Patient is attracted by HOSPITAL promotion into captive PCPs who are employed by the hospital system. To prevent patients from “leaking” outside the loop, hospitals now are linking PCP compensation to “downstream” specialist revenues. This effectively creates a CLOSED LOOP NETWORK against specialists outside the hospital network. But, if a specialty center aligns with any one center, it excludes referrals from other PCPs for the same reason.

By going “Direct to Consumer” with promotion, a specialty center can attract those patients DIRECTLY who can self diagnose their own problem, skip the PCP snare, and go directly to the specialty center. This strategy works especially well for health problems & symptoms that can be self diagnosed (e.g. back an neck pain and other orthopedic symptoms) and where PCPs don’t provide any value and merely protract or delay appropriate diagnosis. The specialty center can appeal directly to the payor for premium reimbursement based on quality measures, rather than merely hospital affiliation.
