HOME-DELIVERED MEALS PROGRAM• BCBS MN has contracted with Mom’s Meals NourishCare (PurFoods
LLC) to provide condition-specific, home-delivered meals to dual eligible members.
• This presentation will provide details on BCBS MN’s new home-delivered meals program and how to administer the program
• The presentation will also discuss the importance of nutrition to members after discharge and when managing a chronic condition

HOME-DELIVERED MEALS PROGRAM
• 4 weeks of meals for members discharged from an inpatient or short-term SNF stay
IP or a short-term SNF stay, they would qualify for the benefit again, this is a “renewable unlimited benefit”

Additional Benefit Details• Member will receive a welcome call from Mom’s Meals Customer Care team
that introduces the program and provides information about the date and time the member can expect the first delivery.
• Meals are packaged and shipped to the member’s home in a temperature controlled cooler and can be refrigerated up to two weeks or frozen up to three months.
• Member will receive the initial Post-discharge (after hospitalization) mealdelivery within 72 business hours (3 business days) of order processing
— If service authorization form is received before 12 pm (noon) CT, Mom’s Meals willprocess and ship order the same day
— If service authorization form is received after 12 pm (noon) CT, Mom’s Meals will process and ship order the next business day
• A signature is not required for delivery• Package will contain program information

Additional Benefit Details
• Meals will be delivered, in most cases, by FedEx or UPS; if FedEx/UPS delivery is not used, meals will be delivered by Mom’s Meals delivery team
• Meals will be delivered to member’s doorstep• If member has not received their initial order of meals, refer member to
Mom’s Meals– Members can find out when to expect their delivery by calling the member
telephone number, 866-204-6111; hours of operation are Monday – Friday from 7am to 6pm Central Time
• Refer members to this same phone number when they have other meal-specific questions or feedback, including meal selections, preferences or complaints

2. Complete the Form— Include the start date— Indicate the address for delivery— Note food preferences or aversions and any delivery details— Contact Mom’s Meals with questions via the Case Manager telephone number,

Man called 911 asking for foodAn 81-year-old man living alone calls 911 asking for food after being discharged. At only 115 lbs. and unable to get out of his chair, he reported being hungry and had returned home to an empty refrigerator.
Woman readmitted after relying on convenience foodsA 78-year-old woman with diabetes and heart failure is discharged from the hospital after an exacerbation. Not feeling up to cooking, she and her husband rely on high sodium processed and take-out foods. She experiences swelling and other symptoms and is readmitted 15 days later.
