NANCY J. BLOOM, PT, DPT, MSOT

In her work as a clinician scientist and practitioner of both physical therapy and occupational therapy, Nancy J. Bloom, PT, DPT, MSOT, demonstrates a commitment to rigorous patient care, and to clarity and precision in thought and writing—traits Bloom shares with the inspiration for this award, Florence P. Kendall.

Bloom is a member of the faculty of the Program in Physical Therapy at Washington University School of Medicine in St Louis. In addition,

she is associate professor of physical therapy and orthopedic surgery at the institution and serves a number of other leadership roles in the program. She holds a DPT and BS in physical therapy, and MS and BS in occupational therapy, all from Washington University. In addition, Bloom earned a BA in psychology from the University of Virginia.

Bloom received one of the first MS degrees awarded by the Occupational Therapy Program at Washington University. She took courses with PhD and medical students, and began patient care as an occupational therapist (OT) in 1979. Her first neurological patient provided a background that enhanced her knowledge and skill in musculoskeletal disorders, and her contact with PTs made her realize that her greatest interests were more consistent with PTs than OTs. She completed the necessary course work and clinical experience for physical therapy and, because of her breadth of ability and experience, was recruited for the PT faculty at Washington University. She taught many continuing education courses about movement system impairment syndromes, receiving more invitations to do so than she could accept. Her thorough understanding of anatomy, kinesiology, and application to examination and treatment has resulted in a strong reputation in both the university medical center and the St Louis community. She has been put in charge of developing a mentoring program for other faculty involved in the clinical practice, and she has worked closely with a premier hip preservation orthopedic surgeon to explore her interest in hip pain in adults. As a faculty member, she has continued in patient care, extending her clinical contributions from 1979 until the present. She guides others in publishing their clinical findings in addition to being productive in contributing research, literature, and professional presentations pertaining to hip-related problems and PT practice.

A member of APTA since 1983, Bloom now serves on the Orthopaedic Section’s Education Committee, is a member of the Sports Physical Therapy Section and Education Section, and serves on the Planning Committee for the Annual Orthopaedic Meeting.

For her embodiment of the best attributes of a clinician scientist, including remaining firmly grounded in clinical practice, APTA is pleased to present Nancy J. Bloom with the Henry O. and Florence P. Kendall Practice Award.

Support and encouragement for my career began with my parents, Marcella and Joseph Bloom. Their strength of character along with their constant love and incredible generosity have allowed me to pursue my dreams. The place I’ve pursued those dreams for the past 40 years is Washington University and I would not be here today without valued and talented colleagues who continue to inspire me. A special person in my life and career is Dr. Shirley Sahrmann. Shirley has been the complete mentor, teaching me skills of observation, critical thinking, and problem solving all while maintaining a sense of humor. She paved the way for me to enjoy so many professional opportunities including meeting Florence Kendall. Henry and Florence Kendall’s work, their dedication to the precise assessment of alignment, strength, and muscle function helped form for my understanding of movement. FREDERICK L. HOBUSCH, PT, DMT, OCS

Frederick L. Hobusch, PT, DMT, OCS, is recognized for his combination of intelligence, ability to problem-solve and integrate knowledge into application, and caring approach to patient care.

Since 1979, Hobusch has been co-owner and president of Westwood Physical Therapy, an independent private practice specializing in orthopedic manual physical therapy. In addition, he has held the posts of vice president, director of faculty development, instructor

and chief financial officer for the Ola Grimsby Institute. Hobusch earned a DMT Honoris Causa from the Ola Grimsby Institute, a master’s of orthopedic manual therapy from the Ola Grimsby Institute, a BS in physical therapy from the University of Utah and has passed the OCS exam.

Hobusch has been on a continuous learning process since beginning his PT career. He became interested in and took courses in manual therapy starting in 1978, and enrolled in every manual therapy continuing education course offered in the United States in his quest to increase his competency as a manual therapist. In 1983, Hobusch took a sabbatical from private practice and spent several months in Norway, where he participated in and brought back to the United States a manual therapy clinical residency. After that time he entered a fellowship in the field, then became certified to teach medical exercise therapy through the Holten Institute. He continues to take courses on manipulative techniques, differential diagnosis of physical therapy, medical screening, radiology in physical therapy, and cervical neuromuscular dysfunction, among others. Hobusch, as a past adjunct faculty member, taught manual therapy at the University of Utah Division of Physical Therapy for 21 years. Hobusch shares his knowledge by teaching such courses as Clinical Problem Solving because it challenges him to delve deeper into a subject as well as share what he has learned with other professionals. He enjoys working with patients, particularly those with complex problems who are referred to him due to his knowledge and expertise.

An APTA member since 1974, Hobusch has been very involved with the Utah Chapter, including service as vice president, on the chapter executive and disciplinary committees, and as program chair. On the national side, he is a member of the Orthopaedic Section and Private Practice Section, and chaired the Sports Medicine Task Force, which addressed the ATC, and served on the ad hoc committee on manipulation that responded to a challenge by chiropractors. For the association, Hobusch has been a voice that isn’t afraid to be heard when it was needed.

For his impact on patients and the profession and his lifelong pursuit of greater knowledge, APTA is pleased to present Frederick Hobusch with the Henry O. and Florence P. Kendall Practice Award.

This is an amazing accolade to be selected as the recipient of an award named in honor of Henry and Florence Kendall whose clinical excellence and service to our profession set a standard for us all. I would first like to mention my high school health teacher, Jerry McCleary, who inspired me to set my goals on physical therapy as a profession. Dr. Carolee Moncur, my PT school professor, taught me to believe in myself. Dr. Ola Grimsby, who has dedicated his career to teaching skill excellence and integrity, has always inspired me to become the best clinician and instructor possible. I am humbled by the honor my colleagues Rodney Miyasaki, Jim Rivard and Robert Rowe have given me by nominating me for this award. Each of them has provided me with an example of unselfish service in their conduct as outstanding clinicians, teachers and leaders. My association with my students, fellow instructors and especially my patients continually enriches my abilities as a physical therapist. Due to the support of my family and especially my wife, Georgia, I have been able to pursue my aspirations. JAN DOMMERHOLT, PT, DPT, DAAPM

Jan Dommerholt, PT, DPT, DAAPM, the first physical therapist to teach concise manual trigger-point examination and trigger-point dry needling, has been responsible for groundbreaking work in treating pain and dysfunction of myofascial origin.

Currently president and CEO of Bethesda Physiocare, Inc in Bethesda, MD, CEO of PhysioFitness, LLC in Rockville, MD, and president of Myopain Seminars, LLC also in Bethesda, Dommerholt is also an

associate professor for Universidad CEU Cardenal Herrera in Valencia, Spain, and a guest lecturer at numerous other universities throughout the world. Dommerholt holds a DPT from the University of St Augustine for Health Sciences, MPS from Lynn University in Boca Raton, FL, and studied physical therapy at the Interstudy Academy of Physical Therapy in Arnhem, the Netherlands. He is currently pursuing a PhD degree from Aalborg University in Denmark.

Dommerholt has engaged in extensive clinical practice for more than 27 years, since graduating as a physical therapist in the Netherlands in 1986 and moving to the United States, where he completed a specialized research program in Performing Artists Disorders at New York University’s Human Performance Analysis Laboratory. He introduced and developed a refined myofascial examination and treatment of patients with pain by utilizing and teaching trigger-point dry needling to physical therapists, introducing a revolutionary treatment option. As recently as 10 years ago, only 4 states considered trigger-point dry needling within the scope of physical therapy practice, but thanks in part to Dommerholt’s advocacy and efforts, today 34 states and the District of Columbia have recognized the practice. He has taught more than 300 workshops and published more than 80 articles, 54 book chapters and 3 books, all related to myofascial pain, chronic pain conditions, fibromyalgia, complex regional pain syndrome, and whiplash injuries. In addition to teaching this approach to many physical therapists, physicians, dentists, nurses, veterinarians, chiropractors, acupuncturists, and other health-care professionals, he has substantiated the technique with solid science and research. His work in co-founding the Janet G. Travell, MD, Seminar Series culminated in the foundation of Myopain Seminars in 2007.

A longtime member of APTA, Dommerholt serves on the Orthopaedic Section, the Performing Arts Physical Therapy Special Interest Group, and the Pain Management Special Interest Group. His work with the Dry Needling Subject Matter Expert Workshop helped him to develop an educational resource paper, “Physical Therapists & the Performance of Dry Needling.”

APTA congratulates Jan Dommerholt on his work in introducing and promoting the practice of trigger-point dry needling to the physical therapy profession and entire medical community and is pleased to present him with the Henry O. and Florence P. Kendall Practice Award.

Back in the Netherlands, I would like to acknowledge my parents for teaching me the basics of life. Professionally, I especially would like to thank one of my teachers in PT school, Mr. Aad Vis, who demanded critical thinking before the term was invented. In the US, I am especially indebted to Drs. Janet Travell, David Simons, Robert Gerwin, and Richard Materson. After meeting Dr. Travell in 1989, she and David Simons have influenced my professional and personal life more than I could ever have imagined. Through her, I was introduced to Robert Gerwin with whom I have worked for many years seeing patients, teaching courses, writing articles and book chapters. Richard Materson showed me that limits are often self-imposed and he gently pushed me along to be the best you can be. Steven Alan Hoffman, PT, ATC, SCS, has delivered evidence-based care to his patients and served as a model of clinical excellence for almost 35 years.

Presently owner and director of rehabilitation, North Hills Orthopedic and Sports Physical Therapy, Sewickley, PA, Mr Hoffman has also held posts at North Hills Sportsmedicine Center, Ohio College of Podiatric Medicine, Western Reserve Therapists, and Rainbow Sportsmedicine

Center. He received his MS in Health Sciences Education from Case Western University, and BS in Physical Therapy from State University of New York at Buffalo. In addition, he studied Physiology of Exercise at the University of Pittsburgh.

Considered a passionate advocate for the physical therapy profession in his years of practice, Mr Hoffman has mentored hundreds of physical therapists, student PTs, and PTAs. In addition, he has contributed to multiple journal articles and book chapters, and has lectured nationally on clinical practice and treatment strategies. Beginning with his early entry-level education, he had the foresight to seek out clinical mentors years before the concept of residencies and fellowships in PT were ever conceived. Throughout his career, Mr Hoffman attended hundreds of educational opportunities, always seeking to apply the most up-to-date evidence to patients and striving to optimize each individual outcome. For the profession, he has contributed his clinical knowledge to advance the next generation of PT professionals. Beyond didactic education and publications, he has served as a clinical mentor and clinical instructor to dozens of PT students from many academic programs.

An active member of APTA, particularly with the Orthopaedic, Sports and Private Practice Sections, Mr Hoffman was presented the Lynn Wallace Clinical Education Award from the Sports Physical Therapy Section in 2010 and the Service Award in 2002. Mr Hoffman also serves on the Pennsylvania Referral for Profit Task Force, Pennsylvania Private Practice Physical Therapy Network, and continues to be a member of the National Athletic Trainers Society, American Orthopedic Society for Sportsmedicine, and American College of Sportsmedicine.

APTA is pleased to recognize Mr Hoffman’s many years of service and contributions to the profession with the Henry O. and Florence P. Kendall Practice Award.

I have had the privilege of being mentored by some of the best in the profession. Dr. Steven Rose was the program chair at the University of Buffalo, where I attended undergraduate school. He set the example for me as well as for so many others – he taught that physical therapy is a profession with a wide range of opportunities. I will be forever grateful for the guiding hand he offered in helping to shape my career.

My second mentor was Lynn Wallace. He was one of the early pioneers of sports physical therapy and gave me the opportunity to work under his tutelage when in graduate school. I would not be the therapist I am today had it not been for his influence.

I have also had the privilege of working with some of the most caring and talented clinicians one can find. I am just a small part of a talented group of professionals, and it is because of their support and influence that I remain inspired to practice physical therapy.

My family has provided the foundation for my professional and personal life. My parents provided the opportunity for me to obtain an outstanding education. My wife and life partner Vicky, and sons Danny and Adam are the light of my life and without their support, I would literally be nowhere.

I accept this award in honor of my mentors, colleagues, fellow therapists, and my family. This is one of the proudest days of my professional career.

Mary Kathryn Wing, PT, DPT, GCS, NCS, has committed herself to a life of outstanding clinical practice, positively impacting the quality of practice and the clinical practice of others while contributing to both the state and national associations.

Presently owner of Southwest Advanced Neurological Rehabilitation, Phoenix,Wing is also adjunct instructor at A.T. Still University, Mesa and Northern Arizona University, Flagstaff;. She has served as director of

clinical affairs for Kinetic Muscles, Tempe, and manager of rehabilitation services for Chris Ridge Village, Phoenix. Wing received a DPT from Northern Arizona University, certificate in physical therapy from Northwestern University and BS in pre-physical therapy from Brigham Young University, Provo, UT.

Wing has been engaged in extensive clinical practice for the past 38 years in a variety of settings, including acute care, skilled nursing and outpatient. In the pursuit of lifelong learning and excellence in patient care, she obtained her DPT and became an APTA board-certified specialist in both neurology and geriatrics. Wing has affected the shape, scope and quality of physical therapy practice. Embracing evidence supporting the benefit of intense rehabilitation programs for individuals with neurological dysfunction following a cerebral vascular accident, she developed an innovative business model to provide treatment at a higher frequency and intensity. She participates in clinical research in collaboration with local universities. In Wing’s world, patients come first, regardless of their ability to pay for services. She serves as an advocate on behalf of patients needing services, and has conducted and published research with implications for physical therapy practice.

Past president of the Arizona Chapter, Wing has served the chapter in numerous leadership roles. At the national level, she was key contact for Senator Jon Kyle, as well as Publications Committee member and chair for the Section on Administration. She has been honored with the Clinical Excellence in Neurology Award from the Section on Neurology, Outstanding Therapist of the Year from the Arizona chapter, and Outstanding Service Award from the Section on Administration.

APTA is pleased to honor Wing’s history of practice and service with the Henry O. and Florence P. Kendall Practice Award.

Colleen Chancler, PT, MS, is committed to ensuring the highest level of practice by serving as a role model of “best practice” for colleagues, through teaching students in almost every PT program in the Philadelphia area, and through sustained service to APTA.

Chancler is site manager and senior physical therapist for the University of Pennsylvania. Previously she held that role at Magee Rehabilitation Hospital and Mercy Catholic Medical Center-Fitzgerald Mercy Division, and was on the staff at Mid-Atlantic Rehabilitation Services, Graduate Health Care System-Graduate Hospital, and George Washington Health System. Currently a matriculated student in the PhD Program for Health Sciences at Seton Hall University, Chancler also studied at Temple University, and earned her BS and MHS from the University of Scranton and Drexel University, respectively.

An active participant in hospital-based committees at the Hospital of the University of Pennsylvania, Chancler has positively impacted therapy practice and patient care. As an integral member of the Falls Committee, Chancler helped to raise awareness regarding fall risk of patients throughout the hospital’s ambulatory offices, which led to a change in the manner in which the entire health system approaches patients with a possible fall risk. In addition, she participated in the assessment and implementation of overhead lifts, which are now used to help minimize possible injury to staff and patients. In this effort, she personally trained most of the hospital staff. Within the Good Shepherd Penn Partners therapy department, Chancler is an advocate for the advancement of the student program and supported the development of an acute-care internship, which challenges students with more intensive study and mentorship.

Active in APTA since 1986, Chancler is currently a delegate and on the Advisory Panel on Practice. For the Pennsylvania Chapter, she is on the Board of Directors, Neurology SIG co-chair, and a member of the Emergency Preparedness Task Force and Professional Development Committee. She has been honored with the Best Case Report from Research Committee of Pennsylvania Physical Therapy Association award and Team Member for Quality and Safety Award for the University of Pennsylvania Health System.

APTA is pleased to present Colleen Chancler with the Henry O. and Florence P. Kendall Practice Award.

Patrick VanBeveren, PT, DPT, MA, OCS, GCS, CSCS, has had a profound impact on his patients and on the physical therapy profession as a whole, particularly in the professional development of local and national physical therapists and the negotiation of direct access in New York State. He has co-authored a home study course and textbook chapter on exercise and aging and authored several online education courses. His significant impact on clinicians is felt through his inspired teaching at the local, state and national levels on

topics related to orthopedics and aging.

Like the Kendalls, Dr VanBeveren has been a dedicated clinician throughout his career. In fact, when asked what he considered to be his greatest professional accomplishment, he replied without hesitation, “Treating patients every day for 38 years.” Using a pathokinesiology approach, Dr VanBeveren enjoys the process of diagnosis and delivery of treatment, and he posesses an uncommon ability to explain complex anatomical and biomechanical principles to patients to help them take responsibility for their healing and recovery. He delights in the fact that he knows and has seen members of extended families, functioning as the “physical therapists of family practice.” Most recently he has developed a large community-based exercise program for aging adults that has expanded into a city-wide influence through community lectures and a memory academy.

Currently supervisor of physical therapy services at Syracuse’s St Camillus Health and Rehabilitation Center, Dr VanBeveren was owner of Lafayette Physical Therapy, Barry & VanBeveren, and VanBeveren & Yankowitz. He earned his DPT in physical therapy from Rocky Mountain University of Health Professions, Provo, UT; as well as a BS and MA in physical therapy from Ithaca College and New York University, respectively. He is dual board certified in Orthopedics and Geriatrics.

A member of APTA since 1973, Dr VanBeveren has served APTA on the Task Force on Physician Owned Physical Therapy Practices and Task Force on Direct Access, as chairman of the Committee of Component Presidents, and as member of the Orthopaedic, Private Practice, and Geriatrics sections. On the component level, Dr VanBeveren has been president and treasurer of the New York Chapter, speaker of the Delegate Assembly, PAC chair, and chair of the Legislative Committee.

Congratulations to Dr VanBeveren for living and practicing the example set by Henry O and Florence P Kendall. APTA is pleased to recognize his achievements with the Henry O and Florence P Kendall Practice Award.

I’d like to acknowledge first my kids, Matthew and Sarah, for their generosity in sharing me with my profession throughout their childhood; then my colleagues in the NYPTA who surrounded me with their competence and taught me so much; my business partner and friend, Gabe Yankowitz, for his unending and sage personal and profession counsel; and most important I acknowledge my life partner, professional partner, and soul mate Dale Avers for sharing her vision and motivation with me. Robert S Gailey, Jr, PT, PhD, is a clinician-scientist with a strong record of leadership, teaching, research and service, particularly in his impact on enhancing other physical therapists’ level of practice with respect to people with limb loss.

Associate professor and clinical educator at the University of Miller Miami School of Medicine’s Department of Physical Therapy, Dr Gailey is also the Director of the Functional Outcomes Research and

Evaluation Center at the Miami Veterans Affairs Medical Center. Outside of the academic realm, Dr Gailey was appointed by the Secretary of Veterans Affairs to the Advisory Committee for Prosthetics and Special Disabilities Programs and also serves as a special advisor to the United States Department of Defense. In addition, he is a clinical physical therapist for Advanced Rehabilitation Therapy Inc in Miami, and earlier honed his skills as a physical therapist at the University of Miami/ Jackson Memorial Hospital. Along with his many publications he has produced several educational programs and documentaries over the years. Dr Gailey holds a PhD in prosthetics and orthotics from the University of Strathclyde in Glasgow, Scotland; and MS and BS degrees from the University of Miami.

During the past six years Dr Gailey has been involved in a variety of capacities with service members and veterans suffering blast injuries, specifically limb amputations, as a result of the wars in Afghanistan and Iraq. Dr Gailey’s work with the DOD and Veterans Affairs has resulted in numerous multi-disciplinary workshops to military medical service personnel on the latest technology in prosthetics and in amputee rehabilitation. A popular University of Miami faculty member with consistently outstanding evaluations and a two-time Teacher of the Year Award winner, Dr Gailey inspires dedication to service in students as well as graduates. For the university, Dr Gailey initiated the Hurricane Challenge, which brings children with disabilities to the campus for a special day of fun. He has been actively involved in the International Paralympic Games and numerous other Sports Abilities competitions, and co-founded the Athletes with Disabilities SIG within the Sports Physical Therapy section of APTA.

APTA salutes Dr Gailey’s commitment to service and continued awareness of prosthetic developments with the Henry O and Florence O Kendall Practice Award.
