Who should apply?

Any undergraduate student who is an ES intended major, currently enrolled or accepted to UB who will have successfully completed all prerequisite courses with a prerequisite GPA> 2.5 by the end of Spring 2014, who wishes to enroll in the upper division exercise science courses.

2- Program Selection

Choose your intended track: General Exercise Science: General 4 year BS degree

Pre-Physical Therapy: applying to DPT next year before completing BS in ES Prerequisite GPA > 3.0 If you do not meet the minimum prerequisite GPA you can still select the Pre-PT

concentration, however if you are accepted you will initially be placed in the General ES Concentration

BS/ES and DPT: complete 4 year BS before applying to DPT program

BS/MS in Exercise Science & Nutrition: 5-year program, BS in ES and MS in Nutrition Science Prerequisite GPA > 3.0

BS/MPH in Exercise Science & Epidemiology: 5 year program, BS in ES and MPH in Epidemiology Prerequisite GPA > 3.0

**This can change once you are accepted into the program and meet with your advisor. This is not set in stone from the application. **

3- Prerequisite Courses

Enter course number, institution, semester & year, credit hours, grade. Course number: Use UB course number. For example, if you took

CHEM111 @ different institution but it articulated as CHE 101 at UB, use CHE 101.

Institution: List actual institution where you completed the course.

Semester & Year: Enter semester and year you completed course.

Credit Hours: Enter credit hours you received for this course.

Be sure to use what your AAR reports for credits and grades. Use the following slides as examples of common situations.

On your AAR, you will see a section titled Exercise Science BS Acceptance Criteria, use this to fill out the application.

If there is nothing listed under it, click on the green arrow to the left to open the section up. Same for individual course requirements.

If your course is not listed here, it is most likely listed under Courses History section. Use that section to input grades.

Entering Grades

Retaking Courses: As stated in the UG Catalog, all 2nd attempt grades will count as

your official grade, regardless if it is better or worse. This is what you will enter on your application.

If you received an “F” in a course but will be retaking it this Spring the “F” must still be reported and will be calculated in the prerequisite GPA. At this point you do not have a 2nd attempt grade.

2nd attempt grades at another institution (after passing the course at UB) will not be reflected in your GPA or give you a grade on your transcript

Entering Grades

Higher Grade in Higher Level Course: If you earn a higher grade in a higher level course in the same

dept you may report the better grade. However, if this course is also an ES elective you can not use it for both a prerequisite requirement and an elective requirement.

For example, if you earn a “C” in CHE 101, and an “A” in CHE 201, you may enter CHE 201 as your prerequisite course and grade. But you will NOT be able to use this course as an ES elective requirement in the future.

Note: You cannot use CHE 101 and CHE 201 – you must use the courses that would satisfy the sequence (i.e. CHE 101 or 201 and CHE 102 or 202)

Missing Prerequisite Courses

If you are missing prerequisite courses, leave the field blank. You MUST list any course you will not have completed in Spring 2014 and indicate why in the Student Appeals box at the end of the application.

Only in exceptional cases will a student be accepted with missing prerequisites.

Articulated Courses

What does this mean? Articulation refers to the process of comparing the

content of courses that are transferred between educational institutions and deciding if they are equivalent.

A course MUST be articulated through UB to be used as a prerequisite on your PPS Application.

Any course that has been articulated by UB should show up on your AAR with a UB equivalent course number. You will use UB course numbers when filling out this application.

Transfer Course Articulation

If a course has ALREADY been articulated by UB, it will show up on your AAR as the UB Course Number

For example, if you took PSYC 120 at a previous school and it shows up on your AAR as the UB equivalent- PSY 101, it HAS BEEN articulated

Use PSY 101 on your application

If your course has not been articulated it will not show up as a UB course number, the course will have an assigned transfer number such as "TR0005” or an unassigned course # such as “PSY 006”

If you plan to use such a course on your PPS Application, you will need to initiate the articulation process for that course and send us proof that you took the course by February 7th (transcripts, schedule, etc.).

For all courses that have NOT articulated you must proceed accordingly ASAP

Transfer Course Articulation: Already Admitted UB Students

If you have already completed a semester or more at UB and plan to use courses on your PPS Application that have NOT been articulated through UB yet, you need to follow the instructions on the Transfer Articulation Services website: http://www.taurus.buffalo.edu/form.shtml.

Please understand that articulation of unevaluated courses will be determined through the specific department not through ENS (unless it is an ES or NTR course).

For example, if you have an unevaluated chemistry course from a different institution, you will have to fill out an articulation form found on the website above and directly contact the chemistry department for course articulation.

Transfer Course Articulation: Incoming Transfer Students

Send an official transcript to the Office of Undergraduate Admissions, 12 Capen Hall, University at Buffalo, Buffalo, NY 14260 if not sent already.

Next, you will need to mail, fax or email the following information to Transfer Articulation Services: A catalog description of each transfer course and its prerequisites The course syllabus for each transfer course The transfer institution’s name for each transfer course The UB requirement you think each course may work toward Your name and 8-digit UB person number

You must initiate this process as soon as possible. The review process will take a minimum of three weeks; therefore we recommend this process begin at least 3 weeks prior to the PPS Application Deadline. If you know that the process will go beyond the application deadline, please let us know

and send us proof that you took the course (transcript, schedule, etc.).

Transfer Course Articulation: Non-Required Courses

If you have additional courses on your AAR that have not been articulated and are not required for ES, you do NOT have to go through the articulation process.

For example if you took Intro to Dance at a different institution and it shows up as TR002, you do NOT have to go through the articulation process if you will not use this for a requirement.

4- General Education Courses

On you AAR, take a look at the GENERAL EDUCATION section.

For each course requirement on the application, check either Satisfied, Not Satisfied or Waived according to what you AAR indicates.

Students are encouraged, but NOT required to have all General Education Requirements completed by the time they apply to the PPS.

5- Student Notes

6- Student Appeals

If any prerequisite courses will not be completed by the end of Spring Semester 2014, you need to

List each course you will be missing

Indicate WHY you will not have it completed

Only under exceptional circumstances will a student who has missing prerequisites be accepted into the upper division.

Be sure your application is filled out completely before submitting! Triple Check!

Partially submitted applications will not be evaluated.

If your application is incomplete you will only be eligible for the alternate list so be sure to submit the completed application and any supplemental materials (i.e. transcripts, course schedules) by Feb 7!

You may save your data and come back to it at a different time, but REMEMBER to officially submit once it is complete.

9- Official Transcripts

1. If you are currently a UB student and have completed at least ONE full semester at UB and have had all of your official transcripts from previous institutions sent to the Office of Admissions, you do not need to do anything more.

10- Proof of Enrollment

If you are taking any prerequisite course outside of UB this Spring Semester and will be reporting it on your PPS application, you need to supply us with documented proof that you are actually enrolled in the course at the given institution. Proof needs to be provided by February 7th. The application will be considered incomplete if proof is

not submitted by the due date.

What happens now?

You will receive a confirmation email after you submit your application. If you do NOT receive a confirmation email, you MUST

If you did NOT receive a confirmation email and do NOT follow up, your application may be considered LATE.

Acceptance

Applications will be evaluated by the ES Promotion to Professional Sequence Committee

Decision letters will be sent out by early March

If you receive an acceptance letter, you need to respond by accepting or declining your spot in the program by stated deadline. This needs to be in writing and returned to 211 Kimball Tower.

Acceptance is conditional based on successful completion of prerequisite courses spring semester. Grades will be checked at the end of the semester.

Alternate List

Students who are not initially accepted into the program may be added to the alternate list. Students on this list will receive a letter indicating their

position/number on the wait list.

We will contact you and offer you a spot if an opening should become available. This may be as late as the first day of classes fall semester.

There are no guarantees that seats will be open and we cannot predict your chances of gaining a seat. Therefore, if you are on the wait list, you should still plan accordingly for alternate plans.
