Facilitate more appropriate use of healthcare resources addressing the issues of underutilization and over utilization

Empower participants with the information, skills, confidence, & motivation they need to actively participate in

When to call a HealthyLiving Health Coach

Call for any kind of health care concern or question:

− Decision Support: Your doctor has suggested surgery, and you want more information

You don’t know where to turn next with a complicated medical problem

− Condition Support: Your spouse has a health problem, and you don’t know what questions to ask

your doctor

You have a chronic disease (like diabetes, heart disease, or asthma), and you want to know about treatments

− Symptom Support:

You are trying to decide whether someone in your family needs to see a doctor for symptoms, and what home treatment you might try

If you are in need of symptom support, decision support, condition support and wellness support that is when to call a HealthyLiving Health Coach

Outreach to our participants:

All participants will receive a welcome letter and a magnet introducing the new program

Participants may receive information by mail or a call from a Health Coach

Participants may send secure messages to a HealthyLiving Coach via the web site, which will be answered within 24 hours


The Dialog Center- accessed via the Web

Through the Dialog Center, participants can access:

E-mail and receive secured messages from a health coach

Access to HealthyLiving Behavior Change Modules− Breathe− Relax− Care− Nourish− Balance

Search thousands of up-to-date health articles from top medical experts

Utilize tools to conveniently track prescriptions or medical symptoms

Access to a self-guided, secured Personal Health Assessment


Personal Health Assessment (PHA)

The PHA helps members improve their health

Provides a Personal Action Plan

Provides access to many interactive tools

The Personal Action Plan provides 3 areas where members can make the biggest difference in their health

All in 3 simple steps:

− Read – where you can go on the Web to learn more about health topics

− Talk – Who you should talk to for support

− Do – How to start making healthy changes TODAY


Educate and Support Onsite promotional materials

Onsite promotional materials create brand awareness and introduce the enhanced program to members and

employees.

Materials encourage members to manage their

health conditions and participate in health fairs,

screenings, and other worksite wellness activities.


Helping Participants become Better Patients

A participant’s interaction with the HealthyLiving Program prepares them to make health care decisions in partnership with their physician

The HealthyLiving Program strives to put the well informed participant in the center of the health care decision process resulting in a more effective and efficient participant / provider dialog

Goal: To achieve quality in medical decision-making a participant must be informed and feel confident that the decision matches the values and preferences her or she deems to be important
