INHS AHA Instructor Packet Attached are the documents that need to be signed by the below parties: Instructor Candidate (the instructor that is applying for initial or renewing status) Instructor Training Center Faculty (TCF) (the person that is monitoring the instructor) Training Center Coordinator (TCC) (Which will be done when the paperwork is turned in to the TC) We need packets for each discipline (ie. ACLS, PALS, BLS, Heartsaver) Initial Instructors: Data Form Monitor Form Candidate Application Copy of Instructor Essentials Certificate Renewing Instructors: Data Form Monitor Form Candidate Application Instructor Tracking Form Once these are signed by the instructor candidate and the training center faculty the instructor can order their instructor card and upload this paperwork packet to the website. https://ctc.inhs.org and “order materials” then select “instructor card” (this will state that it is $10 however you will be charged a total of $40 for the card and the $15 per year alignment fee) Once INHS receives this order we will process and send your instructor card to you. Please allow 10 business days for processing. Please note this is a writeable PDF so you can fill out the top portions of these forms prior to coming to class. Instructions: Training Center Faculty (TCF) or Regional Faculty (RF) should use this form to assess the competencies of instructor candidates and renewing instructors. For each competency, there are several indicators or behaviors that the instructor may exhibit to demonstrate competency.

To be used in conjunction with the Instructor/TCF Renewal Checklist. Role of the RF/TCF Observer: The role of the RF/TCF observer for this monitoring is to observe only. Debriefing or correcting the instructor during the course should be avoided. If critical components are not being completed, contact the TC Coordinator or Course Director outside the classroom setting immediately.
