SECTION 1 - Introduction

1.1 Purpose

This purpose of this plan is to provide all-hazards emergency response guidance for KOC first responders, management, facilities, groups, and company directorates. This plan is also the foundation or link for the subsequent implementation of site or threat specific plans or procedures listed under paragraph 1.6.

1.2 Scope

This plan applies to emergencies within KOC controlled areas, and medical emergencies in Ahmadi Township. The state fire and police services will respond to emergencies affecting KOC facilities and infrastructure that are outside KOC controlled areas, including oil rigs/wells and Ahmadi Township. If required, KOC will provide support to state fire and police when KOC resources are at risk.

1.3 Objectives

To minimize the consequences of an emergency affecting people, the environment, KOC property and its business.

Reduce to a minimum any loss of production and/or export of natural resources and restore normality as soon as possible.

Establish an effective incident management system for the control of an emergency, with clearly defined roles and responsibilities.

1.4 Priority of Response

Human life.

Environment.

Protection of Facilities.

Business Continuity, Reputation and Recovery.

1.5 Classification of Emergencies (Crisis)

Emergencies are classified according to one of the following four levels: Category 0 (Minor)

Small incidents that can be effectively dealt with by personnel on site. A Duty Fire officer will confirm incident is over and the site is safe.

Category 1 (Serious)

Incidents that are resolved by KOC first responder resources (fire team, ambulance, HSE, security)

Category 2 (Major)

Significant incidents that require additional management by an incident management team and the KOC Crisis Management Team

Category 3 (Disaster)

As per Cat 2 above, with additional oversight by KPC and National Authorities.

Appendix 2 provides a complete description of the four levels of classification, including the classification of emergencies by the type of hazard (threat).

1.7 Review and Update of Document

This plan must be reviewed and updated annually in accordance with HSEMS Document Control procedure KOC.GE.013. In addition the following changes will require an immediate updating of the plan:

An organizational change in the Company that impacts on the emergency response plan, or alternatively could be at risk from an emergency event at the facility.

Significant deficiencies identified in the Emergency Response Plan as the result of exercises, drills or response to real events.

SECTION 2 - Emergency Response

2.1 General Concept

To help ensure an efficient and timely amount of resources and management oversight are brought to bear upon an emergency, the KOC response will be scaled according to the classification of the incident. Minor incidents will be generally resolved by the facility itself. In the event of a category 1-3 incident, the affected facility will isolate the hazard, security will cordon off the area and warn people, the fire team will initiate fire fighting or other mitigation measures, the ambulance team will evacuate casualties, and HSE will advise on health, safety, and environmental protection measures. Serious Incidents (category 1) will be coordinated by the On-Scene Commander. Major incidents (category 2) will be managed by the KOC Crisis Management Team supported by an on-site Incident Management Team. Support service teams will also establish control centers and standby to provide support. Disaster incidents (category 3) will involve additional oversight by KPC and State authorities.

Notes: On-Scene Commander. The senior employee present of the facility will assume the duties of the on-scene commander until the arrival of the senior officer of the emergency response organization responsible for mitigating the hazard, usually the fire officer. Incident Management Team (IMT). The senior team leader present will assume the duties of the Incident Management Team Leader until the arrival of the Manager of the asset or facility, Team Leader Fire or Team Leader of the asset or facility. Until the arrival of the Incident Management Unit on site, The IMT shall use any suitable & safe location (such as a vehicle or a building) with any means of communication as a temporary headquarters. Crisis Management Centre (CMC). The Manager Security and Fire will assume the duties of the Crisis Management Team Leader until the arrival of the DMD of the asset or facility.

2.5 General Responsibilities of KOC Organizations

The roles and responsibilities of the organizations described in this section should present an effective and timely response for emergencies in KOC. However; there is always the possibility that an organization may be unable to provide the resources or personnel at the time of an incident, which could detract from the overall response effort. Consequently, all KOC organizations are required to respond immediately to any additional requests for assistance made by either the Emergency Control Centre or the Crisis Management Team.

2.5.1 Asset/Facility

Notify KOC Emergency Control Centre (ECC – Tel: 160)

As required, evacuate personnel from the asset/facility to an upwind assembly or safe area (a minimum of 50 meters from the incident site). If there is the potential of a boiling liquid expanding vapor explosion (BLEVE) or if an oil tank is involved in a fire (potential boil over), evacuate to a minimum of 800 meters in all directions. The fire officer and HSE staff will determine the safe distances.

In accordance with established site specific emergency response action plans, implement measures (operating procedures) to minimize the health, economic and environmental consequences of the incident.

Coordinate and control facility activities until arrival of first responders

2.5.2 Emergency Control Centre

Notify appropriate response personnel and organizations in accordance with the KOC Emergency Contact List.

Ensure that the appropriate organizations have responded to an incident or have coordinated alternate/additional support as required.

Provide coordination support to the response organization

2.5.3 On-Scene Commander

Defined as the senior officer of the emergency response organization (fire, police, ambulance, or port captain) that has primary responsibility for dealing with the hazard in question. For most incidents in KOC, this will usually be the Fire Officer.

Responsible for correcting the emergency as promptly as possible without unacceptable risk to those in or near the affected area. He coordinates and provides guidance to all on-scene resources in the affected area.

In the event of a category 2 or 3 incident, the on-scene commander reports to the Incident Management Team Leader.

2.5.4 Directorate (responsible for the affected asset or facility)

Provide an HSE Team for category 1-3 incidents to oversee health, safety, environmental, and clean-up response procedures. If the incident involves a toxic/flammable gas, this team is to include a minimum of 2 authorized gas testers.

Responsible for the corporate (crisis) and incident management of category 2 and 3 incidents affecting its area or facilities.

o Provide the DMD (Crisis Management Team Leader) for the Crisis Management Team.

o Provide the Manager (Incident Management Team Leader), Team Leader of the asset/area, Team Leader HSE, and the Team Leader Maintenance for the Incident Management Team.

Rigless Operations. o The Incident management for any well control situation on producing

wells shall be handed over to the Development Drilling Manager by the Asset Manager/Well Surveillance Manager.

Post Incident: o Initiate HSE Incident Report in HSE Live in accordance with HSE

Incident Reporting Procedure (KOC.GE.007) o Initiate HSE investigation in accordance with HSE Investigation and

Corrective Action Procedure (KOC.GE.008)

2.5.5 Security Team

Coordinate all security matters and liaise with the Vital & Oil Installation Protection Department (V&OIPD) and State security authorities.

Coordinate access of emergency response vehicles and personnel through gates controlled by V&OIPD.

If there is no direction from an on-scene commander or HSE, establish an initial inner safety perimeter (cordon) a minimum of 1200 meters from the incident to mark the perimeter of the emergency awareness zone. This perimeter marks the required measure of safety, in case of a potential explosion or boil-over.

If there is no direction from an on-scene commander or HSE, establish an initial outer security perimeter (cordon) a minimum of 1300 meters from the incident (a minimum of 100 meters from the inner perimeter) to mark the perimeter of the controlled safe zone.

Adjust the boundaries of the inner and outer perimeters as directed by the on-scene commander or HSE.

Security will permit access past the inner or outer perimeters only to those personnel and resources who have a duty or technical requirement to be in attendance, as authorized by either the on-scene commander, HSE or the Incident Management Team (IMT).

Facilitate access and egress for emergency marked vehicles and staff (pass not required).

Identify staging area(s) in the controlled safe zone for incoming emergency response vehicles and heavy equipment (coordinate with HSE and fire).

2.5.6 Fire Team

Carry out fire fighting, rescue, decontamination and any other operation to minimize the health, environmental and economic (damage) consequences of an incident.

In coordination with the asset owner, HSE, and security, identify the safety/work zones around the incident.

As required, coordinate the response of first responders.

Move the Incident Management Unit (IMU) to a safe area outside the inner perimeter. Based on safety considerations TL HSE will finalize the location of the IMU. The staging area for incoming resources will be in a safe area in the vicinity of the IMU.

Once the emergency is over, make sure the incident site is safe.

Provide advice for preventive measures to prevent similar incident from reoccurring.

2.5.7 Ambulance Team

As a minimum, provide an ambulance with 2 paramedics at the incident site and remain on standby in the safe zone until given further instructions or relieved by the Fire Officer in charge.

Request additional resources depending on the casualty level/triage.

Stabilize and evacuate injured personnel from the incident area.

Coordinate the response of first responders evacuating and treating mass casualties.

2.5.8 Asset/ Directorate HSE Team

Oversee health, safety, environmental, and clean-up response procedures.

As required, measure toxic/flammable gas, noise, and heat levels.

In coordination with the on-scene commander and security, identify the perimeters for the emergency awareness and controlled safe zones.

Authorize entry into the Controlled Safe and Emergency Awareness Zones.

Identify injured personnel and obtain statements from witnesses (for subsequent investigation).

Ensure that appropriate clean-up and waste management practices are adhered to in accordance with accepted procedures/ law.

2.5.9 Incident Management Team (IMT) – Category 2 and 3 only

Coordinate support to the on-scene commander.

Authorize and coordinate movement of resources from the staging (safe) area to the incident site (On-Scene Commander)

Assess and mitigate the consequences of the incident on area oil production.

2.5.10 Crisis Management Team (CMT) – Category 2 and 3 only

Coordinate KOC support to the IMT

Assess and mitigate the corporate impact of the incident on KOC oil operations.

Implement appropriate communication strategy to gain media and public support for KOC‟s response/ handling of the incident.

2.5.11 Support Services

Provide KOC specialty capabilities that contribute to minimize the consequences of the incident. Detailed responsibilities are described under section 3.6.

2.6 General Response for a KOC Production Facility

Described in the table below is the general response procedure for incidents at KOC production facilities. In addition, each KOC facility is responsible for its own unique emergency procedures in accordance with their site specific Emergency Response Action Plan (ERAP).

Activate nearest alarm point

Report incident to Main Control Room.

If it is safe, remove any injured personnel to a safe area (only if in danger) and carry out first aid .

Evacuate area and move to assembly point (upwind) for head count to ensure everyone is safe.

In case of hydrocarbon leak (gas or oil)

Stay upwind of the leak (Check windsock)

Stop any HOT work, vehicles, or other devices in the area that could create a spark and ignite the fuel

If practical, close the feed from upstream In case of H2S leak

2.8 Authority for Access into Work/Safety Zones

2.9 Ordering an Evacuation

Regardless of the emergency category, an immediate evacuation shall be conducted if life might be threatened. The evacuation shall be ordered by: Crude Oil & Gas Production facilities : Shift Controller or Operator 1. Export Operations : Shift Supervisor or Operator 1. Marine Operations : Harbour Master Drilling Operations & Exploration : Rig Supervisor (Company Man) Any other equivalent post for other Groups Note: once an evacuation is conducted, no one shall be allowed to enter the premises until the Fire Officer declares the area safe.

2.10 Authority for Helicopter Medical Evacuation

In situations where evacuation of a casualty by ambulance is not timely or effective, evacuation by helicopter may be authorized by one of the following:

2.11 Notification for Incidents Involving Fatalities

The ECC must be informed of any incident that results in a fatality of a company employee, contractor or visitor. The ECC shall notify relevant authorities. Manager Security and Fire shall inform DMD (TS) and Manager Human Resources. DMD(TS) shall inform the C&MD. Also the ECC shall send an SMS to all concerned. In addition, the following will also be contacted in respect to each type of fatality: KOC Employee: Manager Human Resources shall inform concerned Human Resources team leader for their further action. Contractor Employee: The HSE Team Leader shall inform the concerned TL of the controlling team of the contractor. Visitor: Manager Public Relations & Information shall be informed for their further action.

2.12 Declaring the Classification of an Emergency

Minor Due to their nature, minor incidents are not “declared”. They are simply treated and recorded as such based on the information provided by the caller reporting the incident. Category 1 – Serious A serious incident is declared by the on scene commander (normally the Snr. Fire Officer or Fire Officer) at the site of the incident. Note: Similar to minor incidents, serious incidents may not always be “declared”. They may be simply treated and recorded as such based on the response required by the on scene commander. Category 2 - Major

A major incident must be declared by Team Leader Fire or his deputy (Snr. Fire Officer or Fire Officer). If none of them has arrived at the incident location, then one of the following will immediately declare the major incident through the Emergency Control Centre, tel. 160: Crude Oil & Gas Production facilities : Shift Controller or higher. Export Operations : Shift Supervisor or higher. Marine Operations : Harbour Master or higher. Drilling Operations & Exploration : Rig Supervisor or higher. Any other equivalent post for other Groups However, TL Fire or Snr. Fire Officers have the authority to supersede the decision of declaring or non-declaring a Major Incident. The official in charge of fire fighting, controlling the incident, shall liaise with the Snr. Employee present from the asset owner Category 3 - Disaster A disaster incident can be declared by the Crisis Management Team Leader (DMD ) following the declaration of a category 2 incident or it can be called immediately by the Team Leader Fire or his deputy (Snr. Fire Officer or Fire Officer). However, the Crisis Management Team Leader (DMD ) has the authority to supersede the decision of declaring or non-declaring a Disaster Incident.

2.13 Declaring the End of a Major/Disaster Emergency

The Crisis Management Team Leader in liaison with Incident Management Team Leader and the On Scene Commander shall be responsible for declaring the end of the activities controlling the incident.

The Emergency Control Centre, the Ahmadi Telephone Exchange, Security Control Room, radio station, building maintenance, and other support services will call back all those informed by them to notify the end of Major/Disaster Incident.

The radio station will announce the end of Major/Disaster Incident on all radio channels and SMS.

2.14 Communications

All communications shall be through mobile radio. Mobile phones should only be used if a failure occurs to the Radio Station. A radio-equipped ambulance will maintain communication with the Ahmadi Hospital. All communications shall be time logged by the Crisis Management Centre (CMC), IMU, Emergency Control Centre - Ahmadi, Telephone Exchange and other support

services. If damage or failure occurs at the Ahmadi Telephone Exchange, the Emergency Control Centre is to be informed immediately to coordinate or carry out the call out list of the telephone exchange. In the event of a major or disaster incident:

the KOC radio channel EMERGENCY shall be used for inter-department communications. The Communications and Network Team monitors and coordinates communications on this channel.

Departmental communications will continue through the regular radio channels.

Communications links shall be established between the Incident Management Team and the Crisis Management Team.

2.15 Post Incident Review

Following the end of any Category 2 or 3 incident, there shall be at the earliest opportunity a full investigation into the cause of the incident according to HSEMS Procedure KOC GE.008 & “Incident Investigation and Corrective Action Procedure”.

SECTION 4 - Other Emergencies

4.1 Traffic Accidents

All company hired and owned vehicle traffic accidents shall be reported immediately to the Emergency Control Centre (24 Hours), telephone 160.

The Emergency Control Centre - shall inform those according to the call out list for vehicle accidents.

TL HSE upon receiving the message from the Emergency Control Centre - will carry out their procedure

4.2 Total Power Failure

If a total power failure occurs in the country, the response shall be categorized as a Category 1 emergency. Each asset in KOC shall establish a control centre with a standby generator. The Control Centre shall be occupied by the :

4.3 Evacuation Procedure for KOC Buildings

Wardens

Ensure that all people within their respective zones are safely evacuated, i.e.

search their zones,

urge people out where necessary, and

are the last to leave their zones . Account for their zone staff and visitors and report the results to the senior person or senior fire warden so that the fire service can conduct a search for anyone missing. The fire warden discovering the fire (incident) should be prepared to inform the senior fire officer the details of the incident.

4.4 Evacuation Guidelines for KOC Areas

4.4.1 Purpose

This purpose of this section is to provide a general guide to KOC areas for evacuating personnel. Each KOC Area is required to have its own plan that provides specific detail for an evacuation when dealing with a hostile attack on Kuwait, natural disaster, or any other reason that threatens the safety of people throughout one or more areas. Such an incident will be classified as either category 2 or 3. Notwithstanding the guidance provided in this section, it is expected that national authorities will provide additional direction for the implementation of evacuation of areas of Kuwait.

4.4.2 Levels of Evacuation

Either a partial or full evacuation may be ordered as described below. Partial Evacuation When. Implemented on order when there is a period of warning before the onset of the threat. Declared By. C&MD or in his absence the Deputy Chairman, in consultation with area DMD(s) Who. Applies to all non-essential personnel that are not required for either essential area headquarters(HQ) operations, emergency services (fire, ambulance, & security staff) or plant/oil rig operations. As a guide, minimum manning will be defined by the Area Manager or Team Leader Production operations according to their plans and will normally be limited to: a. Essential Area Headquarters Staff b. Emergency Services (Fire , ambulance, Security and V&OIPD) c. Key Staff to maintain operations at Assets/Plants. Full Evacuation When. 4.4.3 General Evacuation Responsibilities

4.5.2 Immediate Action Drill – All Bomb Threats

4.6 Non-KOC Emergencies

4.6.1 Definition

An emergency that occurs outside the company premises and non-KOC related. Such as a fire in a private complex, electrical sub-stations or sister oil company premises. Any authority within the country may request assistance during an emergency, it will be the discretion of the management represented by DMD (P&G) to respond to such emergencies and provide support. DMD (P&G) will announce the Category of response through the Emergency Control Centre – Ahmadi.

4.6.2 Response

The company response will be based on providing fire and other support services to the authority that requested assistance. KOC will normally restrict the provision of support for non-KOC emergencies to Category 1 incidents only. Support for Category 2 and 3 incidents will only be provided with the approval of DMD (TS) .

4.6.3 KNPC Emergencies

Any emergency that occurs within KNPC that has the potential to effect KOC operations will be classified at least as a Category 1 and maybe raised by DMD (TS) to Category 2 or 3. The appropriate KOC call -out procedure shall be implemented. Production Operations Teams in liaison with Export Operations Team will reduce or increase production as per their operational procedure.

4.6.4 Mina Al Ahmadi (KNPC)

The KNPC Major Incident Plan shall be implemented for emergencies pertaining to KOC assets inside KNPC-MAA area. This plan shall be effective for the CMC and Support Services. Team Leader Fire, Area Team Leader & Area Team Leader Maint. shall report to KNPC-MAA Advance Control and remain in contact with the KOC CMC

SECTION 5 - Emergency Preparedness

5.1 Directorate Responsibility

Each KOC Directorate is responsible for ensuring that site specific emergency response action plans (ERAP) are developed and exercised for each of its facilities/ assets. This includes new projects under development. ERAPs must be consistent with this document. Each KOC and subsidiary site must have emergency response procedures clearly displayed for control room personnel. Each KOC office, room, or work area will have emergency response procedures clearly available. Each employee on site will also have on his/her possession instructions of what to do in an emergency.

5.2 Readiness

As part of the plans developed on and off site, all operating teams and teams providing services must establish procedures to ensure a quick and effective response in the event of an emergency.

Each team leader shall maintain lists of personnel and their alternates that are on-call available to cover all times (24/7).

The Emergency Response Coordinator (Emergency Control Centre - Ahmadi) must be advised of all changes affecting team leaders, head of teams, managers and their alternates.

Team leaders are responsible for ensuring that their own call-out lists are up-to-date at all times and copied to the Emergency Response Coordinator.

All concerned teams shall maintain a list of equipment and resources, which will require deployment in an emergency.

Persons nominated for the Incident Management Team (IMT), Crisis Management Team (CMT) and Support Services will designate alternate persons in the event of` their non-availability and advise the Emergency Response Coordinator accordingly. Only nominated persons or their alternate to be at their designated posts.

The Fire Team will maintain the trailer mounted Incident Management Unit(s) (IMU) and will mobilize it at the incident site upon declaration of a major/disaster incident and shall be accompanied by a security patrol.

5.3 Exercises

An emergency or crisis is not the time to begin reading the “plan” - there is not sufficient time. Instead, plans must be exercised in advance on a regular basis to ensure there is a clear understanding of everyone‟s roles and responsibilities. Consequently, exercises have the following objectives:

5.3.2 Exercise Schedule

Exercises will be conducted according to the schedule produced by the Emergency Response Coordinator The Emergency Response Coordinator, in consultation with the C&MD, will coordinate at least two unannounced Major/Disaster Incident exercises per year (Category 2 or 3).

5.3.3 Exercise Performance Measurement

The Emergency Response Coordinator shall monitor and observe all exercises (less minor drills) and provide feedback/recommendations. Category 1, 2 and 3 exercises will be assessed in accordance with the KOC Emergency Response Exercise Performance Measurement System. Team Leaders shall have a post exercise meeting with their Teams to analyze actions taken during the exercise with the purpose of identifying procedures in need of improvement.

5.3.4 Exercise Request and Notification Procedure

Groups and teams are to request and coordinate their exercises with the Emergency Response Coordinator [email protected] . Notification of an exercise must be given a minimum of three working days in advance. The Emergency Response Coordinator shall inform the key KOC departments, State Fire Brigade, and the Vital & Oil Installations Protection Dept (V&OIPD) 24 hours prior to the exercise.

Appendix 2 – Classification of Emergencies (Crisis) Minor Minor fires, hydrocarbon spillage, leaks, release of toxic gas and injury that are managed by in house

facilities personnel. Example: A small fire that is extinguished or a minor hydrocarbon or toxic substance release has been isolated at the same time by the facility occupant. Injured personnel require first aid only. Category 1 (Serious) An event that calls for the support of first responders (i.e. fire crews and any other support services). The Emergency Control Centre - Ahmadi will coordinate support to the on-scene commander. Example: A fire or a hydrocarbon or toxic substance release that is extinguished or isolated with the help of a fire crew and any other support services. Category 2 (Major) A “significant” event, which demands a response beyond the routine and which because of the scale of the impact, is beyond the scope of resolution by first responders alone. Such incidents will likely require the deployment of an Incident Management Unit (IMU) and some support from external bodies in Kuwait. The media will likely express an interest in reporting the event. In this category, the Crisis Management Team (CMT) will occupy the Crisis Management Centre (CMC) at the KOC Main Office Building, to manage the incident and coordinate support to the Incident Management Team (IMT). Depending on resources and other constraints, the CMT leader may wish to call all or selective members of support services for assistance in dealing with the incident. Example: Major fires, a hydrocarbon or toxic substance release that cannot be readily controlled and might spread further to cause an explosion, threatening life or property. Category 3 (Disaster) A disaster incident, which because of the massive scale of the (potential) impact, is beyond the internal capabilities of KOC. KOC requires extensive support from external bodies in Kuwait, or from outside of Kuwait to assist in the response. The CMTs from both KOC and KPC will likely be activated. There will likely be strong scrutiny of the disaster in the media. Example: State fire and other external agencies are required to assist in the response to a massive fire, oil spill, or toxic substance release. Appendix 3 – Safe Limits for Hydrocarbon Vapours In accordance with KOC Hydrogen Sulphide KOC.HE.001 and the H2S Management Program document, the following are H2S exposure effects and standards:

Some workers may experience eye irritation

15 ppm TLV-STEL(2) Some workers may experience eye irritation

50 ppm Mild eye and throat irritation after prolonged exposure.

100 ppm IDLH(3)

Maximum concentration from which one could escape within 30 minutes without a respirator and without experiencing escape impairing or irreversible health effects. Deadens sense of smell in 3 to 15 minutes. May cause coughing, burning of the eyes and respiratory tract.

200 ppm Immediate loss of smell. Pronounced eye and respiratory tract irritation

300 ppm

Respiratory disturbances after 30 minutes. Dizziness. Severe irritation to eyes and respiratory tract.

500 ppm

Respiratory disturbances in 2 to 15 minutes. Dizziness, collapse and unconsciousness after 30 to 60 minutes

700 ppm

Loss of consciousness quickly. Breathing will stop and death will result if not rescued promptly.

1000 ppm

Immediate unconsciousness. Death in 3 to 5 minutes.

(1) - Threshold limit value for eight hour time weighted average exposure

(3) - NIOSH defined Immediately Dangerous to Life and Health concentration

No responders should enter the loss of containment area if any of the following conditions are present:

o Flammable vapour concentration is greater than 20% of the lower flammable limit (LFL). o Hydrogen sulphide concentration is 100 parts per million or more.

Only essential staff, equipped with suitable respiratory protection (self contained breathing apparatus or forced air respirator), may enter the loss of containment area if the following conditions are present:

o Hydrocarbon vapour concentration is less than 20% of the lower flammable limit (LFL). o Hydrogen sulphide concentration is between 10 and 100 parts per million.

Under such circumstances entry should be for essential emergency response only, e.g.

o To shut off the sources of the leak o Search and rescue o To prevent the loss of containment escaping to a wider area

Availability of live fire hose and use of fire proof bunker gear shall be ensured before personnel are allowed to enter loss of containment area having flammable vapor concentration up to 20% LFL for activities described.

For all other loss of containment response work, responders may only work if the following conditions are present:

o Hydrocarbon vapour concentration is less than 10% of the lower flammable limit (LFL). o Hydrogen sulphide concentration is below 10 ppm.

However, even if these conditions are met, respiratory protection may be required, especially if there is a continuing risk of concentrations increasing above these limits, for example due to further loss of spillage or to change in wind direction. Appendix 4 - HAZARD RESPONSE GUIDE FOR EMPLOYEES

Emergency Evacuation Procedures

If it becomes necessary to evacuate the building, you will hear the fire alarm... 1. Remain calm. 2. Leave your office or work area, close windows and doors behind you

avoid taking personnel belongings (slows the evacuation)

turn off air conditioning

3. If you are not on the ground floor of the building: • use the closest stairway to reach ground level;

DO NOT USE THE ELEVATOR • assist anyone having difficulties walking

down the stairs • give right-of-way to Fire Team or other emergency services personnel

4. Exit the building through the closest emergency exit. 5. Proceed to the pre-designated assembly point. 6. Move away from the building (minimum of 50 metres). 7. Do not re-enter the building until advised that it is safe to do so.

Fire/Smoke

If you should discover a fire… 1. Remain calm. 2.Sound the nearest alarm. 3. Call 160. 4. Warn people nearby. 5. Leave your office or work area, closing the door behind you. close windows and

Flood/Water Leak

If there is a flood or water leak from an overhead pipe or roof… 1. Evacuate the area immediately if there is any danger from an electrical source. 2. Call 160 . 3. Identify the source(s) of the water as quickly as possible. 4. Summon assistance from colleagues. 5. Try to stop the flow of water or channel it by: • covering areas directly under leaks with plastic sheeting • catching the leak with buckets or trays • making dams from local objects or from sandbags 6. Cover books, equipment, and computers with plastic sheeting.

DO NOT: • enter any area where electrical equipment is in contact with or immersed in water until you are advised that it is safe to do so • enter any wet area in bare feet (water could be contaminated, contain broken glass, or be mixed with chemicals)

Power Failure/Shut-Down

In the event of a power failure… Power failure during normal working hours: 1. Turn off all non-essential equipment (e.g. computers). 2. If the power remains off for 30 minutes, exit the building and remain nearby. The Building Services Coordinator will check with the electrical utility to determine the situation. 3. If the projected length of the power outage is unknown, physically check with your supervisor or security every 30 minutes for further instructions. 4. If available, set up back-up generator system

NOTE: Without power, the air handling systems in the building will shut down. Because this will affect the interior air quality, the building should be evacuated within 30 minutes of a power failure. Should there be a major power failure in the region that lasts for an extended period of time, the building will be closed to all staff not designated as “essential.” In such cases you should stay at home until further notice, although it may be possible to enter the building for short periods of time on an as needed basis.

Medical Emergency

In case of an accident involving staff or visitors… 1. Call co-workers for help 2. Keep the patient calm, warm, and as comfortable as possible. 3. Ensure the safety of the patient and yourself. 4. DO NOT attempt to move the patient if there is any possibility of head, back, or neck injuries. 5. Maintain an open airway. 6. Check for bleeding. Apply pressure to affected area. 7. Summon assistance from members of staff qualified to administer basic first aid or cardiopulmonary resuscitation (CPR). 8. DO NOT administer any medication (e.g. aspirin and similar products). 9. If an ambulance or medical assistance is required, call 160. 10. If the patient does not require further medical assistance, ensure they are able to return home safely.

Following the accident: 1. Submit an Accident/Incident Report to the HSE within 72 hours. 2. Report the accident and subsequent injury to the appropriate Team leader or Manager.

If you should feel unwell… Inform a colleague who will take charge of the situation and make arrangements for you to be taken home if you are unable to do so yourself.

Elevator Failure

If the elevator should fail for any reason… 1. Remain calm. 2. Press the alarm button. 3. Use the emergency telephone and or call 160

Explosion

In the event of an explosion… If the explosion does not involve a fire: 1. Remain calm. 2. Call 160 and be prepared to give them the following information: • your name and location • the location of the explosion • details about the extent of damage, number of people injured, and the type of injuries 3. Provide first aid (if you are qualified) or summon assistance from members of staff qualified to administer basic first aid orcardiopulmonary resuscitation (CPR).

If the explosion involves fire or fumes are evident: 1. Sound the nearest fire alarm. 2. Evacuate the building IMMEDIATELY, using the emergency evacuation procedures 3. If people around you are injured, assess the situation. If conditions are life-threatening, assist the injured individual to leave the building. Otherwise, provide first aid (if you are qualified) or summon assistance from members of staff qualified to administer basic first aid or cardiopulmonary resuscitation (CPR). 4. Ensure that the assembly point is a safe distance away from the source of the explosion 5. Be prepared to provide information on the explosion to 160 and your supervisor

Bomb Threat

In the event of a bomb threat… If you should receive a telephone call from an individual warning of a bomb: 1. Listen and do not interrupt the caller. 2. Be calm and courteous. 3. Keep the caller on the line as long as possible. 4. Obtain as much information as possible. Questions to ask the caller: 1. What time will the bomb explode? 2. Where did you place the bomb? 3. What does the bomb look like? 4. What is the bomb made of? 5. What is your name? 6. Why did you place the bomb?

Information to record: 1. Your name. 2. The date. 3. The time you received the call (a.m. or p.m.). 4. Approximate duration of the call. 5. Exact wording of the threat. 6. Characteristics of the caller: • gender • approximate age • language • accent (English, Arabic, Hindi, other) • speech (e.g. loud, fast) • diction (e.g. nasal, lisp) • manner (e.g. calm, emotional, vulgar) • background noises 7. Whether or not the caller sounded familiar, and if so, a possible identity. 8. Whether or not the caller sounded familiar with the area. After hanging up the telephone: 1. Inform 160 . 2. Be prepared to evacuate the building if instructed to do so, following the emergency evacuation plan posted

Exterior Chemical Spill

In the event of a major chemical spill in the neighbourhood… The KOC or State HazMat team responds to incidents of this nature. Clean-up could involve several options. The experts might decide to: • evacuate the whole area • advise staff to remain in the building If a major chemical spill should occur during working hours: 1. Remain in the building until advised that it is safe to leave.

If a major chemical spill should occur outside of working hours: 1. If you are at home, you will be notified by a member of Management and advised to stay at home. 2. If you are approaching the office complex on your way to work, you will be directed by the on-site emergency services personnel – either to take an alternate route or to return home if the entire area has been cordoned off.

Earthquake

In the event of an earthquake… During the earthquake: 1. Stay inside. DO NOT run outside. 2. Move away from windows, glass partitions, mirrors, bookcases, filing cabinets, light fixtures, etc. 3. Take cover under a heavy table, desk, or any solid furniture, and hold on. 4. If you can‟t take cover, crouch down. 5. If in a hallway, crouch down against an inside wall. 6. Avoid doorways. Doors may slam shut and cause injuries. 7. Use your arms to protect your head and face from falling debris and splintering glass. 8. Remain in the protected place/position until the shaking stops.

After the earthquake: 1. Remain calm. 2. Call Security to report any fire, damage to the building, or injured persons. 3. In the event of fire or damage, evacuate the building following the emergency evacuation procedures. 4. If people around you are injured, assess the situation. If conditions are life-threatening, assist the injured individual(s) to leave the building. Otherwise, provide first aid (if you are qualified) or summon assistance from members of staff qualified to administer basic first aid or cardiopulmonary resuscitation (CPR). 5. Expect aftershocks – they may occur for some time after the initial quake. 6. DO NOT re-enter the building until it has been declared safe.

Severe Windstorm/Sandstorm

In the event of a severe windstorm/sandstorm… During the storm: 1. DO NOT TRAVEL. 2. Stay indoors and avoid outside rooms with large windows. 3. Go to an inner hallway or smaller inner room without windows, such as a bathroom or closet. 4. Go to the centre of the room. Stay away from corners because they tend to attract debris. 5. Take cover under a piece of sturdy furniture, such as a heavy table or desk, and hold on to it. 6. Use your arms to protect your head and neck.

After the storm: 1. Remain calm. 2. Call 160 to report any damage to the building or injured persons. 3. In the event of serious damage, evacuate the building following the emergency evacuation procedures 4. If people around you are injured, assess the situation. If conditions are life-threatening, assist the injured individual(s) to leave the building. Otherwise, provide first aid (if you are qualified) or summon assistance from members of staff qualified to administer basic first aid or cardiopulmonary resuscitation (CPR). 5. DO NOT re-enter the building until it has been declared safe. 6. Use telephones only for emergency calls.
