Basic medical care was introduced in the atoll with the establishment of a health centre on 25 December 1970 in the island of Lhaimagu. Health in MaldivesDr Mariyam Shakeela, Minister of Health and Gender, Maldives

Progress on MDGs in Maldives

Maldives is a low lying small island state in the Indian Ocean comprising of 1,190 islands, with a population of 350,000 spread across the 194 inhabited islands. Despite our unique geography and inherent vulnerabilities, the island nation of Maldives has, in the past four decades, shown progress and incredible improvements in its health indicators. The country’s health system is custom designed based on the size of the population and geographical location, offering targeted levels of service. Every inhabited island has a primary health facility, while comprehensive secondary level health services are made available within every atoll and tertiary level health services available at the capital of the country. This structure, coupled with public health programmes, has further facilitated the progress that Maldives has made in the health sector. Future plans exist to develop regional tertiary facilities.

The resilient nature and commitment of the people of Maldives has placed Maldives as the only ‘MDG Plus’ country in South Asia, having achieved five out of the eight Millennium Development Goals )MDGs) well ahead of the agreed timeline of 2015. Maldives is showing potential to go beyond the MDG targets, especially in the health related MDGs 4, 5 and 6 to address the reduction of child mortality, improvement of maternal health and combating of HIV/AIDS, malaria and tuberculosis. Maldives also revels as the only country in the South-East Asia region to eliminate malaria )in 1984) and to have sustained that achievement for 30 years. It is worthy to state that remarkable progress has also been made towards the achievement of the remaining three MDGs: MDG 3, achieving gender equality and women’s empowerment; MDG 7, ensuring environmental sustainability; and MDG 8, developing a global partnership for development.

While we are proud of our achievements, sustaining them poses numerous challenges to the Maldives. Geographical limitations, lack of economies of scale, heavy reliance on an expatriate

Page 91: MINISTERY OF HEATHS PROJECT IN MALDIVES NOT COMPLETED

health workforce and dependence on imports of all medical consumables, pharmaceuticals and other medical products has a profound effect on health expenditure, accessibility and availability of quality health services within the country. The unique nature of Maldives also makes it extremely susceptible to unprecedented climate change events that will unduly affect the already vulnerable and marginalised )often women and children) and thereby intensify existing inequalities.

Furthermore, these extreme weather events are set to impact on the incidence of non-communicable diseases )NCDs), vector borne disease and basic conditions necessary for health development, including access to safe drinking water, food security, shelter and sanitation infrastructure.

The rapid progress in socioeconomic development and lifestyle changes has also prompted a prominent transition to a NCDs morbid situation. Alarmingly, NCDs accounted for an estimated 78 per cent of all mortality in Maldives in 2012. The chronic disease burden has become a huge challenge to health care provision and health expenditure. In today’s interconnected world, Maldives is not and will not be free from risks of emerging and re-emerging communicable diseases, and thus this area should remain a priority.

Despite the difficulties, the primary health care approach to health services, its commitment to ensuring that the Maldivian people continue to enjoy good health and quality of life, has brought systematic reforms to the health sector and enabled Maldives progress towards its health sector achievements. The health system has been undergoing fundamental changes of reform, in relation to legislative and policy aspects, and continues to evolve to meet emerging challenges. Such reforms include the introduction of urgent and appropriate public health measures, with special health programmes targeting adolescents and the prevention of cancer, chronic diseases and non-communicable diseases. Strategies to improve utilisation of enabling platforms such as telemedicine, e-health programmes and the initiatives for increased financial health protection are of equal importance.

Besides these initiatives, Maldives is in dire need of new investments in health promotion, disease prevention, infrastructure, technology and human resources, and availability of medicines to make us better prepared to combat health challenges and continue growth in the health sector.

There is also an urgent need to accelerate the strengthening of the health care system to meet the increasing health care demands and the challenges of social determinants of health amidst financial and resource shortages. Maldives’ position as a country graduating from ‘least developed country’ status and its geography poses unique development challenges.Nevertheless, Maldives hopes for a brighter future and an improved health care system if global collective will and resources are dedicated towards this goal.

Hence, it is with this vision that Maldives wishes to engage in post-MDG framework discussions and reiterate that health is not only crucial to sustainable development, but is a beneficiary of and a contributor to development. Maldives supports the global aspiration towards universal health coverage and control of NCDs at the heart of post-MDG discussions. It is our dearest hope that

Page 92: MINISTERY OF HEATHS PROJECT IN MALDIVES NOT COMPLETED

the developments of sustainable development goals )SDGs) are viewed as a continuation of MDGs and not in isolation with the progress that MDGs have made.

As a country that is extremely challenged by climate changes and unavoidable health risks, it is our wish that SDGs and climate change are jointly addressed, keeping in mind that the links between the two are central to reducing poverty, increasing economic security, enhancing social development and achieving better health for all.

Child and maternal health in MaldivesInfant mortality in Maldives was nine deaths per 1,000 live births in 2012, with an under-five mortality rate of 11 deaths per 1,000 live births. As shown in Graph 1, there has been a consistent decline in the under-five mortality rate since 1990. This decline is impressive and has seen the country exceed its target of 31 deaths per 1,000 live births, as defined by Millennium Development Goal 4 )MDG 4). In 2010 the three most prominent causes of death for children below the age of five years were congenital anomalies )30 per cent), prematurity )17 per cent) and birth asphyxia )15 per cent). Other contributory causes were pneumonia )11 per cent), neonatal sepsis )four per cent) and diarrhoea )three per cent). In the period 2007-11 Maldives had a reported maternal mortality ratio of 140 deaths per 100,000 live births )this figure was estimated at 60 deaths per 100,000 by UN agencies/World Bank in 2010).

Communicable diseases in MaldivesCommunicable diseases along with maternal, perinatal and nutritional conditions in Maldives accounted for an estimated 16 per cent of all mortality in 2008. Prevalence of HIV among people aged 15-49 is 0.1 per cent )2012). Maldives eliminated malaria over the period 1973-1999. Tuberculosis )TB) saw a significant decline in estimated incidence, which dropped by two-thirds, and estimated mortality )when mortality data excludes cases co-morbid with HIV), which dropped by around 90 per cent, in the period 1990-2012.

Non-communicable diseases in MaldivesNon-communicable diseases )NCDs) in Maldives accounted for an estimated 79 per cent of all mortality in 2008. The most prevalent NCDs in Maldives are cardiovascular diseases, which accounted for 44 per cent of total deaths across all age groups in 2008. Non-communicable variants of respiratory diseases, cancers and diabetes contributed 14 per cent, eight per cent and one per cent to total mortality respectively )2008).

Mental health in Maldives

Page 93: MINISTERY OF HEATHS PROJECT IN MALDIVES NOT COMPLETED

There is a lack of information concerning the most commonly diagnosed mental illnesses in Maldives. Neuropsychiatric disorders contributed an estimated 18.7 per cent of the global burden of disease in 2008.

Health systems in Maldivesaldives’ public spending on health was 3.8 per cent of GDP in 2011, equivalent to US$545 per capita. In the most recent survey, conducted between 1997 and 2010, there were 160 doctors, and 445 nurses and midwives per 100,000 people.Additionally, in the period 2007-12, 95 per cent of births were attended by qualified health staff and in 2012, 98 per cent of oneyear olds were immunised with one dose of measles. In 2011, 99 per cent of people were using an improved drinking water source and 98 per cent had access to adequate sanitation facilities. The most recent survey, conducted in the period 2000-11, reports that Maldives has 82 pharmaceutical personnel per 100,000 people.

Maldives has two hospitals on the main island of Malé and six regional hospitals. There are 13 atoll hospitals, 87 health centres, 51 family health sections and 37 health posts. The Ministry of Health and Gender is the primary government ministry in charge of the sector, and has the remit of of meeting the challenge of providing equitable services to a population scattered over many islands. The reorganisation of the health system, with the introduction of atoll hospitals and placement of doctors at health centres, has enabled increased access to medical services for the island communities.

The logistical problems that had to be overcome in order to ensure effective provision of these services included infrequent transport links and high operational costs. Accordingly, providing health care services to the islands has almost doubled the cost of health services delivery. All pharmaceutical products in Maldives are imported, mostly by the private sector. Apart from a specific category of hospital drugs and controlled drugs, all drugs must be bought by patients at private pharmacists. There is no mental health act or plan, and mental health is not mentioned in general health policy. There are 0.3 mental health outpatient facilities and no psychiatric hospitals or psychiatric beds in general hospitals )2011).

Current health issues and progress in MaldivesMaldives is currently working towards achieving the Millennium Development Goals. To achieve the targets for the reduction of child mortality, which forms MDG 4, Maldives should reduce underfive deaths per 1,000 live births to 31, and increase measles immunisation to 100 per cent by 2015. In 2012, under-five mortality stood at 11 deaths per 1,000 live births, and measles immunisation at 98 per cent, so the country has already surpassed its under-five

Page 94: MINISTERY OF HEATHS PROJECT IN MALDIVES NOT COMPLETED

mortality target and is on track to increase immunisation to 100 per cent by 2015. Consequently, it is likely to achieve MDG 4.The global MDG 5 target for maternal health is to reduce the number of women who die in pregnancy and childbirth by threequarters between 1990 and 2015. When applying this target to Maldives, maternal mortality should fall to 208 cases per 100,000 live births. In the period 2007-11 Maldives had a reported maternal mortality ratio of 140 deaths per 100,000 live births )this figure was estimated at 60 deaths per 100,000 by UN agencies/World Bank in 2010) so this target has already been surpassed. Part of the goal also stipulates that 100 per cent of births must be attended by a skilled health professional. In the period 2007-12 this figure stood at 95 per cent, so this target is on track to be achieved by 2015.

MDG 6 aims for a reduction in the prevalence of HIV, malaria and other diseases. Prevalence rates of major communicable diseases such as malaria, HIV/AIDS and tuberculosis are low or declining.

Accordingly, with continued progress, the country is likely to achieve MDG 6.The 2013 World Health Organization )WHO) country co-operation strategy for Maldives reports that Maldives has largely achieved all the health-related MDGs ahead of the 2015 deadline. The WHO has, however, identified the rise of non-communicable diseases in the country as an issue needing further attention. Noncommunicable diseases account for more than 70 per cent of all mortality in the country. Both the obesity rate and the prevalence of low physical activity levels across the population are among the highest in the WHO South-East Asia Region. Furthermore, more than half of the adult population has high cholesterol, and more than a third of adult men are daily tobacco smokers. In the most recent Maldives Health Master Plan, 2006-15, the government acknowledged NCDs as one of the top health priorities. In January 2013 the Ban of Smoking in Public Places regulation was implemented – it is the first tobacco control regulation in the country and that shows steps are being taken by the government to tackle unhealthy lifestyles that can contribute to the rise of NCDs.

THALASSEMA IN MALDIVES

Page 102: MINISTERY OF HEATHS PROJECT IN MALDIVES NOT COMPLETED

Maldivian Thalassaemia Society (MTS) is a non-governmental organization which came into existence in the year 1994 – the sole purpose being to improve and enhance quality of lives of Thalassaemics in Maldives. With support from other parents, the co-founders Mohamed Moosa Didi and Mohamed Rasheed Ibrahim, both parents of Thalassaemic patients, founded this Society because of the urgent need for such an organization at that time.During the infant years, Maldivian Thalassaemia Society was a platform which provided emotional and financial support (when funds were available). However, as time passed, the need for networking, raising awareness, other social and fund-raising activities and most

importantly working at a policy level in advocating for rights of Thalassaemia patients and parents were identified as crucial aims such an NGO should address. The past years has seen MTS working tirelessly to achieve these objectives.

In this context, MTS has focused largely at improving and sustaining the quality of care and clinical management of Thalassaemia patients by exchanging dialogue at Policy level healthcare professionals including those at Ministry of Health and Family, National Thalassaemia Centre and

Page 103: MINISTERY OF HEATHS PROJECT IN MALDIVES NOT COMPLETED

Male’ Health Corporation Services. Many social activities were organized for both Thalassaemia patients and parents and fundraising strategies were established to sustain the programs run at this organization. MTS has also been instrumental in conducting awareness campaigns in Male’ and in outer islands. From 2005 until 2008, one such campaign has been held each year on a large scale.

However, MTS faces multi-faceted problems when trying to achieve these objectives. Based in a country with extremely difficult geographical isolations, it is particularly difficult to achieve the aims as an NGO with limited capacity in terms of both funds and human resources. For example, due to the huge financial burden of travelling within Maldives where all the inhabited islands are scattered widely across the nation, many awareness campaigns and other projects in the islands have to be abandoned in favour of cost effective projects.

Along with these limitations, MTS also faces challenges such as lack of proper mechanisms to sustain funding, lack of technical expertise and lack of policy level commitment including lack of guidelines or strategies to revitalize awareness, prevention and control program and to sustain the best quality clinical management and care for Thalassaemia patients.

A new $7 million award to the Harvard Medical School Center for Biomedical Informatics from the Patient-Centered Outcomes Research Institute (PCORI) will create a far-reaching data platform that will link patients and doctors across 10 health systems around the U.S., fostering critical medical research.

Called the Scalable Collaborative Insfrastructure for a Learning Health System (SCILHS—pronounced “skills”), the network will leverage $48 billion of federal investment in health information technology to enable a common data platform that will cover more than 8 million patients, enabling them to plug universally into a single point of care. This will enable clinicians and patients to participate in research during the patient encounter, researchers said.

LaboratoriesThe Laboratories in the Children's Hospital Informatics Program span a wide range of research interests in bioinformatics and clinical informatics. Our goal is to make significant contributions to biomedical research and patient care by understanding and utilizing various types of genomic and proteomic data and by developing innovative hardware and software technologies.

Intelligent Health Laboratory

Directed by Ken Mandl, the IHL studies the application of medical informatics, computer science, epidemiology, and biostatistics to improve population health, research, and clinical practice. In addition to Mandl's own group, the lab has the Computational Epidemiology Group and the Predictive Medicine Group.

Computational Epidemiology Group

The Computational Epidemiology Group aims to evolve the traditional paradigm of public health practice and surveillance through innovative multi-disciplinary epidemiologic research. The overall goal is to show how emerging technologies can help clarify patterns of disease and promote public health. Our mission has materialized in a diverse set of projects that include describing the emergence of West Nile virus in New York City using satellite data, predicting patterns of Lyme disease based on climate change, analyzing patterns of influenza epidemics, finding new ways to identify problem medications using electronic medical records, understanding the geographic patterns of substance abuse, describing the impact of pollution on chronic disease, and most recently the first documented use of mobile smartphones as public health surveillance tools for both outbreak and post-marketing surveillance.

Computational Genomics

Page 107: MINISTERY OF HEATHS PROJECT IN MALDIVES NOT COMPLETED

We are a bioinformatics group interested in understanding chromatin structure and function in a variety of systems using high-throughput sequencing techniques. We specialize in analysis of ChIP-seq and nucleosome profiling data but also work with RNA-seq, whole-genome sequencing, and other data types. We collaborate with a number of experimental labs, both in the Harvard Medical area and around the world.

Predictive Medicine Group

The Predictive Medicine Group works to realize the vision of predictive medicine and public health. The diverse group of researchers, clinicians, mathematicians, computer scientists, and biologists develop advanced predictive models for a wide range of applications, including clinical risk prediction, predictive pharmacovigilance, health related social networks, and real-time public health surveillance.

Biomedical Cybernetics Lab

The Biomedical Cybernetics Laboratory is an interdisciplinary program of the Harvard Medical School - Partners Healthcare Center for Personalized Genetic Medicine, affiliated with the Children's Hospital Informatics Program and the Harvard-MIT Division of Health Sciences and Technology. Our laboratory brings together researchers from computer science, engineering, artificial intelligence, epidemiology and statistics to develop novel methods for the integrated analysis of biomedical systems.

Natural Language Processing LabThe Natural Language Processing Laboratory's mission is to develop and implement Natural Language Processing (NLP) technologies to apply to the electronic medical record. These technologies include core NLP tasks such as relation extraction, coreference resolution, and parsing, and make use of statistical machine learning methods. In order to use many machine learning methods, manually labeled (annotated) domain- and task-specific data is required. To that end, we are heavily involved in many different clinical document annotation projects. Since manual annotation is a time-consuming, painstaking, expensive process, it is also our goal to develop and use algorithms that minimize the required amount of labeled data required while maximizing the use of existing labeled data. Use cases for clinical NLP include automated phenotyping, cohort identification, and clinical question answering. We are a primary contributor to the end-to-end software system Apache cTAKES (clinical Text And Knowledge Extraction System). The lab is directed by Dr. Guergana Savova.

ProjectsThe research at the Children's Hospital Informatics Program spans a wide range of problems in bioinformatics and clinical informatics. Our goal is to make significant contributions to biomedical research and patient care by understanding and utilizing various types of genomic and proteomic data and by developing innovative hardware and software technologies.

Instrumenting the Healthcare Enterprise for Discovery Research

Since its inception in 2005 i2b2 been designed to provide the instrumentation for using the informational byproducts of health care and the biological materials accumulated through the delivery of health care to – and as a complement to prospective cohort studies and trials - conduct discovery research and to study the healthcare system in vivo. The utility of this approach is demonstrated by the grass-roots adoption of i2b2 by over 84 academic health centers (AHCs) internationally, each implementation of which is a major, local institutional commitment. i2b2 is now at the core of theScalable Collaborative Infrastucture for a Learning Healthcare System grant from PCORI to build a national-scale clinical data infrastructure.

Phelan-McDermid Syndrome Data Network (PMS_DN)

To collect all available patient data from Phelan-McDermid Syndrome (PMS) patients to make meaningful, well-annotated clinical data available to researchers and to share insights with members of the PCORI network. TranSMART platform based on i2b2 is being used to integrate Patient Reported Outcomes and Knowledge extracted from Clinical Notes using cTAKES

SMART Platforms -- the "App Store" for health

Substitutable Medical Apps, reusable technologies A platform with substitutable apps constructed around core services is a promising approach to driving down healthcare costs, supporting standards evolution, accommodating differences in care workflow, fostering competition in the market, and accelerating innovation. Recent developments include the adoption of the SMART API by Cerner and Intermountain Healthcare as well as Hewlett Packard and the Harris Corporation. The SMART Advisory Committee was recently convened including members from HCA, CMS, SureScripts, Lily, the BMJGroup.

Growth Calculator

Growth Calculator is an online anthropometric calculator, helpful for calculating a variety of standard deviation scores and velocities, as well as for predicted height calculations using heights and bone age values.

Health Map

HealthMap brings together disparate data sources to achieve a unified and comprehensive view of the current global state of infectious diseases and their effect on human and animal health. This freely available Web site integrates outbreak data of varying reliability, ranging from news sources (such as Google News) to curated personal accounts (such as ProMED) to validated official alerts (such as

Page 109: MINISTERY OF HEATHS PROJECT IN MALDIVES NOT COMPLETED

World Health Organization). Through an automated text processing system, the data is aggregated by disease and displayed by location for user-friendly access to the original alert. HealthMap provides a jumping-off point for real-time information on emerging infectious diseases and has particular interest for public health officials and international travelers.

Macrobiology

There is an enormous trove of prior knowledge gleaned in the biological sciences. Macrobiology leverages this prior knowledge in the interpretation of whole physiologies (and pathologies) using empirical grounding offered by high-throughput, comprehensive measurements.

Self - Scaling Registries

The Self Scaling Registry project is an open source software platform aimed to simplify multi-institutional patient registry collaboration. Built upon existing successful open source projects i2b2 and SHRINE, the Self Scaling Registry project empowers researchers to form their own data sharing networks, manage data use, and build on top of existing datasets. Our initial deployment of this platform is supporting the CARRA network, a group of pediatric rheumatologists participating from 60 medical institutions, in forming a patient registry to be the basis for future research work, comparative effectiveness studies, and post marketing surveillance. in 2014, we received funding from the National Heart, Lung and Blood Institute to create a self-scaling registry for patients with pediatric pulmonary hypertension

Multi-source Integrated Platform for Answering Clinical Questions (MiPACQ)

Clinical question answering (cQA) systems focus on the physician needs usually at the point of care, or the investigator in the lab. The questions usually asked either require information highly specific to their patient, e.g. the patient’s lab results or previous history, answered by the patient’s health record, or a more general type of information usually answered through generally available information sources.

Pharmaco-Genomics Research Network (PGRN)

We are working on developing a RA disease activity level classifier for clinical notes directly from Electronic Health Records with chart review and with Natural Language Processing techniques.

Shared Annotation Resources (ShARe)

We are developing standards and infrastructure that can enable technology to extract scientific information from textual medical records. We are annotating a 500K word clinical narrative corpus for

Page 110: MINISTERY OF HEATHS PROJECT IN MALDIVES NOT COMPLETED

syntactic information following the Penn Treebank guidelines and for semantic information following the UMLS definitions. The corpus will be made available to the research community in 2014.

Strategic Health IT Advanced Research Projects (SHARP), SHARPn

We are in the process of creating several open source NLP modules for semantic analysis of clinical narratives, which include a module for coreference, relation extraction, and predicate-argument structure of the sentence. We are also currently involved in several annotation tasks that aim to create a richly annotated corpus of clinical texts. This corpus will include multiple layers of syntactic and semantic annotation such as treebank, propbank, and UMLS annotations. We are also involved in projects which focuses on utilizing active learning to reduce the cost of annotation.

Temporal History of Your Medical Events (THYME)

Temporal relations are of prime importance in biomedicine as they are intrinsically linked to diseases, signs and symptoms, and treatments. Understanding the timeline of clinically relevant events is key to the next generation of translational research where the importance of generalizing over large amounts of data holds the promise of deciphering biomedical puzzles. The goal of our current proposal is to automatically discover temporal relations from clinical free text and create a timeline.

TrainingThe Children's Hospital Informatics Program is one of several Boston area research and development groups in medical informatics that together offer the Boston-area Biomedical Informatics Research Training program is a consortium of leading informatics laboratories at Harvard, MIT, Boston University, and Tufts and is supported by a grant from the National Library of Medicine.
