1. Introduction

The IPPHEC project European conference took place at the University of Deusto, inBilbao, on the 23rd and 24th of November 2009. This conference was the final act of atwo year long project that aimed to develop a common understanding of the importanceof providing psychological help in situations of collective crisis and to improvepreparation for rapid intervention when a large number of people have been involved in acatastrophe, as well as collaboration between services and structures who primarily assistthese people.

During these two days the IPPHEC project conference constituted an ideal platform fromwhich to launch the project’s findings, share information with experts from all over theworld and present the main product of the IPPHEC project, the final recommendations.The outcomes of the IPPHEC project, its website, data platform, etc. were presented andexperts on providing psychological help during catastrophes from all over Europe gavetheir opinion about the IPPHEC recommendations, shared their experiences with theaudience and answered questions. We covered a wide variety of subjects, such as theLondon and Madrid terrorist attacks, the Asian Tsunami, psychological help to those whohelp the victims, the Abruzzo earthquake, etc.

J.Agustín Ozamiz (coordinator of WP2), Miguel Ángel González Torres (WP2 official)and Julia González (Vice-rector of the International Relations department at theUniversity of Deusto) welcomed the participants and opened the conference. They thenhanded over to José Antonio de la Rica (representative of the Basque Health department)who addressed and welcomed the audience. This was followed by Dr. GeraldRockenschaub, a member of the World Health Organisation, who shared his experienceswith the participants. He then gave way to Simona Agger, coordinator of the IPPHECproject, who opened the round of presentations.

2. Registration list

A total of 142 people participated in the IPPHEC conference. These participants camefrom over 20 different countries from all over the world, with representatives fromEurope, Asia, Africa and America.Regarding professional background, most participants had a psychology background,although many psychiatrists, doctors, NGO workers, firemen, etc. were also amongstthose attending the conference.One of the successes of the conference was the large number of attendees, which wasmainly due to the effort made by the partners in sending out both information about theproject and the conference to target groups and possible stakeholders well in advance,and also large numbers of invitations.

3. Speakers and their presentations

Many speakers from different professional backgrounds and various countries tookpart in the IPPHEC project conference, offering their expertise to the audience andparticipating in the debates that arose after the speeches.

IPPHEC project partners presenting project related material;

Simona Agger. WP1 Coordinator of the project. Italian Ministry of Labour,Health and Social Policies. Presentation revolving around the project structure andobjectives.Simona Agger has got her doctorate in Architecture at the University of Venice,then taught for several years first in Venice and then in American Universities andgiven lectures in several universities in Europe and in North America.She has conducted studies about Venice urban situation and problems for Unescoand for the Italian Ministry of the Research and worked as an expert for the ItalianSenate in the preparation of the first “special law” for Venice.Her major book “Urban self management – Planning for a new society” publishedby M.E. Sharpe in White Plains, NW – USA, originally published in Italian, hasbeen translated also into Arabic.From 1998 to 2005 she has worked as a consultant with the Company theEuroprogetti & Finanza, engaged in the evaluation of development projects forthe South Regions of Italy, projects co-financed by the European Commission andthe Italian Government.She has been appointed, starting in 2002, as one of the expert for the Ministry ofHealth for the Evaluation Team evaluating Health Investment’s Programs of the20 Italian Regions and for the projects of the major new hospitals.For the Ministry of Health she has participated in several European projectsinvolving many EU Member States and also new EU Countries such as Bulgariaand Slovakia. At the present time she is General Manager of a European project ofthe program “Community Action in the Field of Health” under the DG SANCO.She has extensive international working experience; in fact, she has worked as anArchitect and Planner in Algeria, Gabon and taken part in internationalresearches. Presently she is developing projects in the field of health andrenewable energy in Senegal.In the US she has been involved in research for renewable energy with theOakridge National Laboratory and at the present time she develops projects ofenergy saving and use of renewable sources for hospitals and health facilities.She represents the Italian Association for Health Architecture and EngineeringSIAIS in the Board of the European Health Property Network.

Gerald Rockenshaub. Representative of the World Health Organisation. HealthRisk reduction and Emergency Preparedness in the WHO European Region

Gerald Rockenschaub joined WHO in May 2004 as Regional Adviser for DisasterPreparedness and Response for the WHO European Region. As programmemanager he coordinates preparedness, response and recovery efforts of the WHORegional Office for Europe, within the overall organizational objective to reducethe health consequences of emergencies, disasters, crises and conflicts and tomitigate their social and economic impact. The regional programme operatesunder the umbrella of the division of health systems, with an emphasis tostrengthen crisis management capacities of national health systems of MemberStates in the region.

Since 1986 he gained progressive international experience in humanitarian healthprojects, coordinating medical humanitarian relief operations for the Red Crossand Save the Children in Ethiopia and Eritrea. For more than a decade hemanaged several health programmes in the Middle East as a consultant for thedevelopment cooperation, mainly implementing health projects in the occupiedPalestinian territories. In 1999 he was in charge of a medical relief project inKosovo setting up primary care mobile clinics for the displaced conflict victims.

For WHO he was coordinating several regional support missions and actions inCentral Asia, the Caucasus (during the Georgia crisis in 2008), in the Balkans andthe North Caucasus; he was part of the WHO team to support Turkey during theAvian Influenza support operation in 2007, and as part of the global WHOresponse supporting the health response in most affected areas hit by the Pakistanearthquake in 2005.

Gerald Rockenschaub is a national of Austria and received a medical doctoratedegree from the University of Graz; during his clinical career he trained as GPand completed a specialization in general surgery, followed by several years ofclinical work in surgery and emergency medicine. He holds a Masters Degree inPublic Health from Boston University, with a focus on Health ServicesManagement.

Beatriz Rodríguez Vega. Psychiatrist at La Paz hospital in Madrid, involved inwork in catastrophes and support to experts who deal with them. Presentation on“Staff Care in Insecure Environments. The Experience of Doctors WithoutBorders in Spain”.Beatriz Rodriguez Vega is a Psychiatrist, doctor in Medicine by the AutonomousUniversity of Madrid. She works as coordinator of the link and psychotherapyUnit of La Paz Hospital and is also an associated teacher at the Psychiatrydepartment of the UAM in Madrid.

She has specific experience in the field of Psycho-oncology, chronic pain, genderperspective and disorders caused by traumaShe regularly collaborates with Doctors Without Borders (MSF) since 2003 and ispart of the Critical Incidents Team who travels to give support to members of theMSF teams in situations of trauma or catastrophe. She has taken part ininterventions with the teams in Darfur (Sudan) and TanzaniaBetween 1995 and 2001 she collaborated with Doctors of the World in some warzones (Bosnia and Kosovo). Recently she has collaborated again with the newteam for Psycho-social support in Emergencies and Catastrophes backed up byDoctors of the World, Spain.She coordinates the Psychotherapy Training and Investigation Unit at la PazHospital, where she receives clinical Psychiatry and Psychology resident doctorsfrom all over Spain who wish to get training.Since 1997 she directs, together with Alberto Fernández Liria, a Master onPerspective Integrated Psychotherapy at the Alcalá de Henares University inMadrid. She also directs courses in the field of Narrative Psychotherapy, in otherprovinces. Due to her professional experience in the field of trauma, dealing withprimary victims and teams affected secondarily, she has directed trainingworkshops in emotional regulation of the therapist in various places in Spain andLatin America.She is currently also the main researcher in various projects financed by nationalagencies (FIS) in the field of psychotherapy in Oncology. Amongst her mostimportant publications there are two books: “The Practice of Psychotherapy: theconstruction of therapeutic narratives (2001) and “Interview abilities forpsychotherapists (2002), both published by Desclée de Brower, as well aschapters in various books and articles in specialist magazines.

Francisco Duque Colino. Presentation on “Psychological Intervention on the 11th

of March 2004”Francisco Duque Colino is a Clinical Psychologist at Gregorio Marañón hospitalin Madrid who led a group of 40 psychologists providing psychological support tovictims of the Madrid bombings. He has been working at this hospital for almostthree decades as an expert of interventions in crisis. He carries out his work in theInter-Consultation Section, mainly in acute trauma and with victims of violence.He is tutor to Psychology internal residents.He also gives courses and seminars directed to the Emergency, Casualty andSpecial Units personnel, always from a practical focus.

Annelieke Drogendijk. Presentation on CIE TOOLKIT, the Public Response toChemical Incident Emergencies.Annelieke N. Drogendijk is a psychologist and since 2001 she works as a researchfellow at the Institute for Psychotrauma. She conducts (both quantitative as wellas qualitative) studies with psycho trauma and cultural diversity as centralobjectives on disasters, on mental health and mental health services utilization. In2005 she worked as a researcher at the Utrecht University, department of ClinicalPsychology. Furthermore, Annelieke is the manager for EU funded projects. Thegeneral aims of these projects are the improvement of the response to criticalincidents and disasters and to deal with the subsequent public health and mentalhealth consequences. This is to be achieved by strengthening national and

Dag Nordanger. Presentation on TENTS, the European Network for TraumaticStress.Dag Nordanger is a doctor in psychology and a specialist in clinical psychology.He has since 1997 been working in the field of trauma, both clinically, ininternational humanitarian work, and with research. His latest writings address theimpact of complex trauma on general health and psychosocial development. Hehas for the last four years been the leader of Resource Centre for Violence,Traumatic Stress and Suicide Prevention - Western Norway (RVTS-West). Thiscentre is the Norwegian partner in the EU funded TENTS project, aiming atimproving post disaster psychosocial services across Europe.

Magda Rooze. Presentation on EUTOPA, European Network for Psycho-socialaftercare in Case of Disaster.Magda Rooze is director of the Impact Foundation, the Dutch Knowledge andAdvice Centre for Post-disaster Psychosocial Care in Amsterdam, theNetherlands. She is a clinical child psychologist and has a Masters degree inBusiness Administration. After studying developmental psychology, from 1977Magda Rooze was a psychologist at different institutions for child and adolescentpsychiatry. From 1985 she was head of the department of an outpatient clinic. Herlatest position was manager of the outpatient clinic of the Academic MedicalCentre for Child and Juvenile Psychiatry in Amsterdam. In 1998 she gained aMasters degree in Business Administration at the Haarlem BusinessSchool/Kingston University London.

Lars Weisaeth. Presentation on Psychological Consequences of the 2004Tsunami.Lars Weisaeth is a psychiatrist and a psychoanalyst. His research has focused onhuman responses to severe events: war, terror, criminal acts, accidents, disastersetc. Since 1984 he has had the chair in traumatic stress at the Medical faculty,University of Oslo, Norway.

Caterina Corbascio Presentation of the specific problem of the persons alreadymentally troubled when involved and victims of catastrophic events and thespecial care needed for them. She is a psychiatrist working at the PiedmontRegion - Italy

Giulia Marino. Presentation on the Abruzzo earthquake.Giulia Marino is a Psychologist who works for the Italian Civil Protection.

Monica Thompson. Presentation on the 7/7 London Bombings. NHS TraumaResponse.Dr Thompson is a Consultant Clinical psychologist who specialises in PostTraumatic Stress Disorder. Dr Thompson works as the head of the adult service atthe Traumatic Stress Clinic in London which is a leading trauma clinic that wasset up in response to the Kings Cross fire which was a major disaster in London in1987. Dr Thompson provides training and advice on trauma interventions, carriesout research, specialist assessments and provides evidence based psychologicaltreatments for people who have encountered traumatic events. She has publishedresearch on the psychological aftermath of rape and the London bombingsresponse. She also works with the government to influence policy regardingfuture psychological preparedness for disasters. At her clinic she has treated many

survivors of war, physical & sexual assaults, child abuse, veterans as well assurvivors of major incidents such as rail crashes in the UK, bombings abroad, theAsian Tsunami and the London bombings.SynopsisDr Thompson coordinated the NHS trauma response which was set up to respondto the psychological needs of those affected by the London bombings. Herpresentation will discuss the innovative screen and treat program which wasutilised in the London bombings response and its implications for future majorincidents.

Stefania di Biasi. Presentation on Psychological Help to Children.Stefania Di Biasi was interested in the study of paediatric diseases and inparticular in the study of child and adolescent psychopathology since the earlyyears of University.In 2000 she began attending the Department of Child and Adolescent Psychiatry(University of Rome “Sapienza”), participating in the clinical and researchactivities under the supervision of Professor Gabriel Levi. She began to study thepsychological and psychiatric effects of childhood trauma, in particular the effectsof sexual and physical abuse.In 2001 she won a scholarship to do an internship at the University of Stockholm(Sweden), Karolinska Institutet, Astrig Lingren’s Hospital (Department ofWoman and Child Health; Supervisor: Prof. Per-Anders Rydelius). During herstay in Sweden she attended at the following clinical and research activities:In November 2002 she obtained the graduation in Medicine and Surgery, with fullmarks (110/110 cum laude) with a thesis entitled: "physical abuse and sexualabuse: psychiatric disorders in preschool and school children” (Supervisor:Professor Gabriel Levi, Dr. Francesca Piperno).In May 2002 she won the scholarship for the Post-Graduated School in Child andAdolescent Neuropsychiatry at the University of Rome "Sapienza".During the years of training she continues to carry out clinical and researchactivities on the effects of childhood trauma.In 2005 she has a period of clinical and research activity at the University ofCambridge (UK), Developmental Psychiatry Section (Supervisor: Prof. IanGoodyear). Jean-Yves Gauthier & Didier Cremniter. Presentation on French Organisationfor Medico-psychological help after a Disaster. Network of CUMP.- Jean-Yves Gauthier did his medical and scientific studies in Rennes. Promptorientation towards systemic approach, learnt in Rome and France. Alwayspracticing in family therapy. Training in Psychoanalysis.He worked as a psychiatrist in the French West Indies, now in Brittany: has beenresponsible of an urgency service during 20 years.Now he is a consultant in individual (analysis practice) and family therapy,oriented in psycho trauma. Responsible of a catastrophe situations interventiongroup for the last 10 years (cellule d'urgence medico psychologique)- Dr. Didier Cremniter is a Psychiatrist at the Hospital of Paris, part of the SAMUservices in Paris. He is a referent of the Medico-Psychological Emergency Team(CUMP) of Paris and member of the National Committee of medico-psychological emergencies.As co-founder of the CUMP, he deals with the coordination of medico-psychological help in the Paris region. He is also in charge of a number ofmissions helping French victims of catastrophes abroad.

4. Mass-media presence

The conference got very good media exposure. The subject covered by the IPPHECproject creates lots of interest and awareness, and the speakers were very well knownpeople in their fields of study who attracted lots of attention in their own right.

10 different media attended the press conference that took place on the 23rd ofNovember (1 TV channel, 5 radio stations, 2 news agencies and 2 newspapers). Ashort report was broadcasted on the 23rd on both channels of ETB, the Basquetelevision, during both the lunch time news and evening news. Both newspaperspublished articles on the IPPHEC conference and project on the 24th (one of whichpublished a whole A3 page) and a third newspaper included a feature on its electronicformat. All radio stations attending the press conference broadcasted live andpreviously recorded interviews with some of the speakers as well as the coordinatorof the conference and the coordinator of WP2.

(IPPHEC: IMPROVE THE PREPAREDNESS TO GIVE PSYCHOLOGICAL HELP INEVENTS OF CRISIS / MEJORA DE LA PREPARACION PARA DAR AYUDAPSICOLOGICA EN SITUACIONES DE CRISIS)

Psychosocial interventions after disasters, terrorism and other events that have greatimpact.

Every year millions of people are affected by disasters, conflicts, wars, terrorist attacksand epidemics. They suffer illnesses, lose their houses, get separated from their familiesor die. According to calculations, 142 million people were affected by 724 disasters in2007.

These events generate social and psychological consequences that often reduce thepossibilities of those involved, to get on with their lives. The main problem these peopleare faced with is the feeling of loss, which appears at several levels: in personalrelationships and material possessions, possibility of finding some income, dignity,safety, trust, self-image and image they have of the community they belong to, and aboveall, a great loss of hope for the future.

The majority of people recover from the impact of the disaster they have experienced,through a process of grief for what they have lost and adaptation to change. However,some people develop psychological problems that may become serious and chronic, andin very severe cases (victims of the holocaust, for example) may affect the nextgeneration.

Several research teams from all over the world have studied human response to disastersand the consequences of such disasters. That is why nowadays there is informationavailable that allows us to develop proposals about best ways to prepare for anemergency, how to act immediately after an emergency and what kind of long termresponse is necessary to favour recovery.

The main principles of psychological and social care in a disaster situation must neverforget that the majority of victims are healthy people who have a natural reaction to anemotionally shocking experience. The main point about psychological help is themobilization of social help resources to victims (family, friends…), so that they can adaptto the experience.

Besides the good intentions and collaboration of those close to the victims, it isconvenient to use specific techniques to help victims of these experiences. For thatreason, it is necessary to train professionals and non professionals involved in helping thevictims, so that they know what to expect and how to respond most efficiently. Thistraining is most necessary in a hospital environment, where many victims go toimmediately after the experienced disaster.

The IPPHEC project is aimed at defining and proposing intervention procedures directedto professionals who deal with catastrophes, whether they have psychological training ornot, in order to improve their preparedness to handle psychological effects in victims of

catastrophes who arrive in hospitals and other treatment places. Recommendations totrain professionals on this field are also presented.

The IPPHEC project has been jointly developed by 11 research teams from severalEuropean countries (Italy, UK, France, Norway, Spain…) and financed by the EuropeanCommission. One of the teams taking part in the project has developed its work in theBasque Country and it is made up of researchers from the University of Deusto, theHealth and Consumers Department of the Basque Government and the PsychiatryDepartment of the Basurto Hospital. Simona Agger (Italy) is the leader and coordinatorof the project.

The conference, which is taking place in the Auditorium of the University of Deusto, inBilbao, on the 23rd and 24th of November is the final outcome of this project. The finalrecommendations, which will be distributed to the European authorities involved inhelping victims of disasters, will be presented during the conference. Numerous nationaland international researchers will discuss the final recommendations and exchangeopinions based on their experiences in this type of situations and also on researchdeveloped in this field.

The IPPHEC project takes one step forward in the improvement of psychological andsocial care of people who all over the world suffer the consequences of catastrophes,whether natural or caused by humans. The joint venture between several research teamsfrom various European countries supposes a fundamental collaboration to solve problemsthat often affect more than one country, and without a doubt highlights the will toconsider the problems of certain human groups as problems that affect all human beings,and therefore must be resolved between all of us.

5. Closure

Simona Agger and J. Agustín Ozamiz conducted the final session of the conference,the IPPHEC Follow up and Conclusions. During this session, the most importantconclusions reached both during the length of the project as well as during theconference were outlined. Three Erasmus students of the European Masters inSustainable Regional Health Systems at the University of Deusto made shortpresentations on the health systems of their own countries. Many of the speakers whohad already made their presentations, as well as some stakeholders and sessioncoordinators were invited to give their opinion on the subjects debated during the twodays. Many questions were asked by the audience during these interventions, creatinga good debate. There were some differences of opinion amongst the experts presentregarding subjects such as debriefing which were discussed, giving everyone a fairchance to express and reason their views.

6. Conclusions

The IPPHEC conference was a success in terms of number of attendees, interestcreated by the subject and especially the participation of a wide variety ofinternational experts. These experts contributed to the debate with their own personalexperience in providing psychological help in events of crisis and also withinformation about how these events are handled at national, regional and local levelsin their home countries.

The ample media coverage also reflects the public interest in the subject of theIPPHEC project and shows the relevance of this project and others that might dealwith the same issues.

The conference highlighted the need for a common response protocol to crisis eventsand brought together the relevant global experts to progress towards this goal.
