Journey of Heroes Application Packet
Trip Dates: October 21st-24th, 2019
Journey of Heroes (JOH) honors veterans by sending them to Washington D.C. to see
the monuments and memorials that were erected in their honor. Non-profit partners,
The Vital Life Foundation and Wish of a Lifetime, created this experience to honor
our veterans for their incredible sacrifices. Journey of Heroes highlights that it
is never too late to see your dreams fulfilled and proves the benefits of leading a
physically, spiritually, mentally and socially fulfilling life. For most of the JOH
recipients, this will be their first time visiting the memorials.

About The Vital Life Foundation: The Vital Life Foundation is a non-profit foundation that supports organizations and
programs that provide meaning and vitality in the lives of seniors. Created by Marquis
Companies, a long-term care, assisted living and home health care company, and its
sister company that provides pharmacy, rehabilitation, staffing and consulting to the
long-term care community, Consonus Healthcare Services, The Vital Life Foundation
has been operating since 2008. To find out more about The Vital Life Foundation,
please visit www.VitaLifeFoundation.org
About Wish of a Lifetime: Wish of a Lifetime (WOL) is a non-profit foundation that fosters respect and
appreciation for seniors by fulfilling life-enriching wishes. WOL envisions a world in
which society embraces aging and the wisdom that accompanies it; where seniors are
celebrated for their accomplishments and sacrifices; and where intergenerational
connections are part of our daily life. Founded by two-time Olympic skier, World Cup
gold medalist, entrepreneur, and former NFL football player Jeremy Bloom in 2008 in
living honor of his grandmother, Wish of a Lifetime has made more than 2,000
wishes come true for seniors from all 50 states. To find out more about Wish of a
Lifetime, please visit www.WishofaLifetime.org.

What to expect: Journey of Heroes lasts a total of four days (Monday-Thursday). Two of those days
include travel, which generally precludes scheduling full days of sightseeing activities
on those days. While activities have varied on trips, participants can expect to visit
the three major war memorials (Korea, Vietnam & WWII), the Arlington National
Cemetery, and the Udvar Hazy Museum (National Air & Space Museum in Chantilly,
VA). Veterans are accompanied by one companion and necessary support staff
provided by Wish of a Lifetime and the Vital Life Foundation. A more detailed
itinerary will be provided closer to the actual trip date.
Please be advised: The JOH trip requires commercial airline travel and may pose
issues to veterans with significant medical needs. Likewise, the JOH itinerary requires
a degree of mobility and can be physically taxing. It is advised that most veterans use
wheelchairs provided by JOH. Additionally, we will require that a Medical Verification
Form is filled out by the primary physician of each veteran. Veterans’ companions are
expected to assist JOH staff in ensuring that the veteran has a positive experience and
is as comfortable as possible.

Journey of Heroes provides Roundtrip airfare for veteran and one companion.
Ground transportation in Washington D.C.
Accommodations: One hotel room for each veteran and their companion for
three nights.
Meals: Breakfast, lunch, and dinner during your stay at the hotel.
Wheelchairs: For all veterans who request them.
Not covered by Journey of Heroes: Transportation and/or meals separate from scheduled itinerary/menu.
Incidentals and souvenirs.
Additional Companions There is no guarantee that additional companions and/or family members can
join the JOH trip as attendance is limited. Each request will be reviewed on a
case by case basis.
Please be advised: JOH will not cover any of the expenses for additional
companions; if they are part of a JOH group meal, take the JOH charter bus, or
are included in any JOH activities that involve expenses, the additional
companion(s) may have to reimburse JOH directly.
