ACADEMIC PROGRESS:

Professional Performance: Vanderbilt University School of Medicine has developed medical student milestones for all six ACGME competency domains. In the professionalism domain, has met or exceeded all of the target competencies.

Pre-Clerkship/Basic Sciences (Foundations of Medical Knowledge - FMK): passed all pass/fail coursework in the Foundations of Medical Knowledge (FMK) phase of her education. In addition to a basic science curriculum, the FMK phase included early introduction to clinical activities through the Continuity Clinic course. was recognized by faculty as an "active and enthusiastic Case Based Learning participant; she was a well-prepared, effective leader and team member. She has consistently made significant knowledge contributions to the group Learning Objectives. She asked mechanistic questions and facilitated the group efforts to work through some difficult material." "She is willing and able to develop new approaches for the group and to implement them. She is organized and willing to 'take charge' when others acquiesce their responsibilities." Peers noted, "You got us all to look at issues from multiple perspectives. You are also incredibly knowledgeable and good at explaining things." and ", you are very knowledgeable and are able to quickly and correctly apply your knowledge base to a new case, which is amazing. Additionally, you are consistently one of the first ones to contribute, or volunteer to go up to the board. Overall, I think your contributions to our group have been of tremendous value."

Core Clerkships (Foundations of Clinical Care – FCC): The FCC phase/clerkship year was graded for this student’s cohort on a Pass/Fail System. Clerkships are presented here in the order in which they were taken by this student.

Surgery - P: did very well on the Surgery Clerkship. She actively participated in classroom discussions showing an impressive fund of knowledge. She was fully engaged, attentive and eager to care for her patients in any way possible. She both answered and asked insightful question demonstrating a deeper understanding of difficult topics. Her clinical abilities were outstanding and easily evident while observing her do H&Ps. She quickly understood the essence of each patient's problem in taking their history, confirmed her suspicions while doing the physical exam and outlined a superb workup and management plan. She functioned at the level of an intern. She will do well in the future. "Excellent clinical knowledge and strong grasp of appropriate anatomy. is consistently prompt, efficient and hard-working. Very nice job on clinical presentation of H&Ps." "It was a pleasure working with . She did an excellent job on the Trauma service. Her breadth of medical knowledge is exceptional and her dedication to seeking learning opportunities is commendable. Great initiative." " did a superb job while on the busy trauma service. She was very engaged in patient care, preemptively seeing patients on rounding on them independently during the day and was essential to our signout and evening rounds. She actively sought teaching moments and, given her enthusiasm, was permitted to act as trauma Jr during several trauma activations."

Psychiatry - P: " was very eager to learn and sought out educational opportunities, including receiving information/teaching from team members, interviewing patients and discussing with team, and requesting ongoing feedback. was pleasant, professional, helpful, and overall very beneficial to have on the team during her rotation. It was a pleasure working with her." "I really enjoyed Kianna's persistence with difficult patients. She upheld her agreements with a particularly challenging patient and was able to demonstrate very clear consistency, which is something this child very much lacked in his home life. I appreciated Kianna's even-keel demeanor as well. She showed a lot of compassion and was very professional. I think Kianna's bedside manner and interviewing techniques are at a level above what I would expect from a 2nd year medical student. I know she is undecided in her future specialty, but I know our department would love to have her join psychiatry."

Neurology - P: did a great job on the neurology clerkship. She demonstrated genuine enthusiasm, energy and interest in clinical neuroscience. She was prepared for patient presentations, and delivered them in an organized and efficient manner. She showed proficiency in performing the neurological exam and was able to localize the lesion on several stroke patients based on examination alone. On the stroke service, she did not shy away from complex or critically ill patients and, with a pleasant and unassuming persona, demonstrated a natural ability to establish rapport with patients and their families, with whom she was able to communicate in a manner and language that they could clearly grasp. " did an exceptional job on the neurology consult service. She performed well above her peers. She was able to take a throughout yet concise history, do a great exam and present to the team with minimal help from residents. She showed great confidence but asked for help very appropriately when needed." In summary, is functioning at a high level at this stage of her training. With her strong clinical performance in the clerkship, she is encouraged to consider a career in neurology.

Medicine - P: had an outstanding performance during her Internal Medicine clerkship. Kianna's level of engagement in the clerkship was at the top. She is a passionate and inquisitive learner which is infectious to those around her. She is ready to step up to the challenge, is not afraid to commit, and seeks feedback for the purpose of continuous improvement. Knowledge and reasoning are well applied clinically. She brings sound bedside skills, easily develops rapport, and nicely balances open and closed ended questions when taking a history. " took a tremendous interest in the care of her patients. It truly felt like she was their doctor. She went above and beyond to see that they got the best care...I was impressed at her abilities to interview patients and ask questions based on her understanding of the differential. She is intelligent and eager and will make an excellent resident in any specialty that she chooses." "One of the best clerkship students I've worked with". Overall, just a spectacular performance. The Department was thrilled to have the opportunity to work with and strongly encourage her to seek additional opportunities in the coming years. Would be an outstanding internist if she were to so choose.

Obstetrics/Gynecology - P: Great team player. Always well prepared for rounds and helped with taking care of complex patient. Easy to work with and engaged. Easy to get along with, works well with providers, RN staff, and patients. Knows when and when not to ask questions and/or speak up. Actively conducts physical exam and requires little assistance with the pelvic exam. is well above her peers for a 2nd year student and deserves recognition. She would make a great OB/GYN!

Pediatrics - P: did an excellent job on the Pediatric Clerkship during her second year of medical school. She was noted to be eager, hardworking, and had great rapport with the families. An attending that worked with remarked that "she was eager to work with team, picks up her own patients, managed several patients for her level of training. Quick learner; asks great questions. Fun to work with."

Advanced Coursework (Immersion): Vanderbilt’s recent curricular revision includes a longer post-clerkship experience, enabling individualization of each student’s education to meet goals driven by both personal interests and general competency development. Students select from a broad menu of courses including Integrated Science Courses (ISCs), Advanced Clinical Experiences (ACEs), Acting Internships (AIs) and Advanced Electives (AEs). The assessment of student performance in this redesigned phase is more rigorous than the historical "senior year" of medical school, with less frequent application of the honors designation. Immersion coursework is graded Honors/High Pass/Pass/Fail unless otherwise designated. Histograms are shown for those courses with a higher number of students and Immersion coursework is presented here in the order in which they were taken by this student. Below are Kianna's assessments for Immersion coursework.

Integrated Science Course (ISC): Medical Imaging and Anatomy - H You were an effective teacher of the group due to your engaging delivery and enthusiasm for your facial anatomy and plastic surgery topics with explanations broadly useful to the class as a whole. You were an enthusiastic team member who contributed to each learning environment in a positive manner. Your anatomic prosection of the facial region and internal heart allowed an in-depth correlation with imaging studies and clinical cases. You displayed a responsible demeanor and fulfilled all your assignments and professional responsibilities with excellence this month. Especially noted were your creative teaching techniques. We believe you have a talent for teaching and hope your incorporate educational activities within your career. In summary, we are delighted to share that both your qualitative and quantitative performances this month earned you an official grade of Honors for Medical Imaging & Anatomy.

Research Immersion: Clinical & Translational - H The Research Immersion course has rigorous requirements and has significantly exceeded these requirements. Project Title: Identification of Immunologic Factors Involved in Aged Mouse Response to Deep Partial Thickness Burns Mentor: Wesley Thayer Mentor Comments: was an outstanding student to have in lab. Her project hit multiple bumps along the road, but she remained optimistic, and was extremely proactive in finding new ways to help progress the project. When we discovered we had unusable results for EGF and RANTES, proposed multiple methods to further investigate the effects EOTAXIN, making our data much more robust. She also found ways to connect our data to our previous work in humans, and her work has led to many ideas for future study. In addition to her work on this project, has also been incredibly productive on other projects in this lab. While waiting for results for her RI project, she took the lead on a study investigating emergency hand coverage in TN and obtained a first author publication in Annals of Plastic Surgery for her work. It is impressive for a student to complete all of the work (and get a publication) for a study in just a few months, and I have rarely seen that kind of work ethic in a student. It is even more impressive that she did this while also making great strides on her research immersion project. In summary, has been an exceptional student to have in lab. She is enjoyable to work with and has been extremely productive during her Research Immersion. I have had multiple students join us for their RI, and stands out amongst her peers as an over-achiever. I am excited that she is staying with us to pursue some of the questions raised from her study, and even more excited to see how her career progresses. She is really on top of her game! Perfect student! In the top 5%. She works well w people, has incredible initiative. Has great wealth of knowledge. She helps others in the lab. Excellent student!

Advanced Clinical Experience (ACE): Pediatric Emergency Medicine - H works hard and is eager to learn. She has excellent history-taking skills and can efficiently evaluate patients. She had confidence when it comes to procedures. She works well with a team and is good at following-up with patients and families. She will be a good resident.

Advanced Clinical Experience (ACE): Plastic Surgery – H Outstanding performance on plastic surgery; excellent communication and professionalism. Continue to work on surgical skills.

Advanced Clinical Experience (ACE): Medical Ethics – H During her month working closely with the Clinical Ethics Consultation Service, demonstrated a keen awareness of how to prioritize factors that bear on difficult ethical issues in patient care. Her willingness to engage patients and advocate for them beyond what is expected is exceptional. She can think broadly about possible solutions to highly complex patient care situations that involve seemingly intractable ethical issues. Her skills of analysis in the process of moral inquiry were noteworthy and helpful.

Acting Internship (AI): Plastic Surgery – H functions like an intern. She is pleasant, hard-working and very bright. Gets along well with the team. Has a keen interest in plastic surgery and is eager. Her surgical skills are better than most interns I've seen this year. I think she is excellent. is doing an excellent job. Truly technically much improved. Her floor work is on par with a good intern. Overall, I have been impressed. has really excelled, plastics comes naturally to her, she gets along with the whole group. She is trusted to suture alone and perform finger blocks on her own. She organizes the students into cases and shows leadership. I will be sad when leaves our team, I've worked quite a bit with . She was absolutely amazing. Really interested in the cases / care of the patients. Always available and wanting to engage in cases. Definitely more mature than her level and technically excellent in the OR (She was closing my cash cases without any hesitancy from me, which is rare for a student!). - honors. Doing very well. Working hard. did a stellar job on service as our AI. She was always prepared with the list before we even showed up to round and had all the surgery consents for the day filled out for us. This in particular showed that she was thinking ahead and planning ahead for the day which not many students or even interns do. has been a great team member. She is prompt, responsive and always actively searching for opportunities to help the team. I have observed her remove a JP drain and she did well. She is always very knowledgeable about the patients. I had the chance to work with last year when she was starting out in her clerkships and very new to surgery and compared to this month, she has improved tremendously as far as knowledge, technical skills and even confidence. She is always happy to help the team. Happy to have her part of our team anytime. She has done an excellent job. We already know she is one of the better students and had high expectations for her, but she went even beyond that. She is always present, to a point that I think she has a twin. She is always helpful and thinks ahead. She's been one of or even the best AI thus far.

SUMMARY: is a brilliant, hardworking, compassionate student and leader who has had an exceptional performance at the Vanderbilt University School of Medicine. is the first in her family to attend college and went on full scholarship to MIT graduating with a double major in Biology and Chemistry. CURRICULUM 2.0

PHASES OF THE CURRICULUM

Foundations of Medical Knowledge (year 1) integrates foundational sciences (considered broadly to include basic sciences, social and behavioral sciences, humanities, health systems science), and physical diagnosis. Case-based small-group sessions provide clinical context and foster skills in identifying learning needs and resources.

Foundations of Clinical Care (year 2) includes rotation through six discipline-based clerkships that further medical knowledge and clinical skills. The Diagnostics & Therapeutics course runs throughout the phase to emphasize appropriate, cost-effective diagnostic evaluations for a defined set of presenting problems.

The Immersion Phase (years 3&4) is a highly individualized two-year period including courses that deliberately integrate basic science teaching in the clinical setting (ISC). Advanced clinical experiences (ACE) and acting internships (AI) foster increasing clinical responsibility. Advanced electives (AE) and Special Studies add to individualization.

Longitudinal programs are integrated with each phase: The Foundations of Healthcare Delivery focuses on health systems science. Each student is placed on a healthcare team in year one as a participant in that micro-system. During the clerkships, emphasis is on systems of care and patient safety. In the Immersion Phase, students complete a quality improvement project plus modules in advanced communication, population health, advocacy, and policy.

The Learning Communities are based in advisory colleges and address metacognition, medical ethics, medical humanities, healthcare policy, and leadership. Discussions draw upon actual clinical experiences to anchor this content in the real world.

The Research curriculum in FMK and FCC provides an introduction to research methodology and exposure to various research areas. A mandatory individual research experience of 3-6 months during the Immersion Phase promotes scholarly differentiation and grooms students for future leadership roles.

Curriculum 2.0 is a dynamic system with an emphasis on active, self-directed learning. The program utilizes multiple learning modalities: lecture, case-based learning, team-based learning, discussion, and labs - all intertwined with extensive clinical experience. This integration models the way students will continue to learn throughout their professional lives.

Curriculum 2.0 not only teaches students to be great physicians, but also provides skills to become life-long, self-sustaining learners. Students gain an outstanding knowledge base and learn how to access, interpret and apply evolving knowledge.

Designed to improve patient outcomes in the real world, our curriculum is driven by the competencies expected of a practicing physician. Our assessment program and innovative digital portfolio promote student development in the ACGME Core Competencies and the Core EPAs for Entering Residency.

Vanderbilt’s unique culture of support grooms learners for leadership in medicine through strong mentorship, transparent data-driven self-assessment, and coach-supported personalized learning plans.

Assessment in Curriculum 2.0 Students are assessed in all ACGME competency domains throughout all phases.

Targeted personalized coaching plans address areas needing further development.

Grading: Courses in the FMK and FCC Phases, including the core clinical clerkships in year 2, are graded on a pass/fail basis. This is intended to create a culture in which early students can openly address their development in all competency domains.

Discriminatory grades (honors, high pass, pass) are awarded only in the Immersion Phase. Immersion courses have been designed to promote academic rigor; attaining “high pass” or “honors” in this phase demands excellence across multiple competency domains.

As I watched my best friend fall from 15 feet in the air, there was not a single thought in my mind, onlyinstinct. She landed forcefully in my arms, I sat her down with a smile, and we finished the rest of thewarm-up. In an hour we were competing in my last cheer nationals competition, and that catch left bothof my index fingers fractured and incredibly swollen. Walking away from the competition wasn’t anoption, so I buddy-taped my fingers and let my adrenaline carry me (and my partner) through theperformance. Afterwards, my partner hugged me and said “I should probably stop breaking your fingersif you ever want to be a surgeon someday.”

Ironically, I entered my clerkship year thinking that surgery was the last thing that I would choose. Ischeduled my surgery rotation first to get it out of the way, and I started with neurosurgery. Theneurosurgery cases were crowded, and an older student suggested I scrub into the plastic surgery casesnext door – little did I know this would change my life’s course. The first case that I ever scrubbed intowas a free flap, and I distinctly remember seeing aspects of myself in how the surgeon focused on thedetails and symmetry. That case was the most fun that I had had in medical school, and I was excited toreturn to the department for an official rotation in the spring.

When I returned months later for my first plastic surgery elective, I was eager but incredibly nervous. Ihad not yet fallen in love with any other rotation of clerkship year, and this was the last one – the stakeswere high! I was pleasantly surprised as the rotation lived up to the imaginary bar that I had set in mymind. I loved how every case was unique, and many felt like an art project. I enjoyed watching thesurgeons apply the artistic and spatial creativity that seems unique to plastic surgery. As I was exposedmore to the field, I began to appreciate how many populations were impacted by the work of plasticsurgeons. I knew that I had found the right field when I saw the reaction of my first transgender patientwhen he removed the binder after top surgery.

I believe there is no other field that so beautifully melds medicine, creativity, artistry, and service. I can’timagine myself pursuing anything else. I look forward to the late nights of call and the long but satisfyingdays of residency. I look forward to helping patients feel as if they identify with the person that they seein the mirror. I am thankful for the older student who suggested I go next door, and to my many mentorswho have opened this path for me.

INTERPRETATION OF RESULTS

USMLE transcripts include a complete examination history. On those Step examinations for which numeric scores are reported, a three-digit scale is used. Most scores fall between 140 and 260 on this scale. The recommended minimum passing score is shown on the front of the transcript next to theexaminees score for each administration along with a pass/fail outcome. Test results reported as passing represent an exam score of 75 or higher on atwo-digit scoring scale. The level of proficiency required to meet the recommended minimum passing level for each USMLE Step is reviewed periodicallyand is subject to change. Such changes do not alter pass/fail outcomes from prior test administrations.

For examinations with reported scores, the Standard Error of Measurement (SEM) provides an index of the variation that would be expected to occur if anexaminee were tested repeatedly using different sets of items covering similar content. The SEM is usually in the range of 4 to 8 points.
