HSE – Health, Safety and Environment
• Mines can be hazardous environments and the possibility of fire, flood,
explosion and collapse has the potential to simultaneously affect a
large number of people.
• Sometimes, things get out of control.
• Best practices include:
– Leadership presence in HSE-related processes demonstrates what is
important: their employees’ safety and health
– Integrate HSE processes with standard work procedures
– A broad communication strategy that includes multiple languages and visual
representations of safety practices to help unify and inform workers
– Illustrate lessons learned, safe work practices and HSE tools, often

Everything gets worse when underground
Maybe the operator trusted too much the weight capacity?
Fire
This incident happened a couple of years ago. BMA Blackwater had a bad fire in their main
workshop. This is the only photo that I've ever seen of the incident. Sent in by a reader.

Operational mistakes
It's amazing just how many dozer operators forget where they
parked their own car. Those D11's have poor rear visibility at the
best of times, but with the single-tine ripper, there's no way he
could have seen that car.
