Adrenal Support Approaches

Adrenal Support - Introduction

• As mentioned in the previous lesson, we can debate the relevance or validity of starting a client process with an adrenal test and programme, or introducing it later, say after 90-days of thorough work on the basics. But either way, it’s important to know what to do when you receive the results!


• So, in this lesson, we’ll look at the main approaches to adrenal rejuvenation. Note that as part of this lesson there’s a special 38min audio interview with David Brassey regarding the products he stocks, and how we can incorporate them into our Hompes Method programmes.

• Remove bad foods:

– Gluten, soy, sugar, cow’s milk, processed food, additives, PUFA, etc.

• Optimise eating patterns and habits:

– Meal and snack timing – same time each day; eat every 2.5hr

– Eat good foods – fresh, whole, well balanced macronutrient ratios

• Support blood sugar:

– Appropriate supplements

• Of course, these are just the basics and it’s prudent to then add:

– Remove bad bugs

– Replace good bugs

– Fix the gut

– Support the liver

– Additional detox if needed

– Specific nutrient replacement


• Again, at what point ought we to add in the adrenal and other endocrine support? It’s a tricky call and needs to be done on an individualized basis.

Adrenal Support – Different Approaches

• We can help clients and patients support and rejuvenate adrenal function using different strategies:

1. Nutrition (including nutritional supplements)

2. Herbal (adaptogens)

3. Glandular formulas

4. Bioidentical hormones

Nutritional Support for the Adrenals

• Of course, the whole body requires optimal nutritional status, but there are some specific nutrients that can assist in adrenal support. The more research I do, the more I feel glutathione, vitamin D, vitamins A and E, along with minerals and especially vitamin C are of utmost importance. But you can’t isolate the nutrients and expect miracles!

• Optimal cellular energy production (carnitine, B-vits, CoQ10)

• Vitamins, especially B5, B6, biotin

• Vitamin C –high concentrations are found in the adrenals

• Glutathione

• Vitamin E

• Vitamin D

• Minerals (magnesium, copper, zinc)

• Blood sugar support nutrients (Cr, V, lipoid acid) separate lesson)

• *The value of an Organix Profile cannot be understated.


• Many of the combination adrenal support products have these nutrients, but you may needed higher doses of things like vitamins C, D, E, magnesium, etc. than are contained in those products and they may not contain CoQ10, carnitine, glutathione, etc. See the Kharrazian thyroid book for excellent information on many of these nutrients, as well as others that we’re about to cover.

Phosphatidyl Serine (PS)

• PS is an endogenously produced phospholipid embedded in cell membranes. It’s the major phospholipid of the brain:

– Supports cellular chemical signaling transmission

– Activates cell surface receptors

– Involved in cellular exchange of nutrients


• Endogenous production is an energy demanding process and requires several nutrients – serine, glycerophosphate, fatty acids, methyl donors such as folate, B12, SAM, etc.


• Exogenous supplementation has been shown to:– Enhance cell metabolism and communication

– Protect cells from oxidative damage

– Decrease anxiety & depression

– Improve mood & motivation

– Enhance memory and cognition

– Decrease cortisol


Perhaps the most clinically significant impact of PS is its ability to reduce cortisol, but to do so requires high doses, which is expensive. Intravenous delivery is possible and can be effective, but thankfully there’s also a liposomal cream delivery form now.


“Transdermal delivery utilizes lipoid spheres, known as liposomes, to transport PS through the skin and into the blood. Once there, the liposome shell around the

PS substance degrades and makes PS available for

active response in the bloodstream.”

Phosphorylated Serine – Seriphos

• Not the same as phosphatidyl serine, but has a similar effect on cortisol:

– Dampens ACTH signal from pituitary to adrenals.

– Can use to gently bring cortisol down at points of the day when it’s too high.

Adaptogen Support For The Adrenals

“The term adaptogen is used by herbalists to refer to a natural herb product that is proposed to increase the

body's resistance to stress, trauma, anxiety and fatigue. In the past, they have been called rejuvenating herbs, qi

tonics, rasayanas, or restoratives. All adaptogenscontain antioxidants, but antioxidants are not

necessarily adaptogens and that is not proposed to be

their primary mode of action.”


“The concept of adaptogens dates back thousands of years to ancient India and China, but modern study did not begin until the late 1940s. In 1947, Nikolai Lazarev defined an adaptogen as an agent that allows the body

to counter adverse physical, chemical, or biological stressors by raising nonspecific resistance toward such

stress, thus allowing the organism to "adapt" to the

stressful circumstances.”


“It is claimed that adaptogenic herbs are distinct from other substances in their ability to balance endocrine hormones and the immune system, and they help the

body to maintain optimal homeostasis. Adaptogens are proposed to have a normalizing effect on the body and to

be capable of either toning down the activity of hyperfunctioning systems or strengthening the activity of

hypofunctioning systems. However, they are also proposed to be functional at the level of allostasis, which is a more dynamic reaction to long term stress, lacking

the fixed reference points of homeostasis.”


• An adaptogen is nontoxic to the recipient.

• An adaptogen produces a nonspecific response in the body—an increase in the power of resistance against multiple stressors including physical, chemical, or biological agents.


• An adaptogen has a normalizing influence on physiology, irrespective of the direction of change from physiological norms caused by the stressor.

• Under this definition, adaptogens would be:

– Nontoxic in normal doses

– Produce a general defensive response against stress

– Have a normalizing influence on the body.


• Introducing some of the main players on the adaptogen team:– Licorice

– Rhodiola

– Ginseng• Panax (American)

• Eulethero (Chinese / ”Siberian”)

– Ashwaganda

– Rehmannia

– Bupleurum

Adaptogen Support - Licorice

• Compounds in licorice have a “sparing” effect on cortisol. They can also have:

– Oestrogen modulating activity

– Anti-inflammatory activity

– Anti-allergic activity

– Immune-stimulating activity

– Antibacterial and anti-viral activities


• One specific component of licorice – glycerrhizin –downregulates the enzyme 11-beta hydroxylase, which converts cortisol into cortisone. Thus, glycerrhizin increases the half life of cortisol.


• For use at times of day when there is low cortisol:

– Spares cortisol

– Sublingual form bypasses gut.

– Do not use if person has high blood pressure, high oestrogen or licorice allergy.

Adaptogen Support - Rhodiola rosea

• Adaptogenic plant and may:

– Enhance CNS, act as an antidepressant, be cardioprotective& anticarcinogenic, support immune function, enhance cognitive and mental function, impact effects of catecholamines.

Adaptogen Support – Panax Ginseng

• Korean ginseng – one of the best known adaptogens:– Supports fatty acid

oxidation and spares glycogen.

– May enhance O2 supply to muscles.

– May improve stamina, energy & performance.

– May improve HPA feedback loop.

Adaptogen Support - Eulethero

• Siberian Ginseng - most studies conducted in USSR:

– Enhanced athletic performance of animals, optimise HPA axis under stress, enhance work output, mental and physical responses under stress.

Adaptogen Support - Ashwaganda

• Also known as Withaniasomnifera and Indian Ginseng:

– Similar adaptogenicproperties to Panaxginseng.

– May help couteractadverse physical responses to stress such as in blood sugar management.

Adaptogen Support - Rehmannia

• Enhances adrenal function:– May normalize blood sugar

May provide antiallergyproperties & improved immunity.

– Anti-inflammatory activity in CNS.

– Has been shown to stimulate osteoblasts and inhibit osteoclasts.

– May have cardioprotectiveand antitumor activity.

Adaptogen Support - Bupleurum

• An adrenal adaptogen:– Significant anti-

inflammatory actions.

– Able to stimulate the hypothalamic-pituitary-adrenal system by promoting the release of ACTH.

– Its anti-inflammatory actions may include an inhibitory activity against allergic asthma.

Adaptogen Support – Holy Basil Leaf

• Holy Basil Leaf extract may:

– Mitigate cortisol elevations in acute and chronic stress.

– Modulate HPA axis activity.

– Enhance gastric mucosa

– Be immuno-supportive

Adaptogen Support – Punarva

• Also known as Boerhaavia diffusa:– Supports over and

underactive adrenals.

– Buffers elevations in serum cholesterol.

– May prevent suppression of immune system.

– Can improve cortisol in very low adrenal fatigue.

Glandulars in General

“Healers in the past (and currently) used tissue extracts as one armamentarium in the fight against disease. For instance, extract of bone marrow has been used for the treatment of anemia. Desiccated thyroid is still used by many alternative practitioners in the management of

hypothyroidism.”


“Many people take glandulars as a perceived source for natural hormones. In fact, the use of glandular therapy

forms the foundational elements and components of modern day prescription of hormones including thyroid,

prednisone, and estrogen.”


“Glandular therapy began with the discoveries made by Swiss physician Paul Niehans, M.D. in the 1920s. Dr.

Niehans went on to develop live cell therapy at his clinic in Montreux, Switzerland. Thousands of patients came

to his clinic as a last resort. His therapies became famous for rejuvenating all that came to see him:

wealthy businessmen, royalty, presidents, and

celebrities. Live cell therapy is still practiced in Europe.”


“It is estimated that 5,000 German physicians utilize cell therapy and millions of patients have benefited over

the last 50 years. By the mid 1930s, adrenal cell extracts in liquid and tablet forms were produced by

several companies. By the late 1930s they were being used by tens of thousands of physicians. As recently as

1968, they were still made by some of the leading

pharmaceutical companies.”


“These extracts are used to replenish and eventually normalize adrenal function. An advantage over cortisol hormone replacement is that adrenal cortical extracts can be discontinued once they have done their job of

repairing adrenal function. Glandulars can theoretically come from any animal, but most often they are derived

from cow (bovine), while others come from pig

(porcine) and sheep (ovine).”


“One of the primary set backs in the modern day use of glandulars is the difficulty of standardized testing and thus lack of research. Modern scientific studies involve isolation of variables into one variable to be involved in a double blind experimental setting. This is clearly not

possible in the case of glandulars, which contain enzymes, vitamins, fatty acids, amino acids, minerals,

neurotransmitters and a host of nutrients in addition to

the tissues within the gland.”


“Glandulars contain many substances including hormones. The major problem that arises is not knowing how much of these hormones or other substances are available in

these extracts since they can vary from batch to batch and animal to animal. Also, since there are so many

substances within these glandulars, it is difficult to know which of the substances is having a therapeutic influence

and how they interact with the myriad other substances in the body.”


“Because they have so many substances in them it is difficult to determine and measure what kind of effect

they may have in the long run when ingested as a supplement. Nevertheless, there is little doubt that

glandulars do possess healing properties if used properly

based on antidotal evidence for over 80 years.”


“The best glandulars usually comes from freeze-dried extracts from organic cows raised in New Zealand.

There has been no Bovine Spongiform Encephalopathy in New Zealand. Organic fed cows are raised on grass

and aren't fed any dead animal products (the source of Mad Cow Disease).”


“They are government inspected and raised without the use of pesticides, hormones, or antibiotics. Reputable

glandular products are subject to in-process and finished product testing. These tests include microbial

contamination tests to assure acceptable total bacteria counts and the absence of disease-causing bacteria.”

“Adrenal glandular are widely promoted and used for Adrenal Fatigue. Many report a sense of increased

energy when previously fatigued and a sense of calmness when previously anxious after taking

glandular. These adaptogenic properties, however, are usually demonstrable in those with mild Adrenal Fatigue and not in those with advance adrenal

weakness.”

“Those with advance Adrenal Fatigue are often stimulatory. While this may be considered as a

desirable outcome at first as energy returns, there are significant side effects if taken long term. Taking

adrenal glandulars chronically to sustain energy is one of the common reasons for adrenal recovery failures due to addictive and withdrawal side effects of these

compounds.”

Adrenal Glandulars

“First and commonly, the body may develop tolerance, and over time, more glandular are needed to produce the same effect. Second, at high dose, these glandular may actually trigger and precipitate adrenal crisis as

the body's ability to handle the stimulant reaches maximum tolerance level. Sufferers may experience a

"wired and tired" feeling resulting in insomnia and

hyper-irritability along with extreme fatigue.”

“Third, withdrawal and rebound symptoms may surface when the glandular are stopped, evidenced by even

greater fatigue. Fourth, glandular may trigger a series of paradoxical and undesirable effect, ranging from

extreme fatigue, palpitation, blood pressure sensitivity,

anxiety, among others.”

“Because of these possible side effects, the general consumer should use adrenal glandular on a short term basis provided that the Adrenal Fatigue is in a very mild

or beginning state. The more advance the Adrenal Fatigue, the more one should stay clear of adrenal glandular and its use is best left to the experienced

clinician.”

“Due to the lack of standardization, there are many products on the market available to the consumer. Not all are created equal. Perhaps the most potent product is called Isocort. Isocort is a popular over-the-counter

freeze-dried adrenal cortex glandular supplement containing herbs and medium chain triglycerides that

may have cortisol like properties.”

“It's advertised as providing a standardized, stable, and "gentle" dose of the adreno-cortical substance

equivalent to 2.5 mg of cortisol per tablet. Ingredients of Isocort are listed are as follows: Freeze-Dried Adrenal Cortex (soluble fractionation) from New Zealand Sheep.

Echinacea Extract (trace amount). Prunus and Lomatium dissectum root isolate (kreb concentrate-2%)

in pellet base of lactose and lactase. No cortisol is actually listed in the ingredients.”

Adrenal Glandulars

“Clinically, prescription steroids such as Cortef or hydrocortisone are far more effective on a dose

equivalent basis as compared to Isocort if a strong steroidal effect is desired, though isocort does seem to produce steroidal-like properties. Isocort and Isocortequivalents are popular because it can stimulate the

body, leading to increased energy and reduce fatigue.”

“Long-term intake exposes the body to side effects common with other glandulars. Additionally, long term

ingestion of echinacea (an Isocort component) is not indicated. Isocort should be taken with great care and

under supervision of an experienced health care professional even though it is available as an over-the-

counter nutritional supplement.”

Adaptogens & Glandulars Summary

• As mentioned, these products are discussed in detail during my discussion with David Brassey. Furthermore, the specific protocols have been written out and provided for you in the resources section.

Bioidentical Hormones

• The bioidentical hormones are used by many doctors and practitioners in the US.

• They’re typically derived from wild yam (diosgenin) or soy (“wild yam” on a label does NOT mean the product contains bioidentical hormones!)

• Legalities prohibit use in the UK (prescription only) but the problem is that doctors simply don’t know how to use them (so they’re not used!).

• Patients/clients in the US can use them freely.


• We will discuss the concept of bioidentical hormones more when we talk about our client/patients’ sex lives at the end of the course and I’ll give you the history and science of their use in that sub-module.


• It’s important to note that the purpose of using bioidentical hormones is not necessarily just to replace the missing or low hormone. Rather, the goal is to provide tiny, physiologic doses of the hormone to help reset the HPA axis over time. The hormones do help to “prop” up the person in the meantime, but that’s NOT the main purpose of using them.

• I was taught to use sublingual drops as the delivery route for the hormones because it bypasses the gut and ensures delivery into the bloodstream via the rich capillary bed of the gums. This supposedly mirrors the delivery method of the hormones endogenously (i.e. straight into the bloodstream rather than through the gut).


• One problem with this delivery route is that the serum hormone levels go up relatively quickly and come down again. Another debate hormones ingested by eating the glands of animals have to be digested and absorbed via the gut.


• This issue gets quite complex, which is why I discuss delivery methods right at the end of the course, once we’ve covered sex hormones and thyroid. When I was taught to use bioidentical hormones, the preferred delivery method was sublingual drops, so that’s where I’d like to start!


Specific bioidentical hormone products from Biomatrix.


• They are dropped under the tongue and allowed to absorb through the gum. They’re very easy to use, easy to carry around, and most clients love them for their convenience. These are not the only sublingual bioidentical hormone products on the market by any means.


• On the following slide you’ll see a table from Dr. Kalish’s programme that I took way back in 2007, with simplified recommendations for using the bioidentical hormones pregnenolone and DHEA. This table is a summary / derived from the work of Dr. Timmins at Biohealth and I’ve included the BiohealthInterpretive Guide document for you in the resources for this lesson (it’s well worth a read).


Dr. Kalish’s Summary for the use of Pregnenolone and DHEA


• At Hompes Method, we’re lucky enough to work with clients in the US, so I have a lot of experience. Working with bioidenticals is not as easy as many people will tell you (certain other courses that are out there, for example).


• When I first started using them, I seemed to get positive responses in every client, so much so that I thought the work was almost a miracle cure. But slowly, Mother Nature kicked me in the ass and sent me more and more cases where the bioidenticalscaused more problems than they resolved!


• There can be significant “paradoxical reactions” that many people believe are due to autoimmunity, infections and detoxification capacity. For example, I’ve worked with both men and women who couldn’t tolerate a single drop of pregnenolone – it sent them into a hyper-active “spin” that in some cases lasts 24 hours.


• Yet I’ve seen other people who were already in that spin, and were completely calmed down by 20-30 drops! I’ve experimented with 60 or more drops per day and had no adverse effects. However, with too much DHEA, I develop aggressive tendencies.

“DHEA (dehydroepiandrosterone) has a technical-sounding name because it has never been identified

with a single dominant function, in spite of its abundance in the body. Many researchers still think of it

as a substance produced by the adrenal glands, but experiments show that animals without adrenals are

able to produce it in normal amounts.”


“Much of it is formed in the brain (from pregnenolone), but it is probably produced in other organs, including

the skin. The brain contains a much higher concentration of DHEA than the blood does.”


“In old age, we produce only about 5% as much as we do in youth. This is about the same decrease that occurs

with progesterone and pregnenolone. The other hormones (for example, cortisone) do not decrease so much; as a result, our balance shifts continually during

aging toward dominance by hormones such as cortisone, which use up more and more body

substance, without rebuilding it.”


“Protection against the toxic actions of these specialized hormones is a major function of DHEA and the other youth-associated hormones…Young people produce about 12 to 15 milligrams of DHEA per day,

and that amount decreases by about 2 mg. per day for

every decade after the age of 30.”


“This is one of the reasons that young people eat more without getting fat, and tolerate cold weather better:

DHEA, like the thyroid hormone, increases our heat production and ability to burn calories. At the age of 50,

about 4 mg. of DHEA per day will usually restore the

level of DHEA in the blood to a youthful level.”


“It is important to avoid taking more than needed, since some people (especially if they are deficient in

progesterone, pregnenolone, or thyroid) can turn the excess into estrogen or testosterone, and large amounts

of those sex hormones can disturb the function of the thymus gland and the liver. People who have taken an

excess of DHEA have been found to have abnormally high estrogen levels, and this can cause the liver to enlarge,

and the thymus to shrink.”


“One study has found that the only hormone abnormality in a groupt of Alzheimers patients' brains was an excess of DHEA. In cell culture, DHEA can cause changes in glial

cells resembling those seen in the aging brain. These observations suggest that DHEA should be used with

caution. Supplements of pregnenolone and thyroid seem

to be the safest way to optimize DHEA production.”


• Just from these Ray Peat quotes you can quickly begin to see how important it is to be ultra careful when using the bioidentical hormones:

– Begin with nutrition, lifestyle, gut work, adaptogens and possibly glandulars

– Use Biohealth Interpretive Guide / Dr. Kalish guidelines

– Titrate dose

– Monitor client symptoms and reactions• Mood, energy, sleep, cognitive, memory

– Adjust dose as necessary

• In the next lesson we’re going to shift our focus to the thyroid and begin exploring the relationship between this critically important gland and the adrenals. It’s going to open your eyes to the reasons why Selye’s GAS and the stage I, II, III adrenal fatigue model may not be all it seems.
