Introduction

This leaflet is about the home based treatment you can receive from the Intensive Community Treatment Service.

What is the Intensive Community Treatment Service?

This service works with young people up to the age of 18 with serious mental health problems. These problems have a big impact on you and your family.

Our teams provide you with a lot of support with your mental health problems. This could be by talking with you and helping to make a plan about how to manage your mental health problems.

All the members of our teams are here to support you. If you need help quickly we can see you within 4 to 24 hours.

What is home based treatment? Home based treatment means

that we will support you at home. This can be in a one-to-one meeting or over the phone.

We will work with you, your family and support staff to make a plan to support you.

We hope that we can support you to stop you going into a mental health hospital.

Support when leaving hospital

We can help people to plan their move from a mental health hospital back into the community. We can then support people when they are living back in the community.

Flexible service We can arrange to see you at:

hospital

your home

somewhere else such as your school or your GP surgery

You just have to tell us when

you would like to meet and at what time. We can make sure that we do not meet at times when you are doing any of your activities.

Interpreters Staff can arrange an interpreter

if you need one.

Intensive Community Treatment Service hours are:

8am-9pm, seven days a week, 365 days a year

Outside of these hours you can contact: South of Tyne Initial Response Team Tel: 0303 123 1145

Newcastle/Gateshead Crisis Resolution and Home Treatment Team Tel: 0191 814 8899

They will deal with your call and offer you support over the telephone.

Your details will be passed to the team once we are open again.

Problems, complaints or suggestions Cumbria, Northumberland,

Tyne and Wear NHS Foundation Trust has a complaints policy. We will give you a leaflet about how to make a complaint If you think that we could improve our service then please let a nurse know your idea.
