Safety Flag #05080035 was removed by others after removal of all delaminating portions of concrete. The remaining of the underside concrete deck exhibits cracks with efflorescence and areas of hollow and spall concrete. See underdeck condition sketch for details.

The bearing # 8 sliding plate is bent upwards on its left side such that it could slide above the Left guide plate, rendering the guide plate less effective than designed.

Pedestal # 9 has a similar condition on the Left Side, but to a much less extent (less than 1% loss of bearing). The remaining pedestals would rate "5". This condition has not changed since the previous inspection.

Note ID: 0E101018249000F Span 003 -- Superstructure: Joints -- Rated 4, Was 7 Referenced Photos: "15", "16" Pier 3 joint at N/B and S/B roadway near median exhibit deteriorated and crack sealer. water leaks to underside at several location. Item condition downgraded from "7" to "4" rating.

Note ID: 0E1010182490007 Span 003 -- Pier: Bearings, Bolts, Pads -- Rated 4, Was 4 Referenced Photos: "19", "20" There are a few fixed bearings where the steel rocker is not seated flat on the base plate. The worst case is fixed bearing # 3 on the span 4 side. Debris and corrosion were observed between the two bearing components, the rotating plate is uplifted by 3/8" from the base plate, and the two dowels are visible. Due to this condition, fixed bearing # 3 is rated "4". Expansion bearing # 9 on the span 3 side has 1/4" of clearance at 23 degrees F for expansion between the end of stringer S9 and the face of the step pedestal. Similarly, expansion bearing # 2 on the Span 3 side has 5/16" of clearance. Bearings # 6 exhibit heavy surface corrosion to its elements at span 3 and span 4 side . The above conditions have not changed significantly since the last inspection.
