The 2012 - 2013 Lil Charmer of theYear award winner will berecognized at the Utah Head StartAssociation conference which willbe held November 8, 2013 inSandy, UT.

Award Criteria

Note: Failure to meet any of thecriteria below will result inautomatic elimination.

1. Applicant must be the parent orguardian of a child who attendedHead Start or Early Head Start duringthe 2012 - 2013 school year..2. The local program must be acurrent member of UHSA.

Requirements

1. Include an essay of not more than700 words outlining how this child madeprogress related to his/her experience inHead Start/Early Head Start. This maybe typed or handwritten.2. Include a photo of the child3. Include a letter of support from a staffmember (can be teacher, aide, busdriver, home visitor) that worked directlywith your child.

Ranking

Applications will be ranked on thefollowing:60 points- Essay on how this child hasbenefited from their Head Start/EarlyHead Start experience40 points- Letter from staff member
