VeriChip H2™ Reader can assist

• The VeriChip H2™ Reader is a hand-held Radio Frequency Identification (RFID) reader which activates an implanted VeriMed™ Microtransponder with a low power, low frequency (LF) electromagnetic field.

• The Reader receives a unique 16-digit ID number from the VeriMed™ Microtransponder.

• The ID number is used to provide, with the patient’s consent, physicians, paramedics and other health professionals access to a secure database that will provide the implanted person’s identity and health information provided by the patient.

14

How does it help?

Saves Valuable Time• Vital information is obtained

quickly via password protected, web enabled database.

Decreases medication errors• A patient’s medications, allergies

and diagnosis are easily accessed

through the Patient Registry

Page 15: Gwinnett Medical Center Presentation

15

Patient’s physicians can be notified

• The Emergency Room team can notify physicians that their patient is at the hospital.

• Immediately requesting any specific information about that patient by viewing their Personal Healthcare Record.

Page 16: Gwinnett Medical Center Presentation

16

Patient’s medications will be known

…when they are brought to emergency surgery.

…and when

they are due for

their next dosage.

Page 17: Gwinnett Medical Center Presentation

17

Their loved ones can be called

Page 18: Gwinnett Medical Center Presentation

18

The patient’s caregiver team can be notified

• Before, during and after a hospital stay. Their knowledge of the healthcare arena can assist the patient or their family who’s never dealt with any of these events before.

• They can assist in decisions for patients in regards to paperwork, resources, and implementation of the resident’s choices.

• Decreasing the stress when hospitals discharge patients. Work with discharge planners in the assistance of a patient leaving the hospital, in the transfer to a facility or back home.
