Three crew members died after entering this chain locker. The rusting of the chain reduced the oxygen levels to less than 10%

This hole was cut in the forecastle to drain water. It created an oxygen deficient atmosphere that entered the store and killed two people

This Halogen lamp was put in a chemical cargo hold after it had been cleaned. When the door was opened for inspection it exploded killing two people
