Add Discipline Records 

A discipline record is created when a student violates a campus rule. A record is created for each primary offense committed by the student, and each primary offense may have one or more secondary offenses. All secondary offenses are associated with the primary offense by the incident number.

Each discipline record logs the incident date and an offense code that indicates the nature of the offense. Each offense has an incident number, which can be assigned to multiple students. The discipline record can also contain an action code that indicates the steps taken by campus administrators in response to the offense. All action codes are associated with the primary offense.

Because the record is identified by the date and time of the offense, a student cannot have two discipline records with the same date and time.

Maintenance (Single Student) WST0905

The Maintenance (Single Student) page allows you to add a discipline record for a student.

To access the page:

From the Student menu, select Discipline, and then select Maintenance (Single Student). Or, in the field to the left of the Go button on any page, type WST0905 and click Go. The Maintenance (Single Student) page is displayed.

To create a discipline record:  

1. Under Student Identification in the Campus field, type the three-digit campus ID.

2. Locate the student by Stu ID, SSN, Previous ID, or name search.

For help with these search fields, click the Help icon while on this page.

3. Click Find. The student’s demographic data is displayed. 4. The Year field indicates the school year the discipline offense occurred. This field is display only and

cannot be updated. If you signed on with a district-wide sign-on, when you add a discipline record, the field is

automatically set to the ending school year calculated from the incident date. If the incident date is later than July 15, the year will increment by one.

If you signed on with a campus number, the system uses the first and last days' data to calculate the ending calendar year.

5. The Incident Date field defaults to the current date. If necessary, type another date in the MM DD YYYY format. This field is required.

6. In the Time field, if you want to specify the time when the offense occurred, type the time in the HH MM SS format, where HH MM SS is hours, minutes, and seconds. If you leave the field blank, the field defaults to the system time when the record is added.

7. The Incident Nbr field may display an incident number. If the Assign Incident Nbrs field under Discipline Options on Demographic Information

WST0005 is set to D (district) or S (student), the number is assigned by the system and cannot be changed.

If the Assign Incident Nbrs field on WST0005 is set to N (manual), you can enter the incident number.

There can be multiple offenses for each incident number.

8. In the Behavior Location field, type or click to select the code indicating the location of the

incident. These codes are maintained by the district on Behavior Location Codes WST0042.

This field is required for all offense codes except 21-23, 33-34, 41-45, and 55-56. Offense codes 09 and 10 must have 04 in the Behavior Location field.

9. In the Offense Code field, click to select a valid offense code for the infraction committed. A pop-up window will prompt you to select a sort order for the offense codes.

Select Description to see the codes sorted alphabetically by description, or select Code to see the codes sorted alphanumerically by code.

Click Continue. The offense code list is displayed in the order selected.

This field is required. The following should be considered when entering the offense code(s): The offense codes are established by the district on Discipline Offense Codes WST0040. The

PEIMS-equivalent codes and descriptions are also displayed.

You can enter four offenses on this page. The first offense code is classified as the primary offense code for the student on that date and time. All comments and actions will be tied to the primary offense.

If an action code has a PEIMS-equivalent code, the primary offense must have a PEIMS-

equivalent code and will be reported to PEIMS. If the action code does not have a PEIMS-equivalent code, the primary offense may or may not have a PEIMS-equivalent code.

If the primary offense has a PEIMS-equivalent code corresponding to either conduct requiring mandatory expulsion or conduct requiring mandatory placement in a DAEP (optional expulsion), the action code must have an applicable PEIMS-equivalent code.

If the primary offense has a PEIMS-equivalent code corresponding to conduct requiring mandatory expulsion, the action code must correspond to a mandatory action, or action code 28 (mandatory action not taken) must be selected. Otherwise, the Add 28 Action button will be displayed next to the offense table when the action code is saved.

To add code 28 to the Action Code field, click Add 28 Action. The Action Code field and corresponding fields will be completed with the data for code 28. When the data is saved, the Add 28 Action button will no longer be displayed.

Or, you can add an action code that corresponds to a mandatory action. When the action code is saved, the Add 28 Action button will no longer be displayed.

10. Enter the requested data in the remaining offense record fields for any additional offense codes,

teacher referred by, and administrator. The Campus of Responsibility is the instructional campus at which the student was enrolled when the student code of conduct was fractured and the disciplinary assignment was made. The field may also indicate a continuation disciplinary assignment campus that the student would attend under all normal circumstances if not under a disciplinary assignment.

11. To add more than four offense records, click . The primary offense code remains on the page, and

you can add three additional offense codes that will be associated with the primary offense code.

12. In the Action Code field, click to select a valid action code for the primary offense. A popup window will prompt you to select a sort order for the action codes.

Select Description to see the codes sorted alphabetically by description, or select Code to see the codes sorted alphanumerically by code.

Click Continue. The action code list is displayed in the order selected.

The following should be considered when entering action code(s):

The action codes are established by the district on Discipline Action Codes WST0041. The PEIMS-equivalent codes and descriptions are also displayed.

All action codes will be tied to the primary offense code. Action codes cannot be tied to any additional offense codes.

If the action code corresponds to the PEIMS-equivalent code 06 or 26, the Campus of Assignment field will automatically be updated when the discipline record is saved.

If the primary offense has a PEIMS-equivalent code corresponding to either conduct requiring mandatory expulsion or conduct requiring mandatory placement in a DAEP (optional expulsion), the action code must have an applicable PEIMS-equivalent code. If the action code entered does not correspond to an applicable mandatory PEIMS-equivalent code, a message will appear to the right of the primary offense record indicating that the action is incomplete.

If you enter a mandatory PEIMS action code, then the program will not allow a PEIMS action code of 27. The transaction is complete. A PEIMS action code of 27 can only be assigned to a student when there is a non-mandatory PEIMS action code, and the student is enrolled in special education on the date of the offense.

If you enter a non-mandatory PEIMS action code, a message will appear to the right of the primary offense record indicating that the action is incomplete. If the primary offense date is a valid date that falls within the student's enrollment in special education, then the Add 27 Action button will be displayed.

To automatically create a PEIMS action record of 27 using the date for the first action on the page, click Add 27 Action. To create the record manually, you may enter the action and click Save.

 

 

The default value for the PEIMS action record of 27 for the Official Consequence Dates, Lng (length), and Act Lng (actual length) fields is zero.

If the action code corresponds to the PEIMS-equivalent code 05 or 25, the program will determine the number of consequence days. When the data is saved, the program will add together the number of days in the consequence Lng (length) fields or the number of days in the consequence

Act Lgn (actual length) fields, depending on which field was modified. If either total is greater than three days, a pop-up warning box will appear to warn you of the overage.

Click OK to close the box.

To correct the data, you must make the changes on the page and save the data again.

If you enter dates in the Start Date and End Date fields, the system will automatically calculate the consequence length and display it in the Lng field.

If you enter a date in the Start Date field and a consequence length in the Lng field (this is a 3-digit field), the system will automatically calculate the date for the End Date field.

13. Enter the requested data in the remaining action record fields.

For help with the individual fields on the page, click the Help icon while on this page.

Note: If a student does not serve the discipline term that has been assigned (Lng), then you must enter the actual number of days served by the student in the disciplinary placement in the Act Lng field and use one of the Different Length Reason codes. Include only the actual number of instructional days that the student was in attendance when calculating this number. Do not include days absent.

Note: For action code “07”, you must enter the campus 9-digit number for Houston, Parker or MAP in the Campus of Assignment field.

14. To add more than four action records, click . You can add three additional action codes that will be

associated with the primary offense code.

15. Click Save to save the discipline record for the student. A message at the top of the page will indicate that the data was processed.

16. To add comments about the primary offense, click Comments. Comments WST0902 is displayed. The page is described below in greater detail.

17. To add another incident for the student, click New Incident. The student demographic data and

incident date will remain on the page, but all other fields will be cleared to allow you to add a new discipline record.

18. To print a discipline letter to send to the parents, click Print. The report opens in another window, and

the Print window opens over it. The letter contains the infraction type and date, beginning and ending consequence dates, and the administrator associated with the disciplinary action.

Under Select Printer, select the appropriate networked printer, and then click Print. The Print window closes, and you can close the report to return to WST0905.

19. To view a student's withdrawals and reentries, click WD/Reentry Inquiry. Demographic

Withdraw/Reentry Inquiry WST1140 is displayed. Click Return to return to WST0905.

Maintenance (Multiple Students) WST0907

The Maintenance (Multiple Students) page allows you to assign an incident number to more than one student. Once you establish a discipline record for one student, you can add additional students to the incident.

To access the page:

You can access WST0907 in one of the following ways:

From the Student menu, select Discipline, and then select Maintenance (Multiple Students). Or, in the field to the left of the Go button on any page, type WST0907 and click Go.

From Maintenance (Single Student) WST0905, click the Multiple tab.

The Maintenance (Multiple Students) page is displayed.

To add discipline records:

6. To add an additional student to the incident, click under Student Identification (in the top-right corner). The Stu ID field will be cleared allowing you to add the next student ID to the incident record.

7. In the Stu ID field, type the student ID and click Find. The student's demographic information is

displayed.

8. Change or add any offense or action data that needs adjusting. The Primary Offense code must be the same for all students involved in the incident.

9. Click Save. A message at the top of the page will indicate that the data was processed.

If comments were entered for this offense for the first student entered, a pop-up window will ask if you want to duplicate comments for the next student.

Click OK to duplicate the comments entered for the first student for this offense. The comments can be modified for any of the students, if necessary.

Click Cancel not to add the comments.

10. To view the students associated with the incident number, click .

11. To add another incident for the student displayed, click New Incident. The student demographic data and incident date will remain on the page, but all other fields will be cleared to allow you to add a new discipline record.

Comments WST0902

The Comments page allows you to add, change, and view comments about a student related to an offense or actions taken by the school officials. All comments are tied to the primary offense.

To access the page:

The Comments page cannot be accessed from the Discipline menu or the Go button. You must access WST0902 in one of the following ways:

From the Student menu, select Discipline, and then select Maintenance (Single Student) or

Maintenance (Multiple Students).

Create a new discipline record (described above).

Click Comments to add comments. Comments WST0902 is displayed.  

 

 

 

From the Student menu, select Discipline, and then select Inquiry.

Click under Maint for the discipline record you want to update. Maintenance (Single Student) WST0905 is displayed.

  

From WST0905, click Comments.

From the Student menu, select Discipline, and then select Inquiry.

Retrieve a student’s discipline records (described in the following chapter).

Click under Cmts to see the comments for a discipline record. You will be able to view the comments but not update them.

To add or update comments:

Note: If you accessed this page by clicking under Cmts on WST0904, you cannot add, update, or delete the comments. 1. Under Comments, type comments related to the student discipline record, or type over existing text

to edit the comments.

2. Click Save. A message at the top of the page will indicate that the update was completed.

3. To delete all comments from the page, click Delete All Cmts.

4. Click Save. A message at the top of the page will indicate that the update was completed.

5. Click Return to return to the previous page.

Adding Referrals to Law Enforcement, School-Related Arrests, and Bullying

Information

All incidents of Referrals to Law Enforcement, School-Related Arrests, and Bullying are to be entered on WST0905 or WST0907 as follows: Referrals to Law Enforcement:

• Referral to law enforcement is an action by which a student is reported to any law enforcement agency or official, including a school police unit, for an incident that occurs on school grounds, during school-related events, or while taking school transportation, regardless of whether official action is taken.

• In the Referred to Law Enforcement field on WST0905 or WST0907, select Yes or No to indicate if this incident is to be reported to law enforcement.

School-Related Arrest:

• A school-related arrest is an arrest of a student for any activity conducted on school grounds, during off-campus school activities (including while taking school transportation), or due to a referral by any school official.

• In the School Related Arrest field on WST0905 or WST0907, select Yes or No to indicate if the incident resulted in the student's arrest.

Bullying:

In the following Bully Reason fields on WST0905 or WST0907, select Yes or No to indicate if bullying occurred based on the following:

● In the Sex field, indicate if the bullying was based on gender. ● In the Race field, indicate if the bullying was based on race. ● In the Disability field, indicate if the bullying was based on a disability. ● In the Other field, indicate if the bullying was not based on sex, race, or disability, but some other

reason.

These fields should only be used if the incident is a bullying/harassment incident. A Victim button will appear after you click Save if any of the four Bully Reason fields is set to Yes. Click Victim to go to Victim Information WST0919 to enter student IDs for any students who were victims of this incident.

Information for all victims related to each bullying incident is to be entered on WST0919.

Student Restraints

Student restraints performed by MISD Police Officers are keyed on WST0916: 

MISD Police will provide campus administrators with the “MISD Restraint Use Reporting Form” that will be completed

All restraints of all (general and special education) students performed by campus police  must be reported to PEIMS; 

therefore, this information must be keyed into iTCCS.   

 

WST0915‐Restraint Inquiry‐ The Restraint Inquiry page allows you to view restraint data for any student who is 

restrained by campus police, whether or not the student is a special education student. 

WST0916‐Restraint Maintenance‐ The Restraint Maintenance page allows you to collect and maintain restraint 

data for any student who is restrained by campus police, whether or not the student is a special education 

student.  Only restraint records with a staff type code of 02 can be maintained on this page. 

WST0917‐Restraint Comments‐ The Restraint Comments page allows you to enter comments specific to a 

restraint incident.  

You can access WST0915 and WST0916 from the Student > Discipline menu or the Go button.  WST0917 cannot be 

accessed from the Discipline menu or the Go button.  Access WST0917 by clicking the Comments button on WST0916 

or the check mark under “CMTS” on WST0915.    

Instructions for adding restraint data on WST0916‐Restraint Maintenance: Refer to the 

numbered screen shot for the field location of the following steps.

1.    From the Student menu, select Discipline, and then select Restraint Maintenance. The Restraint Maintenance page is displayed allowing you to create a new record. 

You can also retrieve this page by clicking   for a record on Restraint Inquiry WST0915. 

2.     Under Student Identification in the Campus field, type the three‐digit campus ID. 

3.     To update information for a student, you must specify the student. Use one of the following methods: 

In the Stu ID field, type the student's six‐digit district‐assigned ID number. 

In the SSN field, type the student's nine‐digit social security number. The first five digits are hidden as you type. Only the last four digits are displayed. 

In the Previous ID field, type an ID number that was previously used to report the student for PEIMS. 

Click  to search for a student by name. 

4.     Click  . 

The student's most recent restraint record is displayed. 

5.     To change the date, time, or restraint code for the incident, click Chg Date/Time/Code. The Year, Restraint Date, and Time  fields are enabled allowing you to make changes to the data. 

6.     The Restraint Code field will always be set to 08 (Restraint by school district police/resource officer) on this page. 

7.    In the optional Modified field,  type “physical” for physical restraints and “mechanical” for mechanical restraints. 

8.     In the Campus of Restraint Event, type the three‐digit campus ID where the incident occurred. 

9.     In the Instructor field, type the instructor ID for the instructor who handled the incident. Or, click  to select the code from the Current Year Teacher list. 

10.     In the Administrator field, type the administrator ID for the administrator who handled the incident. Or, click to select the code from the Campus Principal/Counselors/Administrators list. 

11.  The Staff Type Code field will always be set to 02 (i.e., District police officer or school resource officer) on this page. 

12.  Click Save to save the information. 

13.  To view the previous or next restraint records for the student, click Prev Restraint  or Next Restraint  . 

14.  To clear the fields and add a new record, click New Restraint. 

15.  To delete the record displayed, click Delete. A message is displayed prompting you to confirm that you want to delete the record. Click OK to continue. The record is deleted. 

16.  To inquire on restraint records for a student, click Inquiry. Restraint Inquiry WST0915 is displayed. 

17.  To add comments related to the record displayed, click Comments. Restraint Comments WST0917 is displayed. Click Return to return to this page.

Notes concerning restraints of special education students: 

There are two staff type codes for restraints of special education students: 

01‐ School district/charter school employee or volunteer 

02‐ District police officer or school resource officer 

Restraints of special education students with staff type code 01 must be entered under the Student > Special 

Education menu on WSE0765‐Restraint Maintenance.  (Restraints of general education students with staff type 

code 01 are not to be entered into iTCCS.)

menu on WST0916‐Restraint Maintenance or under the Student > Special Education menu on WSE0765‐Restraint

Regardless of the screen that is used to enter restraints of special education students, all restraints with staff type 

code 02 will appear on both Restraint Inquiry screens WST0915 and WSE0755.

View Discipline Records

The Inquiry page allows you to view a student’s discipline records. From the Inquiry page, you can select a discipline record to view or update, and then toggle to the maintenance pages to make changes. However, the primary offense cannot be updated from the maintenance pages. You must use the Maintenance (Change/Delete) WST0906 page to change or delete a primary offense from a student’s record.

Inquiry WST0904

The Inquiry page allows you to see a student’s discipline records in chronological order. The page lists each offense, offense date, action, and consequence dates. The page also indicates if the offenses and actions are reported to PEIMS.

From this page, you can toggle to Maintenance (Single Student) WST0905 to update a student’s discipline record, or you can toggle to Maintenance (Change/Delete) WST0906 to change or delete primary offense data for a student.

You may or may not be able to update discipline records on WST0905 depending on the following settings:

If you signed on with a district-wide sign-on and the Universal Discipline Update field on Demographic Information WST0005 is set to N, you cannot update discipline data on this page. If you signed on with a district-wide sign-on and the field is set to Y, you can update discipline data on this page.

If you signed on with a campus-level sign-on and the Use Closing Dates field on Automatic Calendar Build WST0265 is set to Yes, you cannot update discipline records for a closed attendance cycle. A message at the top of the page will indicate that the attendance cycle is closed. Note: You must contact the IMS Department to have your headers opened in order to update discipline records after each cycle has been closed.

If you signed on with a campus ID, you can only access data for the campus. If you signed on with a district-level sign-on, you can access data for all campuses. To access the page: You can access WST0904 in one of the following ways:

From the Student menu, select Discipline, and then select Inquiry. Or, in the field to the left of the Go button on any page, type WST0904 and click Go.

From Maintenance (Single Student) WST0905 or Maintenance (Multiple Students) WST0907, click the Inquiry tab.

To view a student’s discipline records:

1. Under Student Identification in the Campus field, type the three-digit campus ID.

2. Locate the student by Stu ID, SSN, Previous ID, or name search.

For help with these search fields, click the Help icon while on this page.

3. Click Find. The student’s demographic data is displayed. If discipline records exist for the student, they are displayed in chronological order.

For each record, the primary offense is listed first in the Description field. Any associated offenses are listed below the primary offense.

4. To see the discipline records for a range of dates, type dates in the From Date and Thru Date fields in the MM DD YYYY format.

5. Click Find. If discipline records exist for the student for the specified date range, they are displayed in chronological order.

6. To see comments about the student associated with the discipline record, click under Cmts. Comments WST0902 is displayed. Click Return to return to WST0904.

7. To print the student discipline record report, click Print. The report opens in another window, and the Print window opens over it.

Under Select Printer, select the appropriate networked printer, and then click Print. The Print window closes, and you can close the report to return to WST0904. Be aware of the following details when you print the report:

If you signed on with a district-level sign-on, a campus is not entered, and you do not enter data in the From Date and Thru Date fields, the system will default to the current date. If the month is greater than six, it will add one to the year and will print records for that discipline year.

If you signed on with a district-level sign-on and entered data in the From Date and Thru Date fields, all discipline records will be printed for the student for all campuses for the specified date range.

If you entered a campus ID and do not enter data in the From Date and Thru Date fields, the system uses the campus calendar to determine the discipline year.

If you entered a campus ID and entered data in the From Date and Thru Date fields, all discipline records will be printed for the student for the specified campus and date range.

To update a student’s discipline record:

You cannot change or delete a student’s primary offense from this page. To change the student’s primary offense, use Maintenance (Change/Delete) WST0906, which is described below. 1. Retrieve the student’s discipline records as described above.

2. To update an existing discipline record, click under Maint for the record you want to update.

Maintenance (Single Student) WST0905 is displayed showing the complete discipline record.

Update the fields on WST0905, as needed. You can also update comments, if necessary.

For help with the individual fields on WST0905, click the Help icon while on the page. The page is also described above under Add Discipline Records.

Click Save. A message at the top of the page will indicate that the data was processed.

To add students to the discipline record, click the Multiple Students tab. Maintenance (Multiple Students) WST0907 is displayed.

For help with the individual fields on WST0907, click the Help icon while on the page. The page is also described above under Add Discipline Records.

To return to Inquiry WST0904, click the Inquiry tab.

3. To change or delete primary offense data for a student, click under Chg/Del for the record you want to update. Maintenance (Change/Delete) WST0906 is displayed. The page is described below in detail. Click Return to return to WST0904.

Maintenance (Change/Delete) WST0906

The Maintenance (Change/Delete) page allows you to change or delete a primary offense from a student’s discipline record. If you delete the primary offense, the other offense codes, action codes, and comments associated with the primary offense are also deleted. If multiple students are associated with the discipline record, the record is only changed or deleted for the selected student. The record is not changed or deleted for the other students.

To access the page:

The Maintenance (Change/Delete) page cannot be accessed from the Discipline menu or the Go button. From the Student menu, select Discipline, and then select Inquiry.

Retrieve a discipline record (described above).

Click under Chg/Del for the discipline record you want to update.

The Maintenance (Change/Delete) page is displayed.

To update a discipline record:

1. The Incident Date field displays the date on which the incident occurred. If necessary, you can type a different date in the MM DD YYYY format.

2. The Time field displays the time at which the incident occurred. If necessary, you can type a different time in the HH MM SS format.

3. The Incident Nbr field displays the incident number. If necessary, you can type a different six-digit maximum incident number.

4. In the Offense Code field, type another two-digit offense code, or click to select the code from a list.

5. The Modified field is an optional, user-defined field. If used by your district, you can add or change the six-character code for the primary offense. To change the Modified field for any associated offenses, use Maintenance (Single Student) WST0905.

6. Click Save. A message at the top of the page will indicate that the discipline data was modified.

7. To delete the primary offense code, other offense codes, action codes, and comments associated with the primary offense, click Delete. Any other students associated with the same incident number are not affected. A pop-up window will be displayed to confirm that you want to delete the data.

Click OK to delete the data. A message at the top of the page will indicate that the student information was deleted.

Click Cancel not to delete the data.

8. Click Return to return to Inquiry WST0904.

Jobs and Reports

The IMS Department runs discipline summary reports for each campus at the end of each grading period. These reports will be made available on the IMS shared drive (admpeims-ims) or Report Manager or will be sent on the pony for review, correction, and verification.

DG18M - Offenses & Actions by Grade Level & Ethnicity (JST0551) provides a matrix of discipline offenses and discipline actions by grade level and ethnicity as well as percentage of students who have discipline records.

DG18S – Student Detail Report (JST0592) provides a list of students that includes grade, sex, ethnicity, offense, action codes, and teacher name.

DG18T – Summary Report by Teacher (JST0595) provides a matrix by teacher with report counts broken down by sex and ethnic groups 1-5.

DG18D – Cumulative PEIMS Detail Report (JST0635) provides a disciplinary report of all PEIMS reportable offenses and actions to date.

DG18E – Six Weeks PEIMS Detail Report (JST0635) provides a disciplinary report of all PEIMS reportable offenses and actions for a specific grading period.

Note: Additional error reports may be in your folder at the end of each grading period. These reports will only be in your folder if you have errors that require correction.
