New Medication Forms in 2017 There have been changes to Education QLD medication forms. The ‘Request to administer medications’ form and the Medication Sign-off sheet will no longer be in use – they have been replaced. The requirements of these forms have been merged into one format, but there will be 2 new forms that use this format, to account for the different procedural requirements between administering routine/ occasional and emergency medication. There are now 2 new forms: one for routine or short term medications one for emergency medications The 2 new forms have similarities (for example, they both have a page of instructions which look similar) but are tailored to meet the different processes between medications that are taken routinely/ occasionally and those administered in an emergency. The parent request and recording of medication has been merged onto one page, so that the parent authorisation, dosage requirements and record of administration are visible at the same time. This should help reduce the risk of medication errors. 1

New Medication Forms in 2017There have been changes to Education QLD medication forms.

The new form below is the one that is used on a daily basis to administer medications. It is for use with prescribed medications administered on a routine or daily/regular basis, or for short-term medications.For example, this form will be used for anti-convulsive medication given every day at lunchtime, a short dose of anti-biotics or any medication where there is a set time to administer the dosage.

The other new form is for the administration of emergency medications. This is for medications such as Midazolam, Adrenaline auto-injectors (Epipen) or Ventolin that have been prescribed to the student and used to respond to a medical emergency.

This form is used in conjunction with the student’s Emergency Health Plan, Doctor’s orders or Action Plan.

This form is designed to record the administration of a medication to a student during school camps.

It is the responsibility of the parent to complete the highlighted sections of this form.A Webster Pack is still required.

This form is NOT for recording the administration of emergency medication. The administration of emergency medication record sheet is for this purpose.
