COURSE GOAL & STATEMENT OF NEED

The rapidly changing healthcare environment often leaves the entire CV team struggling to find the most current and up to date information to manage their patients and practices. From new treatment guidelines to the multiple regulatory requirements, this educational conference will provide the CV team an opportunity to identify the most current available information for the clinical and business side of cardiology.

The overall goal of this activity is to improve patient care by increasing learner competence in understanding the newest treatments and guidelines in major cardiovascular diseases and understanding where healthcare is headed from the business side of cardiology.

ACCREDITATION & DESIGNATION STATEMENT

CME/Physician: This activity has been planned and implemented in accordance with the accreditation requirements and policies of the Accreditation Council for Continuing Medical Education (ACCME) through the joint providership of the American College of Cardiology Foundation (ACCF) and the Illinois Chapter of the American College of Cardiology. The ACCF is accredited by the ACCME to provide continuing medical education for physicians. The ACCF designates this live activity for a maximum of 6.25 AMA PRA Category 1 Credit(s)™. Physicians should only claim credits commensurate with the extent of their participation in the activity.

CNE/Nursing: The American College of Cardiology Foundation (ACCF) is accredited as a provider of continuing nursing education by the American Nurses Credentialing Center’s Commission on Accreditation.

The ACCF designates this educational activity for a maximum of 6.25 continuing education hours. Requirements for successful completion are attendance in a session in its entirety and completing the evaluation tool.

While offering credits as noted above, this program is not intended to provide extensive training or certification in the field.

TARGET AUDIENCE

This meeting is designed for Cardiologists, Cardiology Administrators, Nurses, Nurse Practitioners, Clinical Nurse Specialists, Physician Assistants, Pharmacists and any other interested specialties.

LEARNER OBJECTIVES

Upon completion of this program, attendees should be able to:• Define the proper duration of dual anti-

platelet therapy following drug-eluting stents.

• Evaluate risks and benefits of using DAPT in combination with target-specific oral anticoagulants.

• Identify challenges and opportunities for success when establishing a new structural heart program.

• Recognize the role of novel structural cardiac interventions in contemporary practice.

• Define the current indications and appropriate patients who may benefit for TAVR and MitraClip.

• Identify the importance of team based care in the evolving cardiovascular delivery system.

• Recognize the epidemiology, and long term outcomes for adults with congenital heart disease.

• Identify solutions to improve the quality of care for ACHD patients in 2015 and over the next decade.

• Identify one aspect of each of the four main quality programs in the hospital setting (i.e. 30-day readmissions, hospital acquired conditions, core measures, and 30-day mortality rates).

• Identify one aspect of each of the three main quality programs, (i.e. VBP, MU, and PQRS) that has an impact in the ambulatory practice setting.

• Identify the roles and responsibilities that are necessary for a flexible, dynamic, clinically and financially successful team based model.

• Identify four key elements that are critical to a successful CV Service Line.

• Identify what are the key pillars the College has formed to facilitate the dissemination of clinical guidelines and the improvement of critical practice guidelines to the care delivery team.
