Heavy editing this is probably two separate photos maybe 3. The first photo is that of the clock the 2nd is the background of flowers and the 3rd photo is of the hand holding the woman’s face
Symbolism and foreshadowing for the plot of the book is shown in the front cover. Wearing your heart on your sleeve is a theme of the book, which is shown here by having the title of the book on the wrist
Lighting: is above the person head as you can see the shadows of the person’s head on the hand holding her head. They probably used a harsh light, as the shadow is very strong.
Editing notes: the faded clock is interesting, because although it is faded into the foreground the clock still stands out this is due to the lack of stuff in the photo the simplicity of
Editing: while this photo could have been taken without editing it is far easier and more likely that there are two photos here one of the man and another of the two mouths. Furthermore the white and grey backdrop would have been done in Photoshop.
Notes: this photo would have been taken with a low ISO. The photographer would long exposure time of about 2 seconds. And a high F stop to not get to much detail in the photo.
