AGENDA FOR BOARD MEETING

The purpose of this Board is to control and regulate the licensure of all physical therapists and physical
therapist’s assistants within the State of Nevada to ensure public protection. Items relating to the function of
this Board can be placed on the Agenda by writing this Board.
Items on the agenda may be taken out of order.
The Board may combine two or more agenda items for consideration.
The Board may remove an item from the agenda or delay discussion relating to an item on the agenda at any time.
Public Comment Procedure: 1. Public Comment will be taken at the beginning and at the end of each Board meeting; 2. Public Comment may also be taken at other such times as requested so long as the request
that Public Comment be taken will not interrupt ongoing Board business; 3. Those individuals wishing to present concerns to the Board are to sign in prior to speaking; 4. Depending on the number of individuals wishing to address the Board, a reasonable time
limit may be set. The Board will not restrict comments based upon viewpoint; 5. No action may be taken upon a matter raised during Public Comment until the matter itself
has been specifically included on an agenda as an item upon which action may be taken.

Item 4- Licensee Steven Bolick appears due to the results of the recent fingerprinting report which was required as part of the probationary license. (Action). Possible closed session for the Board to discuss the character, professional competence

Item 5- Licensee Ralph Paul Clemmons appears to request release from probation.
(Action). Possible closed session for the Board to discuss the character,

Item 6- Applicant Vincent K. Edwards requests to take the National Physical Therapy Examination for Physical Therapist a fifth time in accordance with NRS 640.140.

person as allowed under NRS 241.030. Item 7- Applicant Neil Andre Deles Palma requests to take the National Physical Therapy
