Special Olympics Illinois and Special Olympics Chicago were awarded Presence Health’s System Inspire Award for Community Service, given annually to honor a leader or organization that exemplifies the values that inspire Presence Health’s service to the community.

Through programming in sports, health, education and community building, Special Olympics changes the lives of people with intellectual disabilities by solving the isolation, intolerance and in activity they face.

Presence Health recognizes Special Olympics Illinois and Special Olympics Chicago for breaking barriers, inspiring us to promote health and wellness in new ways, and enhancing the quality of life for all people it serves.

What is a value based contract?

– The industry is moving from an approach where health care providers (doctors & hospitals) are paid for bringing in more patients for health care services (“fee-for-service model”) to one where providers are responsible for delivering services to a specific population at a pre-determined price and quality level (“value based model”).

– New contracts with payors and in some cases our own contracts directly with employers are emerging based on this shift.

How is Presence Health participating in this “value-based” space?

Presence Health has negotiated contracts to address virtually every patient’s need whether he/she is covered by:

• MEDICARE – Federal coverage program that covers health care for people over age 65.

• MEDICAID – Jointly run by the State and Federal government, this program covers health care for low income individuals and families.

• COMMERCIAL INSURANCE – Offered by insurance companies (such as Blue Cross/Blue Shield) for purchase and is often provided as a benefit by employers.

• HEALTH INSURANCE MARKETPLACE COVERAGE OPTIONS (AKA “OBAMACARE”) – The marketplace encourages competition among private health plans by offering individuals, families and small businesses an opportunity to learn about a variety of health coverage options to compare cost and benefits and to enroll in coverage.

• EMPLOYER BASED NARROW NETWORKS – These are health insurance plans that place limits on the doctors and hospitals available to those who enroll for this coverage. Less choice = lower premiums.

Medicare Medicaid Commercial Marketplace

Narrow Network

Medicare Value Partners (MVP)

•Medicare Value Partners is a Medicare Shared Savings Program Accountable Care Organization. The goal is to help physicians better coordinate care for Medicare fee-for-service patients.

•Beneficiaries: 39,000 + members

Smart Plan Choice Accountable Care Entity (ACE):

•Smart Plan Choice is the Joint venture of Presence Health Partners and Independent Physician Alliance of Illinois that was selected by the state to operate an Accountable Care Entity (ACE).

•Participating providers: More than 1,600 PCPs, OB/GYN and Specialists and growing.

•Smart Plan Choice would go live July 2014 pending final approvals.

Cigna Collaborative Accountable Care Initiative

•Presence Health and Cigna will participate in a new collaborative accountable care initiative.

•Under the program, PHP will monitor and coordinate all aspects of an individual’s medical care.

•Target launch date : 7/1/14

•Blue Cross Blue Shield Strategic discussions are underway to explore the establishment of a commercial ACO arrangement to begin Jan. 1, 2015.

Presence Health branded product on the health care marketplace

•Presence Health has partnered with Land of Lincoln to offer a Presence co-branded “private label” health insurance product to be sold on the public health insurance exchange.

•Effective date: 1/1/2015

Imagine Health/SmartCare Network

•Called “SmartCare Network,” Presence Health is the anchor health system for this network operated by Imagine Health. Clients include employees of Home Depot, Walmart & United Airlines.

Employer Coalition Direct to Employer Initiative

•Presence Health is participating in finalist discussions that would position us as the anchor system serving a large employer coalition located primarily in the Chicago area.

•Initial target start date: 7/1/14.

Presence Health Partners is a physician-led network of health care providers who work together to provide high-quality, efficient patient care.

Payors are seeing that PHP provides physicians with the tools to manage the health of a set population.

Tools & Services:– Access to innovative technology, like our patient registry, digital patient

engagement solutions and our Care Coordination Program for high-risk patients.

PHP strength is expanding: – As an entity of Presence Health, PHP has more than 1,000 providers in the

network, 150 sites of care and access to four million patients.

Innovative contract options: – PHP is a leader in innovative options like shared savings, accountable care,

clinical integration & exclusive contracts and we have relationships with all major health plans.

The creation of Presence Health Partners is driving these new payor arrangements

As the health care industry continues to evolve to become more focused on what our patients want, information technology is taking center stage.

To ensure that the hundreds of IT projects are reviewed, prioritized and approved in a way that makes sense from a strategic and resource perspective, a new Information Technology governance structure will launch this month.

The Steering Council will provide oversight for prioritization and approval of projects, supported by five Functional Councils with representatives from across the System to assist with decisions and recommendations.

– These councils include Finance, System Services, Data Management, Clinical and Technology.

A new project request form is being developed and will include specific criteria and a new process for requesting support of new IT projects.

The governance project is one outcome of the IT Strategic Planning process that took place in 2013.
