overall wellness. The goal of the voluntary
Wellness Incentive Program (WIP) is to create a
culture of health and wellness within the City of
Fort Lauderdale by providing wellness-related
activities throughout the year to encourage and
engage employees in becoming happier and
healthier at work and home.*

2018 Wellness Incentive Program (WIP)
* This is a voluntary program that is offered to eligible employees who are enrolled in one of the City’s Cigna Medical Plans with the exception of Federation of Public Employees group based on the
Collective Bargaining Agreement.
You’ll earn a (taxable) $500 incentive reward for completing the
program.

What’s New?
Wellness activities will be tracked online through your
myCigna.com account in MotivateMe® eliminating
the need for paper forms.
Except for self-reported activities and goals, wellness
activity completion will be automatically reported.
Overview Who’s Eligible?
All permanent, full-time, active City employees enrolled in
one of the City’s Cigna Medical Plans, effective January 1,
2018 or the 1st of the month following their eligibility date (i.e.
hire date or status change date).

Earn Required (Minimum 500) Points
1. You must register online at myCigna.com to
track your wellness activities in MotivateMe®.
2. Eligible employees must complete the
required wellness activities between
1/1/2018-12/31/2018 to receive a $500
(taxable) wellness incentive, payable after
March 31, 2019 or May 31, 2019 (to allow for
reporting of claims data into your MotivateMe®
account).

Complete Required Activities (300 Points)
Must complete ALL 3 for a total of 300 points: Required Program Goals
Cigna’s Personalized Health Assessment online at
myCigna.com or the Marathon Health Questionnaire from
the City’s Health and Wellness Center

Biometric Health Screening 100 points
Annual Physical (Preventive Exam) 100 points
All services above performed at the City’s Health and Wellness Center are at no cost.
Services outside the Center are subject to your medical plan benefits.

No cost if the Affordable Care Act (ACA) guidelines are met and services are provided
in-network.
