Note ID: 5C1010227100001 Approaches: Settlement -- Rated 4, Was 4 Referenced Photos: "13", "16" The sidewalks at all 4 corners of the bridge show settlement across the transition between the bridge and the approach. The end left sidewalk settlement is is measured to vary between 1-1/4 and 1-1/2". The begin right sidewalk settlement is measured to vary between 1/2 and 1". The begin left sidewalk settlement is measured to vary between 0 and 1-1/2". The end right sidewalk settlement is measured to vary between 0 and 3/4". At end left 2 feet of sidewalk slab is missing. This is filled by asphalt and is about 1/2" lower. Condition rating of 4 is retained.

Note ID: 5C1010227100003 Span 001 -- Superstructure: Structural Deck -- Rated 4, Was 4 Referenced Photos: "11" Note: Previous description is retained with slight modification because no significant change in deck condition was noted. In the left fascia bay in both spans, the S.I.P. forms are heavily corroded over large areas. The corroded forms extend for the full width of the bay and reach up to 5 feet long. Additionally, the forms are heavily corroded for 1.5 ft wide strip along the left side of girder G-2. The largest areas of formwork corrosion are located at L/5, L/2 and near the end of Span 1 as well as at L/2 and 2L/3 in Span 2. These areas show large sections of the formwork that is rusted through where the underside of the deck is exposed. The exposed concrete is either honeycombed or spalled. The worst areas of spalling were noted near the end of Span 1. The spalling reaches 1" deep and exposes the "chairs" for the reinforcement. There is no exposed main reinforcement. The forms are corroded for the length of both spans along the left side of girder G-2. There are stalactites hanging from the formwork adjacent to the top flange of the girder. The right fascia bay shows similar corrosion along the right side of girder G-4. There are areas where the formwork is corroded thru. The formwork in Bay 4 is in slightly better condition than the formwork in Bay 1. The formwork within the 2 interior bays show significantly less corrosion. There are only narrow strips along the girders. Overall, the corrosion to the formwork appears to affect approximately 25% of the total exposed surface of the bridge. The end diaphragm in Bay 4 at the end of Span 1 shows heavy corrosion with heavy delaminations, section loss and 4 rust perforations. The rust perforations are near the lower right corner and are scattered along a length of 1'-6". The largest hole reaches 2" in diameter. The bottom flanges have been thinned by corrosion. The bottom flange shows section loss that is estimated to range between 40 and 45%. Other primary members in span 1 are in good condition. Internal redundancy is provided in the design. Rating is based on composite behavior of primary members

The concrete headers in Bays 1 and 4 show spalled concrete that reaches 5" deep. Above deck, the joint components appear to be in good condition. The joint seal is covered with roadway dirt and debris. The exposed portions appear to be in good condition.
Note ID: 5C1010227100009 Span 001 -- Pier: Bearings, Bolts, Pads -- Rated 4, Was 4 Referenced Photos: "8" Note: Bearings condition at pier 1 remains almost unchanged. Previous note is retained with slight modification. The Span 2 expansion bearings show packrust between the sole plate and the load plate that appears to somewhat restricting girder end rotation. The worst conditions were noted on the fascia bearings. The packrust between the masonry plate and the bronze sliding surface has caused the sheet to slightly bulge upward approximately 3/16 inches. There is also pack rust forming between the load plates and the side keeper plates. The right keeper plate on the G-1 bearing and the left keeper plate on the G-5 bearing are nearly corroded away. The interior Span 2 bearings are in better condition and would rate "5". The Span 1 fixed bearings show some packrust between the rocker and the masonry plate. There is very little other corrosion to the anchor bolts or either plate and would rate "5".

It sounds very hollow on outside faces. Left fascia pedestial is moderately spalled and bars are exposed. Rest of the pedestials are in good shape and would rate 5.

General Notes To the Next Inspector: The BIN Plates are attached to the begin pedestals below G-3 and G-5. Maintenance & Protection of Traffic was combined among the 9 overhead bridges between the NY198 split and the NYS Thruway (I-90), requiring a single, continuous Lane closure for the inspection of the WB Abutments/Piers, EB Abutments/Piers and the Center Median Piers for all 9 bridges on 3 separate dates. For the EB & WB Right lane closures, it is strongly recommended to rent 2 variable message boards to warn Route 33 traffic of stopped/merging traffic. Sign may read - "CAUTION MERGING TRAFFIC AHEAD" or "MERGING TRAFFIC AHEAD - SLOW DOWN".
