Mr. Jones has fallen and has a large bruise on his right cheek. The distressed family asks: How could staff at the facility have let this happen? One staff person provides more assistance than another to residents. The second staff is mad at the first because she feels he does not follow process. The tension is thick between them every day. Grandma’s special blanket, the one sewn by her five granddaughters, is missing. Grandma’s baby boomer son is hot about it and directs his fury at the first staff person he encounters in the hallway. A family who is visiting their elderly dad, who recently suffered another debilitating stroke, is arguing in his room over decisions about his care. Voices are raised and the situation is quickly escalating. Does any of the above sound familiar? Although providing care to vulnerable seniors suffering from dementia and other mental health pathologies can yield many rewards, sometimes client responsive behaviours can escalate into acts of verbal and physical aggression despite our best efforts. Every year, countless care providers sustain injuries from being grabbed or stuck in various ways,

adversely affecting morale and contributing to employee burnout. With this premise in mind, this highly-interactive session, with the aid of specific demonstrations and video simulations, will show safe, effective, respectful, senior-friendly techniques to manage different types of client aggression. This opportunity may be of particular interest to front-line care providers, personnel in nursing administration and staff educators. John has been consulting in the field of geriatric healthcare for over fifteen years. In addition to holding degrees in psychology, education and social work, John has facilitated well-established programs as well as developed his own curricula geared towards managing behaviours secondary

to cognitive impairment. His professional career, both clinical and consultative, has centered on the most vulnerable seniors suffering from serious mental illnesses including affective disorders, anxiety, psychotic illnesses as well as dementia. He is currently a mental health consultant for Baycrest Health Sciences in Toronto. Most of his current practice involves providing mental health training to long-term-care personnel. He is passionate about assisting staff transfer knowledge to practice while tailoring behavioural management strategies to individual client needs. John has provided education on a variety of different topics to hundreds of professional caregivers in various parts of Canada.

9:05am – 10:00am Reducing Antipsychotic Medication Use in Long Term Care: A Canadian Foundation for Healthcare Improvement

Spreading Healthcare Innovations Initiative Dr. Lori Mitchell, Joe Puchniak, Cynthia Sinclair

One in three long term care residents in Canada is on antipsychotic medication without a diagnosis of psychosis even though the medications are minimally effective in managing behavioural issues and have serious risks associated with them. Through the CFHI EXTRA Program, two Managers with the Winnipeg Regional Health Authority designed an initiative to help healthcare providers better use non-pharmacological approaches to address behavioural issues associated with dementia. Focused in one facility, the initiative resulted in a significant reduction in antipsychotic use and improved quality of life for target residents while saving costs.

To tackle the inappropriate use of antipsychotic medications in long term care in Canada, starting in 2014 CFHI supported 15 healthcare organizations from across Canada as they implemented the WRHA innovation during a 14-month spread collaborative. Over the course of the collaborative 52% of target residents were reduced or completely discontinued from antipsychotic medication. The presentation will provide insight into the design and implementation of non-pharmacological approaches to care planning and of how data can be used in real time to track and communicate performance. The process, experience and outcomes achieved by the WRHA initiative and CFHI spread collaborative have learnings that are applicable across all healthcare sectors.

Dr. Lori Mitchell is a Researcher with the Winnipeg Regional Health Authority (WRHA). Her primary role is to develop, support, and contribute to research, evaluation and quality improvement activities that assist in evidence informed decision-making. In addition, her work assists quality of care, program performance, and policy development. Prior to joining the WRHA, Dr. Mitchell worked for 11 years in academia conducting gerontological and health services research. Her doctoral training is in Community Health Sciences from the University of Manitoba and graduate training in Gerontology from the University of Waterloo and Simon Fraser University. During the 2014/15 CFHI Reducing

Joe Puchniak, MSW, CHE, has many years of experience working in the healthcare field at national, regional, facility and clinical patient levels, with a continued focus on evidence informed quality improvement and promoting best practice. In 2012 Joe completed his EXTRA Fellowship with CFHI and also achieved his Certified Health Executive designation. Joe's and his partner’s work through their EXTRA Fellowship focused on using health data to better manage antipsychotic medications and

improve the quality of care in Long Term Care facilities. This collaborative effort received much positive attention, and has been spread to healthcare organizations across the country through a national CFHI initiative which Joe supported as a faculty member.

Cynthia Sinclair is a passionate educator who is committed to improving the quality of life for seniors in care. She is a 2012 Graduate of the Executive Training for Research Application (EXTRA) fellowship offered through Canadian Foundation for HealthCare Improvement 2012. Currently, Cynthia is the Regional Manager for the Interlake-Eastern Regional Health Authority, Personal Care Home Program, responsible for Standards and Development. As an EXTRA fellow, Cynthia and her team partner developed an intervention project to reduce the use of antipsychotic medications in residents with dementia living in Winnipeg PCHs. The success of this project has led to a national collaborative hosted by CFHI, and Cynthia is working collaboratively with CFHI as lead faculty.

9:05am – 10:00am Tweeting, Pinning and Posting: Social Media in Long Term Care

Marissa Ambalina Facebook, Twitter and other social media platforms have grown up, left home and joined the work world. They are quickly transforming the way people in health care share and receive information. Despite its growing popularity and relevance, many working in health care remain reluctant to use social media and are unsure of even where to begin. This engaging exploration of online tools will explain the value of social media in Long Term & Continuing Care. It will answer the questions: Who is using social media in long term care? How are they using it? How can you benefit? How can you use social media to stay in the know? Canadian Virtual Hospice was identified as a world leader in social media by the International Journal of Palliative Care Nursing. The presenter, Virtual Hospice Communications Consultant Marissa Ambalina, will demystify social media tools, such as Twitter, Facebook, LinkedIn, and Pinterest and share insights, do’s and don’ts, stories and lessons learned by drawing upon Long Term & Continuing Care examples. The session takes communications beyond newsletters, and conferences and into a realm of exponential sharing, learning, and connecting in a global community. Marissa Ambalina is a Communications Consultant with the Canadian Virtual Hospice. In this role, she focuses on raising awareness and public engagement, website content management, and promoting interaction with the website through the use of social media. She is a graduate of the Asper School of Business from the University of Manitoba with a major in Marketing and additional training in Public Relations from UCLA. For the last three years, Marissa has worked for Grosney Promotions, playing a lead role in the planning, development and production management of the company’s fundraising products, website, social media accounts and marketing material. Marissa also interned in New York for a contemporary clothing company, where she discovered she wanted to transition her career into the non-profit sector.

10:45am – 11:50am Sexuality and Dementia: Strange Bedfellows

Gayle Appel Doll, PhD

Older adults who may wish to have intimate relationships already have two strikes against them because most people believe that older adults are asexual or if they aren’t, should be. But to make matters worse, dementia can complicate many decisions about the ability to provide consent and to remain sexual. How do couples who are affected by

dementia navigate their intimate relationship as memory fades? How do nursing homes and other residential living environments impair the ability for residents to have intimate relationships? Adult children of older residents tend to be the ones making determinations about their parent’s sexuality while in the nursing home. How could education be offered for families and staff that will allow the possibility of continued interest in intimacy? How could staff policies and education improve resident quality of life? All these questions and more will be addressed in this interactive presentation. This topic will be of interest to people working in long-term care as well as family members of persons with dementia. Anyone who is getting older and fears losing

opportunities for sexual expression when they have to move to a nursing home may also be interested. Gayle Appel Doll, M.S., Ph.D., is Associate Professor and Director of the Kansas State Center on Aging where she directs academic programs and a research agenda relating to culture change in nursing homes. She wrote an award-winning book “Sexuality in Long-Term Care: Understanding and Supporting the Needs of Older Adults” and travels extensively speaking about sexuality and dementia. She has recently been quoted in articles for the Provider, the New York Times, and AARP.

10:45am – 11:50am Walking Through Grief – Helping Others Deal with Loss

John Koop Harder Every person at some point in life will experience grief and loss, a profound sense of final separation from something or someone important. Grief is a normal and appropriate reaction to the death of a loved one, the experience of separation/divorce, intergenerational loss or the loss of opportunity. While there are similarities in how individuals work through loss, grief is a complicated and unique

experience that requires helpers to be sensitive to the needs and experiences of those they are supporting. This presentation is designed to give helpers an increased awareness of the dynamics of grief to best support someone who is grieving. Cultural and popular understandings of grief and loss, and the influences these have on how we experience and work with grief will be explored. John has been working and training in the field of trauma recovery since 2000. He is an active clinician, specializing in family therapy, crisis management and trauma healing. John’s understanding of trauma is informed by his experiences working with communities impacted by trauma locally and in Colombia, Albania and Northern Ireland. He currently provides consulting and therapy services for families in crisis through a local community organization and a private practice.

1:30pm – 2:30pm Managing Your Manager so You Both Enjoy the Experience

Ron Martyn MSc Regardless of one’s position, everyone is accountable to somebody! While effectively communicating to subordinates requires good leadership, getting one’s message through to those above can be even more challenging – especially when the message may not be what the manager wants to hear. While working for a great manager is wonderful, one doesn’t usually choose their manager. This session takes a light-hearted, but realistic, look at four steps to managing

one’s manager, regardless of the type of boss one has, so that you both come out as winners. Step 1 – Understanding Bosses (Challenging & Otherwise) – Recognizing the types of bosses (based on their behaviours) - the good, the bad and the ugly. Step 2 - Making Them Look Good – Taking action that elevates the boss, based on the boss type (even when you may have to take a deep breath while doing it). Step 3 - Communicating Your Concerns – Sharing issues and responding to the boss’s demands (again, depending on the boss type) so you don’t get fired. Step 4 - Making You Look Great - Effective self-leadership strategies that always work with reasonable bosses, and seldom result in creating problems for you with unreasonable bosses.

Adults sometimes lose the ability to understand or express themselves due to stroke, head injury or dementia. We can help them to participate in their care and in life by helping them to communicate. Simple but powerful changes to our own communication can support conversation, leading to improved quality of life. These changes are called “supported conversation.” We will use four case studies to demonstrate principles of supported conversation in real life circumstances. Strategies for making information more accessible to people with communication impairments will also be discussed and demonstrated. Allison Baird, MA, CCC-sp is a Speech-Language Pathologist who has worked extensively with older adults for 25 years both as a clinician and researcher.
