This Addendum shall make the following amendment to the Bid Documents:

A. NOTICE TO BIDDERS - Prospective bidders are hereby notified that the receiving of sealedbids, scheduled for 2:00 P.M., August 4, 2021, is hereby POSTPONED until 2:00 P.M.,August 13, 2021. The attached NOTICE TO BIDDERS dated r07/29/2021 will beincorporated and made a part of the original NOTICE TO BIDDERS.

B. SECTION 10 - Replace page 10-2, dated 06/15/2021 with the attached page 10-2, datedr07/29/2021 (remove 10.3 (A) 3. Certification from these specifications)

C. APPENDICES -

1. Replace page F-1 with the attached page F-1, dated r07/29/2021.

H-3 FREEWAY, HOSPITAL ROCK TO HALEKOU INTERCHANGE, PROJECT NO.

H3(FGH)-01-21C, scheduled for 2:00 P.M., Wednesday, August 4, 2021, via HIePRO,

is HEREBY POSTPONED until 2:00 P.M., Friday, August 13, 2021.

3. Certification. At the date of bidding, the Contractor or RME shall possess thefollowing and shall submit this to the State upon the State’s request:

a. Valid certification and in good standing as a Landscape Industry CouncilCertified Landscape Technician (CLT) Exterior/Maintenance

b. Valid certification and in good standing as a Landscape Industry CouncilCertified Landscape Technician (CLT) Exterior/Irrigation

4. The State may request a list of similar projects to substantiate the Contractor’sexperience. List shall contain a minimum of three (3) different landscapeprojects and shall include: a) Name of company that contractor worked with or iscurrently working with, b) point of contact, c) phone number, d) description oflandscaping work performed and e) size of project (cost wise).

The list of three (3) different landscape projects shall reflect a positiveperformance evaluation by the respective agency.

5. The Contractor shall possess the required business and tax license to conductbusiness in the State of Hawaii.

6. The Contractor shall have all required equipment (E.g. handheld tools, utilitytrucks, mowers, etc.) necessary to perform the specified services in thisdocument.

(B) Work Zone Safety and Temporary Traffic Control Personnel

1. If the Contractor plans on using a subcontractor(s) to perform flagging/trafficcontrol/closing of lanes, subcontracting and approval process shall be inaccordance with Section 8.2 Subcontracting. The Contractor shall obtain theDirector’s written approval to subcontract prior to the issuance of the Notice toProceed.

2. Training and Certification. At the date of bidding, the Contractor shall ensure thateach employee whose actions affect safety in the temporary traffic control (TTC)zone have received training in accordance with the recommendations described inaccordance with the recommendations described in the Manual on UniformTraffic Control (MUTCD) 2009, Sections 6B.01 (Fundamental Principles ofTemporary Traffic Control) and 6D.03 (Worker Safety Considerations), and inaccordance with Hawaii Administrative Rule (HAR) 12-110-50(b)(2)(A).

As a minimum, the training shall meet the following criteria:

a. Each person whose actions affect work zone safety, from upper-levelmanagement through the field level employees, shall be trained as necessaryto meet their responsibilities in the work zone.

b. Workers shall be trained on:

Supervisors: Supervisors shall be trained in the principles of workzone safety and TTC. Only those individuals who are trained in properTTC practices and understand the principles of TTC may supervise the
