/ University

(i) Date of issue

3) In case of Provisional Pass Certificate (PPC) or Degree Certificate (DC) following details should be present.

(a) The Board/ University name

(b) The name of the Candidate

(c) Course completed

(d) Year of completion

(e) Signature, Name (optional), Designation of issuing authority

(f) Seal of the Board/ University

(g) Date of issue

4) The Provisional Pass Certificate (PPC) duly signed by university authority or Degree Certificate (DC) duly signed by the Vice Chancellor will only be considered. Only in case of Autonomous Colleges, the Provisional Pass Certificate issued by the Principal can be accepted.

5) Verify whether all the content of Original document appears in photocopied document as it is. If original document is of the size greater than A4, the photocopy should be taken on A4 size paper by adjusting the size.

6) The mark cards and Provisional Pass Certificate issued by University only be accepted. Mark cards and Provisional Pass Certificate issued by College not acceptable.

7) The mark card and Provisional Pass Certificate signed by Controller of

Examinations/ Asst. Registrar/ Dy. Registrar/ Registrar/ Vice Chancellor of the University/ Board only to be accepted. Mark cards and Provisional Pass Certificate issued by Principal is not acceptable.

8) In the absence of Degree Certificate, the candidate has to attach all the years / semester mark cards along with the Provisional Pass Certificate issued by the University.

9) PGDCA / PGDIT Certificates from recognized University will only be accepted. PGDCA / PGDIT Certificates from computer institutes will not be accepted.

10) In case of Lateral Entry to MCA 3rd semester 2 year Diploma Certificate issued by NIIT/ APTECH only to be accepted. Certificate issued by the institutes where the candidate has studied is not acceptable.

11) Internet copies of the result from the official website of the recognized Board/ Government Colleges / University can be considered for Provisional Admission provided the Candidate is able to produce the document before the last date of Admission. (20th November 2011). The Candidate has to provide an undertaking mentioning his/her admission is liable to be cancelled in case of non submission of the document before the last date. If the internet copy of the result is not clear or not in line with regular mark card or do not provide clear result status will not be acceptable.

12) Only degrees from UGC recognized Universities are acceptable. (The list of recognized Universities is available in AIU Directory publication, UGC website, Dept. of Education, GOI website).

13) As per MHRD letter No. 6-7/2009-DL dated 29th July 2009 and DEC letter No. DEC/2009/2629 dated 11th August 2009 – B.E/B.Tech programs through distance education mode are not recognized and approved by DEC.

14) For fake Universities/Institutions/Boards list refer to UGC website, IGNOU website.

15) In case of Degrees obtained from foreign Universities in India, the Candidate has to provide an equivalency certificates issued by Association of Indian Universities (AIU). A letter from UGC/AICTE on the recognition status of the Degree and the institution offering foreign degrees in India is mandatory.

16) Computer course Certificates/Diplomas issued by the Approved institutes are acceptable. Certificate/Diploma issued on Letter heads and handwritten certificates/diplomas are not acceptable. The certificate/diploma should have the duration, name and designation of the issuing authority. Certificates/Diplomas with course abbreviations are not acceptable.

17) In few of the cases the SMU-DDE ask for Degree Certificate/ Provisional Pass Certificate even though all the semesters/years mark cards are produced. This is done only in those cases where it is not possible to judge whether the candidate has completed the Course or all the Semesters/ years successfully.

18) Bridge Course - The candidates who have successfully completed a 2 year degree on or before 1998-99 from a recognized university are eligible to seek admission to PG program.

19) The candidates who have successfully completed 2 years graduation after 1999 and having one year Bridge Course from any government approved Institution / University after the graduation are eligible to seek admission to PG program.

20) Insufficient documents - In the absence of eligibility documents, the Learning center has to advise the Candidate to give an undertaking by which date (Date should be prior to last date of closure of Admission) they are able to submit the documents to SMU-DDE. If the candidate fails to produce the document on or before that date, the application is liable to be rejected without any intimation.

21) Learning Center is responsible for application with incomplete documentation.

22) One sitting Degree Examination – The degrees of the candidates enrolled for the one-time Bachelor’s Degree program, up to the year 1995-1996 will only be treated as valid.

23) Hindi/ Sanskrit Qualifications – The Hindi/ Sanskrit qualifications from recognized bodies with 2 years duration will be considered for Bachelor’s degree and 3 years duration will be considered for PG degree. In both the cases the candidates are requested to produce proof of duration of the program, equivalence certificate along with the documents.

24) If the candidate has submitted fake certificates/ documents or stated

wrong facts in the admission form, such admission is liable to be

cancelled and fee remitted will be forfeited.

15 Experience Certificate

The three years experience only after the Graduation will be considered for eligibility.

The experience certificate submitted should have the following information

(a) The experience Certificate should be on the letter head of the Company/ Institution/ Firm containing full correspondence address with pin code and contact numbers like Phone No., Fax No., email, etc.

(b) Date of Issue

(c) Name of the Employee

(d) Father’s Name of the Employee

(e) Designation of the Employee

(f) Period of Service

(g) Name and Designation of the Issuing Authority

Note:

In case of any mismatch between the Name mentioned in Academic records and Experience Certificate, an affidavit is to be produced. Marriage Certificate is to be produced if name is changed after marriage.

Any experience certificate not providing the details as required above is treated as invalid document.

Appointment / Offer of Appointment letters (or) salary certificate will not considered as equivalent to Experience Certificate.

16.1 Additional Application form for foreign Nationals.

Foreign students seeking admission to the Distance Learning Programs of SMU should fulfill either of the following requirements.

1) Must be pursuing a regular Course from a recognized University in India.

The Candidate should be undergoing a regular course in a college affiliated to any recognized University in India. The minimum duration of the course should be >= to the duration of SMU-DDE program to which candidate is seeking admission. The candidates have to provide a proof which proves that the college has a permission to run that course under the University System, if required.

2) Should be working in India

3) Should be dependents of a person in India.

Foreign nationals seeking admission in India are required to submit the duly filled in additional application form.

Foreign students seeking admission in India (domestic centers) the eligibility for admission will be governed by the equivalency norms as laid down by the Association of Indian Universities (AIU).

In case of qualifications not reflected in the AIU equivalency list, the candidates are advised to approach AIU for issue of an equivalency certificate. (Refer to Important note, Prospectus).

Candidates who have passed the qualifying examination from a foreign University / Board / Ministry should submit all the relevant information / documents which justify the equivalence of their qualifications.

Mark Cards / Degree Certificates in language other than English should be translated into English language and a copy of the same duly attested to be attached with the admission form.

An attested copy of the relevant pages of the passport containing details of the individual. Passport Number, Date & Place of Issue & Name of the Country issuing the same.

An attested copy of the relevant pages of the VISA containing details of the individual. Any VISA issued in the name of ‘Sikkim Manipal University’ will not be acceptable.

An attested copy of the valid Residential Permit. The validity of Residential permit should cover subsequent examination i.e., January 2012 Examination.

4) International students in India will not be eligible to appear for university examinations without submitting the valid Residential Permit to SMU-DDE.

Note:

SMU-DDE will not issue eligibility certificate to a candidate who wants come to India only to join programs in distance mode.

Foreign nationals seeking admission in India are required to submit the additional application form. The form is available on the website www.smude.edu.in.

The form should be complete in all respects. The admission form submitted along with the incomplete additional form is liable to be rejected.

16.2 Additional Application form for PGDCRRA/MScCRRA.

Candidates seeking admission to PGDCRRA / MScCRRA program are required to fill

up a ‘PGDCRRA/MScCRRA Additional Application Form’.

Students are required to opt for ONLY two optional subjects in each semester at the time of admission as per the criteria and procedure laid down for the program.

Follow the criteria mentioned therein to claim for exemption on elective subjects.

The optional elective subjects once selected cannot be changed.

The candidates are required to enclose all semester/year marks cards or transcripts of all qualifications along with the form.

Based on verification of the documents, the university will accept/rejects the exemption applied for and intimate the same to the Learning Center and the candidate.

17 Format for sending the documents of Problematic / Provisional Admission applications.

The documents of Pending Application/ Provisional Admission cases are to be forwarded along with the summary sheet as mentioned below.

This sheet should be prepared for each candidate separately and documents are to be attached with this sheet.

18 Dispatch of documents of Pending Application / Provisional Admission Application

Learning Center has to forward the documents of Pending Application / Provisional Admission Application in a separate cover clearly mentioning on the cover “Documents of Pending Application / Provisional Admission Application”.

The documents received within the date specified will only be considered.

NOTE: Student admitted with provisional admission status will not be permitted to re-register to higher semester or re-sit to arrear paper(s) till the confirmation of provisional admission.
