Maple PTO—Parent Survey 2014-2015: The Maple PTO is looking for ways to im-prove and expand the levels of communication and the services we provide to our families and students. We need your help to accomplish this task. Please take a few minutes to answer the following questions. Your feedback will help us in our quest to determine the best ways to communicate with you as well as increase parental par-ticipation in classroom/school activities and events.

All lunch meals include milk and 2 selections from the side bar. Side bar options include: An assortment of celery sticks, baby carrots ,
assorted fresh fruit and canned fruit, fresh romaine salad, sliced cucumbers, 100% fresh juice cup. Most of the bread and grain items
served at lunch contains whole grain as the first ingredient.
We serve fresh Sages apples. EVERYDAY ALTERNATIVES: PB & J, Salad, Yogurt & Graham Crackers. Student Lunch

On the third Saturday of every month the Cleveland Food Bank’s Mobile Food Pantry provides fresh food to
families in need at Celebration Lutheran Church, from 10:00 a.m. until noon. The next food pantry will be open
April 19. A family of four whose annual income is below $48,000 would be eligible to accept food. For complete
eligibility guidelines, go to www.celebration-lutheran.org/pdf/FoodPantryEligibility.pdf. Celebration Lutheran
Church is located at 10621 Auburn Road. If you need transportation to the food pantry, please contact the Char-
